Provide a brief development history of the site.. Submit numbered and fully labeled mapddrawings to show the following: 0 the site boundary marked in red (sale of map 1 :,500); nearest sensitive reeptors (dwelling houses, et.) to the site, stating the distane (using an appropriately saled map); loation of designated storage area for raw materials (inluding disinfetants, hemials, fuels, et.) and waste (e.g., arasses, fluoresent tubes, domesti waste, et.) on site, indiating the materials stored at eah loation; 0 a drainage map larifying the routes of the surfae water drainage for all yard and roof drainage inluding proposed monitoring points; and 0 the loation of the well on site. 3. Confirm the main some of water supply to the installation. 4. Provide a brief desription of the on-site well (year of onstrution, depth of borehole, wellhead protetion measures, if any, et.,) and omment on the frequeny of usage. Provide details of the urrent monitoring frequeny of the well and group water sheme supplies. Provide laboratory analyses results for water extrated from the on-site well, if available. 2 5. Provide details of the liene or permit register number(s) for the omposting failities used for the reovery of poultry litter. Provide a letter of aeptane of the poultry litter generated at this unit from eah omposting faility. Alternatively, if poultry litter is sent off site for landspreading, please demonstrate adequate reovery apaity is available in aordane with EPA guidane provided in Marh 008 (detai I s attahed) Provide details of the quantity of wash water generated and the total usable area of land available to provide reovery apaity. Provide details of how you will omply with Artile 10 of S.I. No. 378 of 006 in relation to minimum storage for wash waters and hiken litter. Give details of annual oil and gas usage and typial quantities stored on site. Provide details of the maintenane shedule for heaters and the bak-up generator. Provide an assessment of how: 0 automati drinkindfeeding systems, and 0 ventilation systems, the poultry house design and maintenane; in the poultry houses ompare with the measures outlined as BAT for the Poultry Setor, in the Integrated Pollution Prevention and Control (IPPC) Referene Doument on Best Available Tehniques for Intensive Rearing of Poultry and Pigs (July 003). In addition to the above please also provide an updated non-tehnial summary to reflet the information provided in your reply. All mapddrawings must be no larger than A3 size and saled appropriately suh that they are learly legible. The requested information should be submitted to the Ageny within six weeks of the date of this notie, in order to allow the Ageny to proess and determine your appliation. 4 Information Required in an NMP INFORMATION REQUIRED IN A NUTRIENT MANAGEMENT PLAN (NMP) PREPARED UNDER THE REQUIREMENTS OF AN IP/IPP LICENCE OH LICENCE APPLICATION (1) Information to be submitted to the Ageny a) Nutrient Management Plans shall be prepared aording to Setion (3) below, using the format in Attahment 1 as a template for individual farms. The NMP inluding map(s) must be submitted to the Ageny for approval under the relevant ondition of the liene, or as part of the IPPC appliation proess. This information will be assessed by the Ageny and may be returned to the liensee/ appliant following suh assessment. If the liensee/appliant wants the NMP to be returned to them, it shall be submitted in a standalone envelope with a over letter stating that the information is provided in order to demonstrate adequate apaity for slurry reovery and that it shall be returned to the site one the assessment is ompleted. The following items must be submitted to the Ageny and will be put on publi file. b) Sign-off by a suitably qualified person (agriultural siene graduate or other suitably qualified professional) using the template speified in Attahment 3; ) Summary table or ustomer list using the template speified in Attahment ontaining Farmer Referene Code, ounty and townland(s) where slurry/manure may be spread, and total amount of slurry/manure that may be spread per farm (extrated from Nutrient Management Plan/ Fertiliser Plan); d) Total amount of slurry/manure to be produed at the liensed faility per year; e) Total amount of slurry/manure that may be applied to lands within the Nutrient Management Plan/ Fertiliser Plan; f) N and P ontent of slurry/manure produed by the liensed site. This setion also applies to any proposed additional lands to be added to the Customer List during 007 by an existing liensee. () Information to be retained on-site The liensee/appliant shall retain all the information speified in this doument on-site for inspetion by the Ageny or by any other authorised person or member of the publi. The liensee/appliant shall also keep on-site maps to a sale of- 1 : identifying eah parel of farmland inluded in the NMP, and the buffer zones appliable under the liene Shedule for existing lienes, or under S.J. 378 for liene appliants. These maps shall be available for inspetion by the Ageny or by any other authorised person or member of the publi. issued on 1 June 007

5 Information Required in an NMP Preparation of an NMP Nutrient Management Plans shall be prepared using up to date S.I. 378/Teagas/REPS fertiliser appliation rates, and the liensee must speify whih is used. The NMP must show the farmer s name and address, agronomi requirement of eah parel of farmland for nutrients, the amount of pig slurry that may be applied to satisfy these requirements, together with the total amount of nutrients and the total amount of pig slurry supplied from the liensed site. Attahment 1 shows the Table 1 1 B(ii) for NMP alulations, whih shall be used as a format for eah farm. All setions of the Table shall be ompleted with the information as speified. Map(s) to a sale of 1:50,000 shall be inluded, identifying eah parel of farmland inluded in the NMP prepared under point 3(a) above. Soil P tests shall be ompleted every three years and the results shall be used to verify the fertiliser requirements in the Plan, as required under existing lienes, P Index 3 may be assumed for new lands in the first season of their proposal for use by an IPPC appliant or for additional lands proposed by an existing liensee. Lands shall be assessed for suitability and unsuitable lands exluded based on the following. Condiiions of an individual IPC/IPPC liene; The provisions of the European Communities (Good Agriultural Pratie for Protetion of Waters) Regulations, 006 (S. I. No. 378 of 006); Land that is subjet to any restrition by virtue of its i.nlusion in a designated area, e.g. NHA, SAC or Protetion Zone for a publi water supply. Chek with the Heritage Setion of the Department of Environment, Heritage & Loal Government, and/or the Loal Authority, as relevant, for designated sites/protetion zones within the spreadlands; Sand or gravel soivsubsoi1, or shallow limestone soils. Regard shall be had in the preparation of this setion to Geologial Survey of Ireland publiations (e.g. Groundwater Protetion Shemes), to Teagas reords and publiations on soil types, and to the Ageny s guidane on landspreading of organi wastes. Persons signing the assessment form speified in Attahment 3 of this doument must be able to demonstrate that the lands submitted have been appropriately heked against available data. In partiular they must walk the land and mark the speitied features on a map(s). Copies of the map(s) with all the buffer zones and exluded areas marked in shall be maintained on-site for inspetion and shall be provided to the person spreading the manure/nutrients so that they are aware of any suh restritions. 8 Information Required in an NMP Attahment 3 Sign-off of lands, to be ompleted by suitably qualified person I have ompleted Fertiliser Plans (Nutrient Management Plans) for farm referenes XX to reeive slurry from IPC liensed site [insert name], Register Number POXXX-XX during 00X. These Fertiliser Plans omply with the requirements of the European Communities (Good Agriultural Pratie for Protetion of Waters) Regulations, 006 (S. I. No. 378 of 006). Summary information on the Fertiliser Plans is attahed and the omplete Fertiliser Plan as speified in Ageny orrespondene of 0th June 007 is available on-site for inspetion by any relevant Authority. In my professional opinion, animal manurednutrients may be used to fertilise any of those farmlands, and the appliation of pig rnanure/nutrients from the liensable site to any of those lands in a quantity identified in the Fertiliser Plan and manner that omplies with the requirements of the Code of Pratie and Buffer Zones of IPC liene Reg. No. POXXX- XX, and European Communities (Good Agriultural Pratie for Protetion of Waters) Regulations, 006 (S. I.
