The elevations of the well were surveyed in August by the Bay County Surveying Crew (Please see Robert Zierden, PLS memo attached). The coordinates for the well were obtained with a Trimble Geo7X unit on 9/26/18. The well is located at N and N. The Top of Casing elevation is feet and the Ground elevation is 44.0 feet. BAY COUNTY SOLID WASTE SERVICES DEPARTMENT Landfill Road Panama City Beach, Florida Subject: Report of Monitoring Well Installation and Abandonment SteelField Landfill Monitoring Well Installations Bay County, Florida NOVA Project Number Dear Ms. McDonald: NOVA Engineering and Environmental, LLC (NOVA) has completed the monitoring well installation services for the subject project. Please find the attached Monitoring Well Construction Permit included in the Appendix. One (1) Standard Penetration Test boring (SPTB) was performed on March 21st, The soil test borings were performed using the guidelines of ASTM Designation D-1586, "Penetration Test and Split- Barrel Sampling of Soils". A mud rotary drilling process was used to advance the borings. At regular intervals, soil samples were obtained with a standard 1.4-inch I.D., 2.0-inch O.D., split-tube sampler. The sampler was first seated six inches and then driven an additional foot with blows of a 140-pound hammer falling 30 inches. The number of hammer blows required to drive the sampler the final foot is designated the "Penetration Resistance". The penetration resistance, when properly interpreted, is an index to the soil strength and density. Representative portions of the soil samples, obtained from the sampler, were placed in sealed containers and transported to our laboratory for further evaluation and laboratory testing. Test Boring Records show the standard penetration test (SPT) resistances, or N-values, and present the soil conditions encountered in the borings. These records represent our interpretation of the subsurface conditions based on the field exploration data, visual examination of the split-barrel samples, laboratory test data, and generally accepted geotechnical engineering practices. The stratification lines and depth designations represent approximate boundaries between various subsurface strata. Actual transitions between materials may be gradual. Laboratory tests results, including ten () grain size analysis and visual manual classifications, are presented in the appendix. All laboratory testing was performed in general accordance with current ASTM and industry standards. The subsurface soils encountered in borings B-1 generally consisted of loose to very dense fine-grained sands, silty fine-grain sands, clayey fine-grained and silty-clayey fine grained sands (USCS classifications of SP, SM, SC, SC-SM, respectively) to the SPTBs terminal depths of approximately 80 feet below existing grade. 4 Steelfield Landfill Monitoring Well Installations June 7, 2018 Bay County Solid Waste Services Department One (1) monitoring well was installed on April 23 rd 2018 utilizing FDEP approved methodology and F.A.C. requirements. The monitoring well at boring location MW-13 was installed to a depth of approximately fifty-five (55) feet below the existing ground surface. This well replaces the existing damaged monitoring well which was identified as MW-13 and was abandoned as part of our scope of work for this project. The monitoring well was installed at the location specified by Bay County personnel. See the attached Well Construction, Completion, and Abandonment reports for additional information.
