1 Public Advisry Grup (PAG) Meeting Minutes April 24, 2014 The meeting started at 5:05 p.m. at the Sayler Park Cmmunity Center. Facilitatr: Deb Lenard, Envirnmental Quality Management, Inc. 2 Meeting: Facilitatr Deb Lenard welcmed the grup and asked everyne t intrduce themselves. 1) Plant Safety & Envirnmental Perfrmance Update Brad Fattlar, Envirnmental Engineer Safety Perfrmance OSHA Recrdable Injuries - The plant has nt experienced any OSHA recrdable injuries since July The plant s Ttal Recrdable Injury Rate (TRIR) is currently 0.6 (end f April 2014), which puts the plant in the tp 10% f petrchemical sites in the wrld. - Eric Cassisa, Site Manager, stated that he is very prud f the plant s safety perfrmance. When he first arrived, the TRIR was at 2 and the Addystn plant was amng the wrst perfrming f the INEOS sites. - If there are n injuries by the end f July 2014, the plant s TRIR will be at 0. First Aid Incidents - The plant has experienced 4 first aid injuries since the last PAG meeting n February 27, 2014: 3/2/14: Technician was struck by a rubber bale, which caused a hyperextensin f the knee 3/4/14: Abrasin n electrician s finger 4/22/14: Laceratin n cmpunding technician s finger 4/24/14: Laceratin n tank farm technician s thumb Envirnmental Perfrmance Malfunctins f Air Pllutin Cntrl Devices - T date in 2014, there have been 13 malfunctins, ttaling 331 punds f chemicals released t the atmsphere. - Since the last PAG meeting in February, 3 equipment malfunctins have ccurred. Warmer weather has helped in reducing the number f malfunctins. 3/3/14 Pwer blip caused a trip n the main duct (piping) t the biler (air pllutin cntrl device), releasing abut 3.32 punds f vlatile rganic chemicals (VOCs) t the atmsphere. 3/5/14 A high Lwer Explsive Limit (LEL) detectin caused emissins frm the Scripset prcess t divert frm the duct (piping) t the thermal xidizer (air pllutin cntrl device). Abut 1 pund f chemicals was released int the atmsphere. An LEL is the lwest cncentratin f a gas capable f prducing a fire/explsin if an ignitin surce was present. LEL sensrs are lcated 2

3 Spills t the Ohi River within the ducts leading t the thermal xidizer and biler. If the sensrs detect high cncentratins f chemicals (that exceed the LEL), a valve pens t divert the chemicals t the atmsphere instead. This prtects the biler and thermal xidizer frm fire/explsin. In this case, the LEL sensr was nt accurately reading chemical cncentratins, mainly due t the mixture f gas and water in the duct system. The plant is currently cnducting a study n hw t imprve the reliability f this system. 3/17/14 Emissins frm the Scripset prcess diverted frm the duct t the thermal xidizer due t a technician errr. Less than 1 pund f chemicals was released t the atmsphere. - The plant has been experiencing issues with chemical releases t the Ohi River, mainly related t its sewer system. Since the last PAG meeting, there has been tw incidents: 3/14/14 A dirty sewer (that discharges t the plant s n-site wastewater treatment plant) leaked thrugh a hle int a clean water sewer that discharges t the Ohi River. The hle was plugged, and the incident was reprted t the Ohi EPA. 4/7/14 An il sheen was fund n the Ohi River. The spill ccurred when a few milliliters f residual fuel il in a clean water sewer were flushed thrugh t the river fllwing plant washing activities. The il was unable t be skimmed frm the sewer s catch basin because the basin was under water due t high river levels. The fuel il was inadvertently spilled last December and thught t have been captured by the catch basin. All il sheens must be reprted t the Ohi EPA. - The plant has received a Ntice f Vilatin due t recurring spills int the river. A Cmpliance Attainment Meeting was scheduled with the Ohi EPA t discuss these issues. INEOS is implementing new plicies, such as sandbagging sewers, and is ging t trace the sewers arund the plant. Cmmunity Feedback - T date in 2014, there have been 7 cmmunity cmplaints: 5 fr chemical drs and 2 fr wastewater treatment plant (WWTP) drs. - Since the last PAG meeting in February, there were 6 cmplaints, 5 f which were linked t the plant: March 3 Tw calls came frm a cmplainant in Sayler Park. One cmplaint was linked t the plant cleaning a tank. The ther cmplaint was linked t the WWTP. March 7 A Miami Twnship resident called t reprt a burnt plastic smell. It was linked t the startup f a plant prcess. March 8 A call came in abut a sweet chemical smell. It was linked t the cleaning f a tank. March 18 A call came in frm Shady Lane. It was linked t the cleaning f the same tank. 3

4 Plant Alarms March 26 A call came in abut a skunk-like, garbage smell. The smell was determined t cme frm a skunk. - The plant has experienced the fllwing alarms since the last PAG meeting in February: March 3, 2014: Small mtr fire at rubber cnveyer pull bx. The Fire Brigade did nt respnd; the fire was put ut with nearby extinguishers. March 28, 2014: False alarm due t issues at the pull bx. March 24, 2014: Plant wide drill. Fire Brigade respnded. - The alarm is tested nce a mnth at 11 a.m. (first Wednesday f each mnth). Kibby Lane Mnitring Statin - The Suthwest Air Quality Agency, a divisin f the Hamiltn Cunty Department f Envirnmental Services, perates the Kibby Lane mnitring statin in Sayler Park. - The statin takes 24-hur cmpsite air samples every six days using SUMMA canister technlgy. The samples are analyzed fr abut 60 different chemicals, including butadiene, acrylnitrile and styrene, the three main chemicals used at the plant. The results are publicized nline at - The Kibby Lane mnitr takes lng-term samples fr chrnic risk analysis (lifetime expsure t chemicals). It des nt take real-time samples r measure acute risks. - The plant is reprting the average year t date (2014) cncentratins (in parts per billin) f acrylnitrile, butadiene, and styrene as cmpared t the three previus years. All cncentratins are well belw 1 ppb. Greenhuse Gas Emissins - Since 2010, INEOS has been reprting greenhuse gas emissins t the U.S. EPA. Greenhuse gas emissins are determined by lking at prductin data n hw much fuel the plant has burned fr the year. Using this infrmatin, the EPA estimates the site s emissins f carbn dixide, methane, and nitrus xide. Frm 2012 t 2013, there was an apprximately 63% reductin in greenhuse gas emissins frm the plant. This drp is due t the shutdwn f the cal biler and startup f the new natural gas-fired biler during the secnd half f The plant expects a bigger reductin in

5 Cnsent Decree - INEOS will meet with the U.S. EPA in early May t begin discussing clsure f the Cnsent Decree and incrpratin f envirnmental requirements int the site s Title V air permit, including the leak detectin and repair (LDAR) prgram and peratin f the plant s flare. 2) Citizen Cmplaints Mike Fair, Suthwest Ohi Air Quality Agency The agency received ne dr cmplaint t reprt drs n April 20, 21, and 22. 3) Plant Business News Eric Cassisa, Site Manager, INEOS ABS The plant is behind its financial targets fr the year. A large investment prgram is taking place t imprve plant peratins. The plant has been hiring new emplyees: 1 maintenance emplyee, 2 technicians, 1 maintenance manager, and 1 engineer have been hired. INEOS is lking t hire 2 mre technicians and 1 mre engineer by summer. 4) Other Business All the past newsletters and meeting minutes are n the website. 5) Cmmunity News Miami Twnship - Within the next six weeks, bids will be ut fr the new park behind the new schl. Miami Twnship will share layut fr the park at ur June 2014 PAG meeting. - The ld gas statin n the crner f Shady Lane and Bridgetwn was purchased. The prperty will be demlished, and grass will be planted. The twnship hpes t put additinal turn lanes in that area in the future. - The cunty sheriff n Shady Lane is spnsring a prescriptin drug take back prgram n Saturday frm 10 a.m. t 2 p.m. Addystn - The cmmunity is preparing fr its annual We Thrive celebratin. This year s celebratin will be a health fair n June 14 frm 11 a.m. t 3 p.m. - Rhnda Carter is a new Addystn cuncil member. - The cmmunity s Great American Cleanup is May 17 th frm 9 a.m. t 3 p.m. Three Rivers - The new stadium is underway, and it will be in use starting September. A nearby park will have baseball fields, sftball fields, and tennis curts. 5
