PURPOSE To ensure that individuals trained in the management of hazardous and nonhazardous wastes at the Wyandotte Site have a full understanding of the process used to document waste determinations. 2. SCOPE Any trained employee tasked with the creation of a new waste stream at the Wyandotte Site. 3. INSTRUCTIONS: 3.1. Complete Sections A&B of the Waste Determination Profile identified below as provided in Attachment I Attach Material Safety Data Sheet(s) for waste constituents(s) Make a copy of the Waste Determination Profile for your own records Forward the original to OH-MI-CAN EHS Upon receiving the BASF Waste Determination Profile and MSDS from the generator the OH-MI-CAN HUB will review and complete remaining information A sample of the waste stream may be required for analytical information and/or waste stream approval at a treatment or disposal facility. Contact OH-MI-CAN HUB for sample requirements as they apply to your waste stream The completed Waste Determination Profile will be returned to the generator with the proper label text information on page 6 for completing and ordering of preprinted labels If you have any questions contact EHS. 4. SECTION A - GENERAL INFORMATION

(i.e. lbs, gal. kg) Indicate the frequency the waste is generated Shipping Information: Information to be completed by OH-MI-CAN HUB Indicate the type and size of container used for the waste. 5. SECTION B - PROPERTIES AND COMPOSITION OF THE WASTE 5.1. Physical Properties List color(s) associated with the waste material Identify any odor associated with the waste material Indicate the physical state of the waste material at room temperature and standard pressure, noting whether or not layering is applicable Indicate the relative viscosity of the waste material Enter the melting point and boiling point of the material in degrees Fahrenheit Enter the vapor pressure of the waste material Enter the specific gravity of the waste material Enter the density of the waste material in pounds per gallon or pounds per cubic yard Enter the heating value of the waste material in Btu/lb. or Btu/gal.

Indicate method used Indicate the ph of the waste material or enter the exact value Chemical Composition: Chemical name: Do not use trade names Enter specific percent or range, maximum column must be greater than or equal to 100% (include water content if applicable) CASRN: Chemical Abstracts Registry Number RQ: Reportable Quantity Enter if known Concentration Range. 6. SECTION C - REGULATORY DETERMINATION (TO BE COMPLETED BY EHS) 6.1. Solid Waste Determination Review the definition of Solid Waste and review Table 1, to determinate Solid Waste status Exclusions: A. Review the Recycle Chart 261.4(a) determine if material is exempt. B. If another specific exclusion applies not in the determination C. If material is not excluded check radio box.

Indicate if the waste is a process waste. B. Determine if the process waste is subject to an F or K listing Product Waste Determination A. Indicate if waste is a Commercial Chemical Product B. Determine if waste is subject to P or U listing Characteristic Waste Determination. A. Determine through review of the physical properties section if waste exhibits a Characteristic. B. Review process chemistry or submit for analysis waste to determine if waste exhibits Characteristic of Toxicity (D004- D043). C. Note all codes that are applicable Conclusions Document waste determination conclusions Attach relevant information such as MSDSs and analytical reports. 7. SECTION D - LAND DISPOSAL RESTRICTIONS: 7.1. In the Table provided list all applicable hazardous waste codes Review the Treatment Standards Table in and note all applicable Treatment Standards and or concentration limits If the waste is subject to the one-time notification under 268.7(a)(7) note here and provide reasoning Determine if waste is subject to Universal Treatment Standards If waste is subject to UTS list all known compounds from the Table in and the treatment concentration limit.

Provided in the Table List all constituents and or waste codes with a Federal Reportable Quantity If a mixture RQ applies calculate the mixture RQ 8.3. If a RCRA Default RQ applies (e.g. D001 RQ = 100 lbs.) list the RCRA RQ. 9. SECTION F - RCRA AIR EMISSIONS APPLICABILITY DETERMINATION 9.1. Determine through knowledge or analysis if the VOC concentration exceeds the following thresholds: Subpart BB greater than 10% Subpart CC greater than or equal to 500 ppmv Determine container or tanks compliance Level If the unit is a recycle unit document if VOC level is greater than 10 ppm Determine applicability for Subpart AA. 10. SECTION G - DOT SHIPPING INFORMATION Determine DOT shipping name and related information Determine appropriate package information. 11. SECTION H - MISCELLANEOUS INFORMATION Enter Transporter and TSD approval and contact information. 12. ADDITIONAL ANALYTICAL DATA Enter any additional analytical data supporting the waste determination.
