1 SEPA ENVIRONMENTAL CHECKLIST Purpose of checklist: Governmental agencies use this checklist to help determine whether the environmental impacts of your proposal are significant. This information is also helpful to determine if available avoidance, minimization or compensatory mitigation measures will address the probable significant impacts or if an environmental impact statement will be prepared to further analyze the proposal. Instructions for applicants: This environmental checklist asks you to describe some basic information about your proposal. Please answer each question accurately and carefully, to the best of your knowledge. You may need to consult with an agency specialist or private consultant for some questions. You may use not applicable or "does not apply" only when you can explain why it does not apply and not when the answer is unknown. You may also attach or incorporate by reference additional studies reports. Complete and accurate answers to these questions often avoid delays with the SEPA process as well as later in the decisionmaking process. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. Instructions for Lead Agencies: Please adjust the format of this template as needed. Additional information may be necessary to evaluate the existing environment, all interrelated aspects of the proposal and an analysis of adverse impacts. The checklist is considered the first but not necessarily the only source of information needed to make an adequate threshold determination. Once a threshold determination is made, the lead agency is responsible for the completeness and accuracy of the checklist and other supporting documents. Use of checklist for nonproject proposals: For nonproject proposals (such as ordinances, regulations, plans and programs), complete the applicable parts of sections A and B plus the SUPPLEMENTAL SHEET FOR NONPROJECT ACTIONS (part D). Please completely answer all questions that apply and note that the words "project," "applicant," and "property or site" should be read as "proposal," "proponent," and "affected geographic area," respectively. The lead agency may exclude (for nonprojects) questions in Part B - Environmental Elements that do not contribute meaningfully to the analysis of the proposal. The proposal is to completely replace the existing Mann Middle School buildings and site improvements in two phases. The first phase will include construction of as much infrastructure and site amenities as can be feasible with minimal to no disruption of the existing school and its programming. On the western half of the property one small building and site features will be demolished and the new 136,100 square foot school building and associated site amenities and utilities will be constructed. The second phase will demolish the existing 83,056 square foot school buildings, including portables and amenities, and construct the remaining site features and infrastructure to support the new school. 4 The completed project will contain a combined football/soccer field, baseball/softball field, three tennis courts, 189 vehicle and 18 full and small bus parking spaces. Staff and bus access will be provided from 116 th Street SW. Visitor and special education bus access will be provided from Holden Road SW. A new pedestrian connection will be provided from the east. Upon completion the new school will have a capacity of 1,000 students and will accommodate students from both Mann Middle School and Woodbrook Middle School. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. 5 B. Environmental Elements 1. Earth a. General description of the site: (circle one): Flat, rolling, hilly, steep slopes, mountainous, other The site is generally flat with minor slopes along the perimeter. b. What is the steepest slope on the site (approximate percent slope)? 2-5% c. What general types of soils are found on the site (for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any agricultural land of long-term commercial significance and whether the proposal results in removing any of these soils. According to a NRCS Web Soil Survey, most of the site is shown as Spanaway gravelly sandy loam. Much of the northern part of the site shows Alderwood gravelly sandy loam or Everett very gravelly sandy loam. A small area of the site, directly south of the off-site stormwater detention pond is listed as Dupont muck. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. The geotechnical study did not identify unstable soils. e. Describe the purpose, type, total area, and approximate quantities and total affected area of any filling, excavation, and grading proposed. Indicate source of fill. The site is relatively flat and generally slopes from the northwest corner of the site to the southwest corner. The proposed location of the building will be filled slightly to raise the proposed finished floor elevation above the existing grades. Proposed grades surrounding the building will be approximately 2-5%. During subsurface investigation of the site, low quality fill was encountered. It is possible to use this fill under fields and landscaping areas, but it will not be suitable as structural fill. This fill is estimated at 17,000 cubic yards. This fill will likely be capped with cement treated base and left in place to avoid the cost of importing or exporting additional soil. It is anticipated that there will be a net fill of 6,000 cubic yards for the entire project. This number is based solely on a comparison of the existing grade and finished grade elevations. It does not take into account existing or proposed surfacing depths, foundations, utility excavations, etc. f. Could erosion occur as a result of clearing, construction, or use? If so, generally describe. Erosion may occur during construction. A temporary erosion and sediment control plan will be prepared following the requirements of the City s Stormwater Manual. 6 g. About what percent of the site will be covered with impervious surfaces after project construction (for example, asphalt or buildings)? Upon completion approximately 57 percent of the site will be covered with impervious surfaces. h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: All construction stormwater will need to be maintained on the site per Washington State Department of Ecology standards. Erosion control measures are anticipated to include a rock construction entrance, filter fabric fence, storm inlet sediment projection, and if required a temporary sediment pond. Cover measures are also anticipated and include stockpile protection as well as temporary seeding and mulching. All construction equipment will be required to enter through a gravel construction exit. Other BMP s such as catch basin inlet protection will need to be placed on all existing stormwater catch basins and on proposed catch basins as they are constructed. Daily inspections of the site and maintenance of track off onto surrounding roads will be required throughout construction. 2. Air a. What types of emissions to the air would result from the proposal during construction, operation, and maintenance when the project is completed? If any, generally describe and give approximate quantities if known. Airborne particulates (dust) along with vehicular exhaust from construction equipment are the most likely short-term sources of emissions during construction activity. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. Aside from vehicular exhaust originating with traffic on the adjacent rights-of-way, no offsite sources of emissions and/or odors have been identified. c. Proposed measures to reduce or control emissions or other impacts to air, if any: Airborne particulates will be minimized through surface watering during the construction phase of the project. Construction emissions will be further reduced through emission equipment in an effort to fully comply with Washington State emission standards. 3. Water a. Surface Water: 1) Is there any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. Carp Lake is located directly to the southeast and a stormwater detention pond is located directly north of the site. Carp Lake is a man-made waterbody, not classified as a wetland or lake. 7 2) Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. Yes, the construction of the football/soccer field will occur within less than 100 feet of Carp Lake. 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. Not applicable. No fill or dredging material would be placed in or removed from adjacent surface waters. 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities if known. No 5) Does the proposal lie within a 100-year floodplain? If so, note location on the site plan. No 6) Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. No b. Ground Water: 1) Will groundwater be withdrawn from a well for drinking water or other purposes? If so, give a general description of the well, proposed uses and approximate quantities withdrawn from the well. Will water be discharged to groundwater? Give general description, purpose, and approximate quantities if known. No, the school uses municipal water. 2) Describe waste material that will be discharged into the ground from septic tanks or other sources, if any (for example: Domestic sewage; industrial, containing the following chemicals... ; agricultural; etc.). Describe the general size of the system, the number of such systems, the number of houses to be served (if applicable), or the number of animals or humans the system(s) are expected to serve. Not applicable. c. Water runoff (including stormwater): 1) Describe the source of runoff (including storm water) and method of collection and disposal, if any (include quantities, if known). 8 Stormwater runoff will meet the requirements of the Department of Ecology s 2014 Stormwater Management Manual for Western Washington as adopted by the City of Lakewood. All stormwater that runs off of pollution generating surfaces (pavement with vehicle traffic) will be treated for stormwater quality. Treatment will be provided by shallow bioretention facilities. The treatment facilities will be equipped with a type 1 catch basin with a beehive grate to handle runoff flows in excess of the required treatment flows. 2) Could waste materials enter ground or surface waters? If so, generally describe. The project will utilize infiltration trenches sized to store and infiltrate 100 percent of the runoff from the tributary basin, up to and including the 100-year, 24-hour design storm event. 3) Does the proposal alter or otherwise affect drainage patterns in the vicinity of the site? If so, describe. No. d. Proposed measures to reduce or control surface, ground, and runoff water, and drainage pattern impacts, if any: Onsite infiltration will be utilized to infiltrate 100% of the stormwater from the project site. As required by the stormwater manual and the City treatment of this stormwater will be provided prior to infiltrating. 4. Plants a. What kind and amount of vegetation will be removed or altered? The site is previously developed and contains a functioning middle school. Existing mature trees will be preserved and incorporated into the new landscape design. c. List threatened and endangered species known to be on or near the site. 9 d. Proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on the site, if any: The proposal includes perimeter and interior landscaping. The landscape for this school will be designed to achieve low water use and low maintenance requirements. An emphasis will be placed on using native plants and / or drought resistant ornamentals that have proven to be adapted and successful in the Puget Sound climate so that irrigation can be minimized after the plant establishment period. e. List all noxious weeds and invasive species known to be on or near the site. Unknown. 5. Animals a. List any birds and other animals which have been observed on or near the site or are known to be on or near the site. Examples include: birds: hawk, heron, eagle, songbirds, other: mammals: deer, bear, elk, beaver, other: fish: bass, salmon, trout, herring, shellfish, other b. List any threatened and endangered species known to be on or near the site. According to WDFW s PHS List, there are no threatened or endangered animal species known to exist on or near the site. c. Is the site part of a migration route? If so, explain. Pacific Flyway for migratory birds. d. Proposed measures to preserve or enhance wildlife, if any: No mitigation measures are required or proposed. e. List any invasive animal species known to be on or near the site. To the best of our knowledge, there are no invasive species known to be on or near the project site. 6. Energy and Natural Resources a. What kinds of energy (electric, natural gas, oil, wood stove, solar) will be used to meet the completed project's energy needs? Describe whether it will be used for heating, manufacturing, etc. Electricity and natural gas will be used for lighting and heating. b. 10 No impacts have been identified that limit or restrict the use of solar energy on adjacent properties. c. What kinds of energy conservation features are included in the plans of this proposal? List other proposed measures to reduce or control energy impacts, if any: We are using LED lights, daylighting, and high efficiency HVAC systems to save energy. 7. Environmental Health a. Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spill, or hazardous waste, that could occur as a result of this proposal? If so, describe. 1) Describe any known or possible contamination at the site from present or past uses. The presence of contamination in the existing building materials is currently being evaluated. There are no other known contamination sources on site. 2) Describe existing hazardous chemicals/conditions that might affect project development and design. This includes underground hazardous liquid and gas transmission pipelines located within the project area and in the vicinity. There are no known hazardous chemicals/conditions that affect development and design. There are no underground hazardous liquid or gas transmission pipelines in the vicinity. 3) Describe any toxic or hazardous chemicals that might be stored, used, or produced during the project's development or construction, or at any time during the operating life of the project. The only toxic or hazardous chemicals that could potentially be stored and used onsite will be those associated with the science/chemistry educational curriculum, which is carefully regulated by the state. 4) Describe special emergency services that might be required. No special emergency services are required beyond that which already are available to serve the existing facility. The school will follow State protocols for the handling of science program materials. 5) Proposed measures to reduce or control environmental health hazards, if any: If hazardous building materials are found to be present in the existing facility, abatement will occur following City and State regulations. The recommendations in the Hazardous Building Materials Report (being prepared) will be followed. b. Noise 1) What types of noise exist in the area which may affect your project (for example: traffic, equipment, operation, other)? 11 The only identified source of noise in the immediate vicinity occurs within the surrounding road network from vehicular traffic and military aircraft traffic associated with Joint Base Lewis McChord. 2) What types and levels of noise would be created by or associated with the project on a short-term or a long-term basis (for example: traffic, construction, operation, other)? Indicate what hours noise would come from the site. Short-term noise associated with construction activity will be limited to construction traffic and equipment. The maximum noise levels should be expected to occur within the range between 57 and 89 dba. These construction noise impacts will be limited by City Code to certain hours of the day (typically starting at 7:00 a.m. and ending in the early evening hours on weekdays). All construction activity associated with the project will be done in full compliance with City Code requirements. Long-term noise impacts are expected to be relatively minor and limited to typical schoolyard noise. Noise levels are not expected to exceed current ambient noise levels after project construction. 3) Proposed measures to reduce or control noise impacts, if any: Noise impacts associated with the construction phase of the project will be limited in duration. To mitigate general noise impacts during the construction phases, measures such as using and regularly maintaining efficient mufflers and quieting devices on all construction equipment and vehicles will be taken. Construction hours will roughly be limited to the normal workday, 7:00 a.m. to 6:00 p.m. 8. Land and Shoreline Use a. What is the current use of the site and adjacent properties? Will the proposal affect current land uses on nearby or adjacent properties? If so, describe. The current use of the site is Mann Middle School, adjacent properties on all four sides are residential. The proposal is to demolish and replace the existing school with a new middle school. The proposal will not affect adjacent land uses. b. Has the project site been used as working farmlands or working forest lands? If so, describe. How much agricultural or forest land of long-term commercial significance will be converted to other uses as a result of the proposal, if any? If resource lands have not been designated, how many acres in farmland or forest land tax status will be converted to nonfarm or nonforest use? The site is currently a middle school, to our knowledge the site has never been used as working farmlands. 1) Will the proposal affect or be affected by surrounding working farm or forest land normal business operations, such as oversize equipment access, the application of pesticides, tilling, and harvesting? Will any structures be demolished? If so, what? Yes, two portables will be demolished/removed in phase 1 and all of the remaining school buildings will be demolished in phase 2. e. What is the current zoning classification of the site? Public/Institutional Zoning District. f. What is the current comprehensive plan designation of the site? Public & Semi-Public Institutional g. If applicable, what is the current shoreline master program designation of the site? Not applicable, there are no regulated shorelines within 200 feet of the subject site. h. Has any part of the site been classified as a critical area by the city or county? If so, specify. The site has not been classified as a critical area. i. Approximately how many people would reside or work in the completed project? The reconstructed school will have a student capacity of 1,000. Although staffing has not been determined, it is estimated that there will be approximately 95 employees. j. Approximately how many people would the completed project displace? None k. Proposed measures to avoid or reduce displacement impacts, if any: Not Applicable L. Proposed measures to ensure the proposal is compatible with existing and projected land uses and plans, if any: The property is currently being used as a middle school. The proposal will expand the school and provide significant site improvements, including aesthetic improvements such as new landscaping and new building materials, improved fields for sports and play, etc. Separation of bus and vehicular traffic and longer onsite queue lanes will allow for efficient access and circulation and reduce impacts between the different modes of travel. 13 m. Proposed measures to reduce or control impacts to agricultural and forest lands of longterm commercial significance, if any: Not Applicable 9. Housing a. Approximately how many units would be provided, if any? Indicate whether high, middle, or low-income housing. None the proposal is to replace an existing school with a new school. b. Approximately how many units, if any, would be eliminated? Indicate whether high, middle, or low-income housing. None the proposal is to replace an existing school with a new school. c. Proposed measures to reduce or control housing impacts, if any: Not Applicable 10. Aesthetics a. What is the tallest height of any proposed structure(s), not including antennas; what is the principal exterior building material(s) proposed? 40 feet; Concrete Masonry Veneer will be the principal exterior material. b. What views in the immediate vicinity would be altered or obstructed? Views will change slightly as the school is reconstructed in a new footprint with new building and site improvements. c. Proposed measures to reduce or control aesthetic impacts, if any: None required. The new construction will be an enhancement from existing conditions. 11. Light and Glare a. What type of light or glare will the proposal produce? What time of day would it mainly occur? The new building interiors and exteriors will be lit as will the parking lots and grounds. The overall amount of light and glare will not be increased from current levels as a result of this project. b. Could light or glare from the finished project be a safety hazard or interfere with views? No. Lighting will be produced to enhance safety. It will be directed downward so as not to interfere with views or cause glare. c. What existing off-site sources of light or glare may affect your proposal? None known. d. 14 Lighting fixtures will be shielded and lighting will be cast downward to reduce light and glare impacts to adjacent properties. 12. Recreation a. What designated and informal recreational opportunities are in the immediate vicinity? Washington Park is approximately 1 mile from the site. b. Would the proposed project displace any existing recreational uses? If so, describe. No, however there may be temporary restrictions to the use of the outdoor recreation areas during construction. c. Proposed measures to reduce or control impacts on recreation, including recreation opportunities to be provided by the project or applicant, if any: None 13. Historic and cultural preservation a. Are there any buildings, structures, or sites, located on or near the site that are over 45 years old listed in or eligible for listing in national, state, or local preservation registers? If so, specifically describe. The existing school was originally constructed in According to the Washington Information System for Architectural and Archaeological Records Data (WISAARD) online mapping system, there are no buildings, structures, or sites eligible for preservation registers on the project site. b. Are there any landmarks, features, or other evidence of Indian or historic use or occupation? This may include human burials or old cemeteries. Are there any material evidence, artifacts, or areas of cultural importance on or near the site? Please list any professional studies conducted at the site to identify such resources. There is no evidence of Indian or historic landmarks, features, or occupation on or within the vicinity of project site. c. Describe the methods used to assess the potential impacts to cultural and historic resources on or near the project site. Examples include consultation with tribes and the department of archeology and historic preservation, archaeological surveys, historic maps, GIS data, etc. The Washington Information System for Architectural and Archaeological Records Data (WISAARD) online mapping system was utilized to assess potential impacts to cultural and historic resources on and near the proposal. d. Proposed measures to avoid, minimize, or compensate for loss, changes to, and disturbance to resources. Please include plans for the above and any permits that may be required. No disturbance to cultural or historical resources is expected. If any are discovered, the Washington State Department of Archaeology and Historic Preservation will be notified and appropriate measures will be taken. 15 14. Transportation a. Identify public streets and highways serving the site or affected geographic area and describe proposed access to the existing street system. Show on site plans, if any. Access is proposed via two existing driveways. One driveway is located along Holden Road SW and would accommodate school drop-off/pick-up and special education buses and one driveway along 116 th St SW, which would accommodate bus drop-off/pick-up. Access to the driveways is via either 112 th St SW to the north or Military Road SW to the west. b. Is the site or affected geographic area currently served by public transit? If so, generally describe. If not, what is the approximate distance to the nearest transit stop? Pierce Transit Route 214 has 2 stops within a quarter mile. c. How many additional parking spaces would the completed project or non-project proposal have? How many would the project or proposal eliminate? Mann Middle School currently has 86 striped parking spaces plus an additional parking area without striping. The completed project will have approximately 189 vehicle parking spaces and 14 full and 4 small size bus parking spaces. The Transpo Group s Transportation Impact Analysis (TIA) describes that a study of parking demand was completed for both schools and determined that the combined demand was for approximately 90 spaces. With the proposed capacity increase it is anticipated that the peak parking demand will be 98 spaces. The study concludes the proposed parking supply will accommodate the peak demand. d. 16 As shown in Table 7 of the TIA, the completed project is anticipated to generate approximately 211 net new weekday AM peak hour trips and 158 net new weekday PM school peak hour trips. The TIA anticipates 776 average vehicular trips per day. The trip generation is based on the following assumptions described in the traffic study: With completion of the proposed project enrollment is anticipated to be approximately 1,000 students. It is anticipated that not all Woodbrook Middle School students would be relocated to the Mann Middle School site. Additionally, because the schools have different trip generation rates a split in the anticipated enrollment was assumed similar to the current split between Mann and Woodbrook Middle Schools. Given the uncertainty of how school boundaries may change over time and the possibility that Woodbrook School students may no longer utilize bus service, it was assumed that some Woodbrook Middle School students would follow the higher trip generation profile of Mann Middle School. The future trip generation calculation is based on approximately 60 percent of the future student population following the Mann Middle School trip generation profile and 40 percent following the Woodbrook Middle School trip generation profile. g.
