E. 19 (Environmental Protection Act) for approval of: Alterations to the subsurface disposal works for the collection, transmission, treatment and disposal of domestic sewage at the Youthdale Treatment Centres' property in the Unorganized Township of Lount. Sewage works on the property include: PROPOSED WORKS: A proposed on-site Sewage Works No. 8 rated at a maximum capacity of 9,000 litres per day (L/d) to service the Lodge located on Lot 33, and alterations to the existing Septic System No.5 to be rated at a maximum rated capacity of 75 L/d to service the Nursing Station located at the Youthdale Treatment Centre as follows: Proposed Sewage Works No. 8 (Lot 33) Proposed on-site sewage Works No. 8 rated at a maximum capacity of 9,000 litres per day (L/d) to service the Lodge (21 bedrooms, meal service three times per day for 106 people) consisting of the following: Oil and Grease Interceptor - One (1) oil/ grease interceptor rated at a minimum capacity of 76 L/min to be installed in the Lodge kitchen; Septic Tank - One (1) two compartment septic tank with a capacity of 9,000 L, collecting wastewater from the Lodge and discharging via gravity to a dosing chamber as described below; Dosing Chamber - One (1) dosing chamber with a capacity of 9,000 L located downstream of the septic tank equipped with a high level audible/visual alarm system and two (2) submersible effluent pumps (1/3 Hp), each rated at approximately 1.58 L/s at 3m TDH discharging to the MicroFast Treatment System, as described below;

2 MicroFAST Tertiary Treatment System - one (1) MicroFAST 3.0 treatment system, housed in one (1) underground precast concrete tank with a total capacity of 10,000 L/d, equipped with an external source of aeration, discharging treated effluent into a proposed Dosing Pumping Chamber as described below; Pumping Chamber - one (1) pumping chamber, equipped with a high level audible/visual alarm system and two (2) submersible effluent pumps (1/3 Hp), each pump rated at 0.9 L/s at 4.6 m TDH, discharging a dosing volume of 266 L via a 50 mm dia forcemain to two (2) distribution boxes and then to infiltration beds, as described below; Subsurface Disposal System - Two (2) infiltration beds, each with dimensions of 8.4 m x 5.6 m and an area of 49.9 m2. Each filter bed contains six rows at m long Infiltrator 36 chambers, evenly spaced over a minimum 750 mm deep approved filter sand, a minimum of 900 mm above rock or soil of T greater than 50 min/cm and least 0.5 m above the high groundwater level, complete with an extended minimum 250 mm deep sand mantle (T not greater than 15 min/cm) extending a minimum of 15 m beyond the outermost distribution pipes in any direction which the effluent will move laterally in the soil away from the filter bed, with a minimum clearance distance of 18 m from a well with watertight casing to a depth of 6 m (or minimum 30 m from any other well), minimum 35 m from the lake and minimum 8 m from the Lodge; Alterations to Septic System No. 5 (Lot 33) Alterations to Septic System 5 to service the Nursing Station to be rated at 75 L/d consisting of the following: - Disconnection of the Lodge from Septic System No. 5 (to be serviced by Sewage Works No. 8) and removal of the existing septic tank and pumping chamber; - - Existing tile field containing 232 m long 100 mm diameter perforated pipe consisting of 8 runs of 29 m long pipes to treat the grey water from the nurses station; PREVIOUS WORKS (Amended ECA Number CAK4Z issued October 16, 2013) LOT 30 (Youthdale Treatment Centres (Re-Entry)): Septic system to service Cabin #2, rated flow 3000 L/day A replacement septic system to service Cabin #2, a six bedroom residence with an approximate area of 210 m 2 located on Lot 30, with a rated capacity of 3,000 litres per day, consisting of: a) Septic Tank: an existing prefabricated double compartment concrete septic tank, 4,545 L in capacity; b) Treatment Unit: Discharge from the septic tank directed by gravity to a MicroFAST 0.9 Treatment Unit (rated capacity maximum 3,400 L/day daily sewage flow) installed in within a 4,500 L single chamber concrete tank; c) Filter Bed: Discharge from the treatment unit directed to a new filter bed via a distribution box levelled and set on a 24" x 24" concrete pad, directing flow to the bed of 38.1 m 2 filter surface and base contact area, via six runs of Infiltrator 36 chambers installed in lieu of stone in the filter bed, including a 75 mm dia. perforated pipe in the middle, evenly spaced over a surface of minimum 750 mm deep approved filter sand, a minimum of 0.9 m above rock or soil of T greater than 50 min/cm and at least 0.5 m above the high groundwater level, complete with an extended minimum 250 mm deep sand mantle (T not greater than 15 min/cm) extending a minimum of 15 m beyond the

3 outermost distribution pipes in any direction which the effluent will move laterally in the soil away from the filter bed, with a minimum clearance distance of 18 m from a well with watertight casing to a depth of 6 m (or minimum 30 m from any other well), minimum 35 m from the lake and minimum 8 m from the Cabin; PREVIOUS WORKS (Amended ECA Number QVR issued August 23, 2010) LOT 30 (Youthdale Treatment Centres (Re-Entry)): Septic system to service Cabin #1, rated flow 3000 L/day (approved by the Ministry August, 2010) : A replacement septic system to service Cabin #1, a six bedroom residence with an approximate area of 239 m 2 located on Lot 30, with a rated capacity of 3,000 litres per day, consisting of: a) Septic Tank: a prefabricated double compartment concrete septic tank, 6,135 L in capacity and equipped with an effluent filter at its outlet, discharging by gravity via a 100 mm dia. pipe to a small distribution box with a level concrete pad underneath to evenly dose the two proposed filter beds; b) Two aboveground filter beds, each with a filter surface area and a base contact area of m 2, consisting of 4 runs of 5 infiltrator chambers installed in lieu of stone in the filter bed, including a 75 mm dia. perforated pipe in the middle, evenly spaced over a surface of minimum 750 mm deep approved filter sand, a minimum of 0.9 m above rock or soil of T greater than 50 min/cm and at least 0.5 m above the high groundwater level, complete with an extended minimum 250 mm deep sand mantle (T not greater than 15 min/cm) extending a minimum of 15 m beyond the outermost distribution pipes in any direction which the effluent will move laterally in the soil away from the filter bed; Septic system to service Cabin # 3, rated flow 2,650 L/day (approved by the local Conservation Authority, August, 2004) : A septic system to service Cabin # 3, a five bedroom residence with an approximate area of 210 m 2 located on Lot 30, with a rated capacity of 2,650 litres per day, consisting of a prefabricated double compartment concrete septic tank, 7,570 L in capacity, discharging by gravity to a small distribution box to dose two filter beds, one approved in 2004 and the other already existing (installed 2001), with 2004 filter bed described as having a surface of the filter medium equalling 54 m 2 with a contact area of 126 m 2 and mantle loading area of 468 m 2, and the 2001 bed described as having a surface of the filter medium equalling 35 m 2 with a contact area of 125 m 2 and mantle loading area of 442 m 2. LOT 29: Septic system to service Schoolhouse, rated flow 1200 L/day (approved by the Ministry November, 2009) : A septic system to service a new Schoolhouse approximately 237 m 2 in area for 32 students and eight teaching staff located on Lot 29, with a rated capacity of 1,200 litres per day, consisting of: a) Septic Tank: a prefabricated double compartment concrete septic tank, 4,500 L in capacity and equipped with an effluent filter at its outlet, discharging by gravity via a 100 mm dia. pipe to a small distribution box with a level concrete pad underneath to evenly dose the filter bed; b) an above-ground filter bed with a filter surface area of 21.6 m 2, and a base contact area of 28.3 m 2, consisting of 3 lines of 6 m length of infiltrator chambers installed in lieu of stone in the filter bed, including a 75 mm dia. perforated pipe in the middle, evenly spaced over a surface of minimum 750 mm deep approved filter sand, a minimum of 0.9 m above rock or soil of T greater than 50 min/cm and at least 0.5 m above the high groundwater level, complete with an extended 250 mm deep sand mantle (T not greater than 15 min/cm) extending a minimum of 15 m beyond the outermost distribution pipes in any direction which the effluent will move laterally in the soil away from the filter bed; LOT 33: Septic system 1: servicing cabins 1 and 2, rated flow: Q= 2*425 = 850L/day : - one (1) 2,700 L

4 capacity septic tank, and a tile field containing 51 m long 100 mm diameter perforated pipes via a 100 mm diameter solid header gravity pipe from a distribution box and appurtenances; Septic system 2: servicing 20 people for cabins 3, 4 and 5: rated flow 2,250 L/day : - one (1) 4,500 L septic tank and a tile field containing 122 m (420 feet) long 100 mm diameter perforated pipes consisting of 7 runs of m (60 feet) long pipes via a 100 mm diameter solid header pipe from the septic tank, complete with distribution box, a pump chamber for cottage 3 with an one (1) 2,700 L septic tank, associated pipes and appurtenances; approved under user permit application number 1/LO/79, issued by the Ministry dated July 4, Septic system 3: servicing three brick dwellings numbered 6,7 & 8, rated flow 3*750 = 2,250 L/day : - one (1) 9,000 L (2500 gals) septic tank, and a tile field containing 183 m (600 feet) long 100 mm diameter perforated pipes/tiles consisting of 8 rums of m long pipes via a 100 mm diameter solid header pipe by a submersible pump complete with controls and high water alarm system from a manhole near the septic tank, complete with distribution box, associated pipes and appurtenances; approved under user permit application number 2/LO/79, issued by the Ministry dated July 16, Septic system 4: Triplex- equivalent to 3 one bedroom cottages: rated flow 2,250 L/day : - one (1) 4,500 L septic tank, and a tile field containing 128 m (420 feet) long 100 mm diameter perforated pipes/tiles consisting of 7 runs of m long pipes via a 100 mm diameter solid header pipe, a submersible pump complete with controls and high water alarm system in a manhole near the septic tank, complete with distribution box, associated pipes and appurtenances; Septic system 5: servicing the Lodge-10 bedroom, 2-3 PCs bath and a nursing 1 sink 1 dishwasher, Q = *300 = 3,875 L/day : - one (1) 9,000 L capacity septic tank, and a tile field containing 232 m long 100 mm diameter perforated pipe consisting of 8 runs of 29 m long pipes via a 100 mm diameter solid header pipe from a pump tank, complete with distribution box, associated forcemain and appurtenances; Septic system 6: servicing Shower Station, estimated lpcd, Q=5,000 L/day : - one (1) 9,000 L septic tank, and a tile field containing 300 m long, 100 mm diameter perforated pipes consisting of 10 runs of 30 m long pipes via a 100 mm diameter solid header pipe from a pump tank, complete with distribution box, associated forcemain and appurtenances; Septic system 7 to service cabin 9, rated flow 425 L/day (approved by the Ministry June, 2009) : - one (1) 3,600 L septic tank with an integral effluent filter and a 15 sq.m. filter bed comprising of a minimum 750 mm deep sand material with percolation rate T= 8 to 12 cm/min., topped with a minimum 300 mm thick layer of 19 mm diameter clear stone aggregate fed with effluent from the septic tank via three (3) runs of 4 m long 100 mm diameter perforated pipes and a 75 mm diameter solid gravity header pipe, complete with distribution box, associated piping, sand mantle and appurtenances, together with all other controls, electrical equipment, instrumentation, piping, pumps, valves and appurtenances essential for the proper operation of the aforementioned sewage Works; all in accordance with the following submitted supporting documents are in schedule A. For the purpose of this environmental compliance approval, the following definitions apply: "Approval" means this entire document and any schedules attached to it, and the application; "Average Daily Flow" means the cumulative total sewage flow to the sewage works during a calendar year divided by the number of days during which sewage was flowing to the sewage works that year;

5 "CBOD5" means five day carbonaceous (nitrification inhibited) biochemical oxygen demand measured in an unfiltered sample; "Director" means a person appointed by the Minister pursuant to section 5 of the EPA for the purposes of Part II.1 of the EPA; "District Manager" means the District Manager of the Sudbury District Office; "EPA" means the Environmental Protection Act, R.S.O. 1990, c.e.19, as amended; "Licensed Installer" means a person who holds a licence under Article of the Ontario Building Code. "Ministry" means the ministry of the government of Ontario responsible for the EPA and OWRA and includes all officials, employees or other persons acting on its behalf; "OBC" means the Ontario Building Code. "Owner" means Youthdale Treatment Centres and its successors and assignees; "OWRA" means the Ontario Water Resources Act, R.S.O. 1990, c.o40, as amended. "Previous Works" means those portions of the sewage works previously constructed and approved under an Approval; "Professional Engineer" means a person entitled to practice as a Professional Engineer in the Province of Ontario under a licence issued under the Professional Engineers Act. "Proposed Works" means the sewage works described in the Owner s application, this Approval, to the extent approved by this Approval; "Rated Capacity" means the Average Daily Flow for which the Works are approved to handle; "Works" means the sewage works described in the Owner's application, and this Approval, and includes both Proposed Works and Previous Works. You are hereby notified that this environmental compliance approval is issued to you subject to the terms and conditions outlined below: TERMS AND CONDITIONS 1. GENERAL PROVISIONS (1) The Owner shall ensure that any person authorized to carry out work on or operate any aspect of the Works is notified of this Approval and the conditions herein and shall take all reasonable measures to ensure any such person complies with the same. (2) Except as otherwise provided by these conditions, the Owner shall design, build, install, operate and maintain the Works in accordance with the description given in this Approval, and the application for approval of the Works. (3) Where there is a conflict between a provision of any document in the schedule referred to in this Approval and the conditions of this Approval, the Conditions in this Approval shall take precedence,

6 and where there is a conflict between the documents in the schedule, the document bearing the most recent date shall prevail. (4) Where there is a conflict between the documents listed in the Schedule submitted documents, and the application, the application shall take precedence unless it is clear that the purpose of the document was to amend the application. (5) The Conditions of this Approval are severable. If any Condition of this Approval,or the application of any requirement of this Approval to any circumstance, is held invalid or unenforceable, the application of such condition to other circumstances and the remainder of this Approval shall not be affected thereby. 2. EXPIRY OF APPROVAL This Approval will cease to apply to those parts of the Works which have not been constructed within five (5) years of the date of this Approval. 3. CHANGE OF OWNER (1) The Owner shall notify the District Manager and the Director, in writing, of any of the following changes within thirty (30) days of the change occurring: (a) change of Owner; (b) change of address of the Owner; (c) change of partners where the Owner is or at any time becomes a partnership, and a copy of the most recent declaration filed under the Business Names Act, R.S.O. 1990, c.b17 shall be included in the notification to the District Manager; (d) change of name of the corporation where the Owner is or at any time becomes a corporation, and a copy of the most current information filed under the Corporations Informations Act, R.S.O. 1990, c. C39 shall be included in the notification to the District Manager; (2) In the event of any change in ownership of the Works,other than a change to a successor municipality, the Owner shall notify in writing the succeeding owner of the existence of this Approval,and a copy of such notice shall be forwarded to the District Manager and the Director. 4. CONSTRUCTION (1) The Owner shall ensure that the construction of the works is supervised by a licensed installer or a Professional Engineer, as defined in the Professional Engineers Act. (2) Upon construction of the works, the Owner shall prepare a statement, certified by a licensed installer or a Professional Engineer, that the Works are constructed in accordance with this Approval,and upon request, shall make the written statement available for inspection by Ministry staff and staff of the local municipality. 5. OPERATIONS, MAINTENANCE, MONITORING AND RECORDING (1) The Owner shall sign a Service and Maintenance Agreement with the manufacturer or approved

7 agent of the MicroFAST 0.9 and 3.0 treatment systems. The maintenance agreement must be retained at the site, kept current and made available for inspection by the Ministry staff. (2) The Owner shall receive from the manufacturer or distributor of MicroFAST printed literature that describes the unit in detail and provides complete instructions regarding the operation, servicing, and maintenance requirements of the unit and its related components necessary to ensure the continued proper operation in accordance with the original design and specifications. (3) The Owner shall ensure that the treatment system is at minimum inspected annually by the MicroFAST authorized personnel, and maintained according to the manufacturer's recommendations including minimal yearly effluent sampling for CBOD 5 and Total Suspended Solids to ensure that it meets design objectives of 10 mg/l for both CBOD 5 and Total Suspended Solids in a grab effluent sample before discharge to the filter bed; (4) The Owner shall ensure that all septic tanks on the property are pumped out every 3-5 years or more often if required and the effluent filters are cleaned out at minimum once a year (or more often if required). (5) The Owner shall ensure that grass-cutting is maintained regularly over all the subsurface disposal beds on the property, and the drainage operations in the beds are visually observed on a monthly (once every month) basis for any breakouts. In the event a breakout is observed from any subsurface disposal bed, the Owner shall ensure that the sewage discharge to the bed is discontinued and the incident immediately reported verbally to the District Manager,followed by a written report within one (1) week. The Owner shall ensure that during the time remedial actions are taking place the sewage generated at the site shall not be allowed to discharge to a surface water body or to the environment, and safely collected and disposed off through a licensed waste hauler to an approved waste disposal site. (6) The Owner shall maintain a logbook to record the results of Operation and Maintenance activities specified in the above subclauses, and shall keep the logbook at the site and make it available for inspection by the Ministry staff. (7) The Owner shall retain for a minimum of three (3) years from the date of their creation, all records and information related to or resulting from the monitoring activities required by this Certificate. The reasons for the imposition of these terms and conditions are as follows: 1. Condition 1 is imposed to ensure that the Works are built and operated in the manner in which they were described for review and upon which approval was granted. This condition is also included to emphasize the precedence of Conditions in the Approval and the practice that the Approval is based on the most current document, if several conflicting documents are submitted for review. The condition also advises the Owners their responsibility to notify any person they authorized to carry out work pursuant to this Approval the existence of this Approval. 2. Condition 2 is included to ensure that, when the Works are constructed, the Works will meet the standards that apply at the time of construction to ensure the ongoing protection of the environment. 3. Condition 3 is included to ensure that the Ministry records are kept accurate and current with respect to the approved works and to ensure that subsequent owners of the Works are made aware of the Approval and continue to operate the Works in compliance with it. 4. Condition 4 is included to ensure that the Works are constructed, and may be operated and maintained such that the environment is protected and deterioration, loss, injury or damage to any

8 person or property is prevented. 5. Condition 5 is included to enable the Owner to evaluate and demonstrate the performance of the Works,on a continual basis, so that the Works are properly operated and maintained at a level which is consistent with the design objectives specified in the Approval and that the Works do not cause any impairment to the environment. 9 Upon issuance of the environmental compliance approval, I hereby revoke Approval No(s) CAK4Z issued on October 16, 2013 In accordance with Section 139 of the Environmental Protection Act, you may by written Notice served upon me and the Environmental Review Tribunal within 15 days after receipt of this Notice, require a hearing by the Tribunal. Section 142 of the Environmental Protection Act provides that the Notice requiring the hearing shall state: 1. The portions of the environmental compliance approval or each term or condition in the environmental compliance approval in respect of which the hearing is required, and; 2. The grounds on which you intend to rely at the hearing in relation to each portion appealed. Pursuant to subsection 139(3) of the Environmental Protection Act, a hearing may not be required with respect to any terms and conditions in this environmental compliance approval, if the terms and conditions are substantially the same as those contained in an approval that is amended or revoked by this environmental compliance approval.
