2 SUMMARY This annual report is in response to requirements under amended Certificate of Approval to Operate # SUPTV dated April 24, 2012 for the Greenway Pollution Control Centre. GREENWAY POLLUTION CONTROL CENTRE a) A summary and interpretation of all monitoring data and a comparison to the effluent limits outlined in Condition 7, including an overview of the success and adequacy of the Works; The monitoring data is included in the tables and graphs of the report. There were no monthly average concentrations or loading compliance exceedences in Phosphorous levels in 2012 averaged 0.33 mg/l. The annual average flow of 115 ML/day was within the limit of ML/day. The average daily plant peak flow was 187 ML/D in The ph of the effluent ranged from 7.1 to 8.1 which are within the required range of 6.0 to 9.5. The monthly average dissolved oxygen levels were greater than the minimum required in The Greenway PCC Environmental Assessment for an expansion has been completed. The Multiple Hearth Incinerator removal was completed. This area is going to be used by the centrifuges. The selection of Greenway Expansion consulting team and pre-design and detailed design was initiated in May Construction of the Biosolids Dewatering System (centrifuges, sludge pumps) is underway. A second Rotating Drum Thickener for waste activated sludge thickening was ordered in January The goals of a Pollution Prevention and Control Plan (PPCP) are to outline the nature, cause and extent of pollution problems, examine alternatives and propose remedial measures for reducing and mitigating pollution from sewer overflows and then recommend an implementation program for achieving those goals. The City of London has contracted engineering consulting firm CH2M HILL Canada Limited in 2012 to develop the plan through a detailed analysis of the impacts of rainfall events on our existing sewer systems and the Thames River. The existing and proposed changes in the works are sufficient to meet the requirements of the Certificate of Approval to Operate. b) A description of any operating problems encountered and corrective action taken; On January 13, 2012, the clarifier mechanism for three section number four final collector mechanism failed. This clarifier handles 40 percent of the section three flows or roughly 36 Megalitres per day. This represents a substantial portion of the total flow treated at the Greenway plant. The Ministry of Environment was immediately notified of the failure and the emergency repair process started Monday January 16. Three suppliers were contacted regarding the supply of drawings and guidance on the repairs and Yarmouth Crane and Rigging was brought in to assess the equipment needed to disassemble and repair the mechanism; the tank is not close to paved access and a large crane would be needed. Removal of the damaged mechanism started January 23, 2012, reinstallation began February 14, 2012 and the clarifier was returned to service February 24, 2012.

3 All clarifier mechanisms in use at the City s Wastewater Treatment Plants receive annual preventative maintenance (PM) inspections. This failure was attributable to a faulty torque limiting switch in combination with a structural failure and excessive wear on previously unidentified wear blocks. The PM inspections will now include closer scrutiny of these items. Fouling and transmittance issues with the UV system yielded poorer than expected disinfection in June and July. Alum was used in August as ferric was suspected as an underlying problem. Trojan inspected the wiper system but found no issues with the wipers. Transmittance issues continued with the alum and the dosage of UV was increased in September. c) A summary of all maintenance carried out on any major structure, equipment, apparatus, mechanism or thing forming part of the Works; A Work Order list for the Greenway Pollution Control Centre is attached. d) A summary of any effluent quality assurance or control measures undertaken in the reporting period; A Plant and effluent monitoring program was conducted and the data is in tables of this report. Samples were obtained as required in the Certificate of Approval. The ph samples were done on grab samples once a week and the composite samples the rest of the time. The UV channel analysis includes bypasses of the screen room and aeration section due to high flows during rainfall/snow melt events. Analyses are based on 24 hr. samples for the Greenway Pollution Control Centre with the exceptions of E. Coli, temperature and Dissolved oxygen which are grab samples. The ph samples were done once a week on grab samples beginning the week of January 23 rd and the rest of the results were based on composites. Grab samples for ph were missed in the week of August 20 th and the week of November 20 th, Composite sample phs were obtained during these weeks. e) A summary of the calibration and maintenance carried out on all effluent monitoring equipment; Calibration information is in the report. f) A description of efforts made and results achieved in meeting the effluent Objectives of Condition 6; The plant met the monthly average concentration and loading objectives for BOD, suspended solids and Un-ionized Ammonia 100% of the time in The minimum Dissolved Oxygen monthly average concentration objective was achieved 100% of the time in Monthly Geometric means for E. Coli in Greenway's effluent in 2012 disinfection period were achieved 66% of the time. As noted above, Alum was used in August as we suspected ferric was an underlying problem.

4 g) A tabulation of the volume of sludge generated in the reporting period, an outline of anticipated volumes to be generated in the next reporting period and a summary of the locations to where sludge was disposed; Sludge wasted in 2012 from the aeration sections was 685,000 cubic metres with average solids of 0.68%. The primary sludge generated in 2012 had a volume of 189,753 cubic metres with average solids of 3.5%. The Greenway activated sludge is thickened in dissolved air flotation units or Rotating Drum thickeners. Sludge from the other City of London Wastewater Treatment Plants is hauled to Greenway. In 2012 we anticipate generating 800,000 cubic metres of aeration sludge and 200,000 cubic metres of primary sludge at the Greenway PCC. In 2012, the press cake was incinerated or processed in the Bioset process and hauled to the W12A landfill. In 2013 we plan to incinerate or process the sludge in the Bioset process and haul to the W12A Landfill if the incinerator is not operating. The volume of sludge is included in tables in the report. h) A summary of any complaints received during the reporting period and any steps taken to address the complaints; There were 9 complaints received at Greenway in i. On March 19, 2012 there were two complaints due to odours. The wind was from the South Southeast. Any open doors on the sludge disposal building were closed. ii. On March 20, 2012 a complaint was due to odours. The wind was from the South Southeast. No odours were detected at the Greenway PCC and odours were detected on Riverside Drive but they were not plant odours. iii. On September 17, 2012 a complaint was due to odours. The odour complaint was likely due to a clean-up of our sludge cake hopper. The hopper had some sludge in it leftover from a recent bioset operation and it was being cleaned out. Due to construction there is a large opening on the side of the building. The building will be closed when construction is completed. iv. On October 9, 2012 there was a complaint about odours. The wind was from the South Southeast. All scrubbers were operating normally based on ORP readings. Due to construction there is a large opening on the side of the building and the large garage door was open. The building will be closed when construction is completed. A slight odour of sewage was noticed by the operator when he checked out the complainant s site. v. On October 17, 2012 there was an odour complaint. The wind was from the South Southeast. Section 1 aeration was being drained for maintenance which could have contributed to the odour. All scrubbers were operating normally based on ORP readings. The operator drove to the site and did not notice any sewage odour. vi. On November 5, 2012 there was an odour complaint. The wind was from the West Southwest. All scrubbers were operating normally based on ORP readings. The complaint mentions November 3 rd as strong smells. The November 3 rd Screen room scrubber ORP readings were 678 and 683, the Grit scrubber ORP readings were 758 and 692, the sludge building scrubber #3 ORP readings were 695 and 770 and the #4 scrubber ORP readings were 684 and 765. The Greenway scrubbers were inspected and found to be working properly.

5 vii. On November 22, 2012 there was an odour complaint. The wind was from the South Southwest. All scrubbers were operating normally. The complaint mentions that odours were noticed for the past six weeks and occurred mainly later in the day. Contractors were told to close openings in the sludge disposal building. viii. On December 24, 2012 there was an odour complaint. The wind was from the East Southeast from 8:00 to 11:000 am. December 24, All scrubbers were operating normally. When the operator went to the site no odours were detected. i) A summary of all By-pass, spill or abnormal discharge events. There were 6 by-pass events in 2012 at the Greenway Pollution Control Centre. Rainfall for 2012 was 660 mm compared to 931 mm for 2010 which was a more average year for rainfall. There were 6 secondary by-passs events (received primary treatment) and of the 6 events there were 3 that included raw sewagee bypasses. The total bypasses receiving primary treatmentt was ML. The total raw sewage bypassed was 0.86 ML. The raw by-pass was % of the volume receiving full treatment and the primary treated by-pass was 0.01% of the volume receiving full treatment. The bypassed primary sewage and raw sewage mixes with the treated flow priorr to discharge to the Thames River. The bypasss data is in the tables of the report. ASH: : The ash basins consist of a north and south unit and aree used on an alternating basis. When one unit becomes full, the ash is dewatered and removed by dragline and trucks. In August 2012 geotubes were started to be used to dewater the ash and the ash basins weree taken out of service. Geotubes containers are constructed of a specially- woven polypropylene material used as dewatering technology for agriculture, aquaculture, environmental remediation, pulp and paper, mining and mineral processing, power generation, and sludge and septage dewatering in water and wastewater treatment.

6 The geotube treatment process consists of filling the geotube container with slurry of ash. Polymer is introduced into the media to promote flocculation which allows the solids to bind and separate water more efficiently. The geotube container then dewaters the clear effluent commonly known as filtrate to section #3 aeration. Once the bag is filled to its recommended storage capacity the dewatered contents are left to dry further and then the ash can be removed. St. Mary s Cement is currently recycling the dewatered ash. The ash from the ash basins was hauled to the St. Mary s Cement in Total tonnes of ash removed were 3,105 tonnes.
