1 FOR YOUTH INITIATIVE Job Posting POSITION TITLE: Case Lead, Employment-Youth in Transition DETAILS: Full Time Contract (with potential for yearly renewal) REPORTS TO: Manager, Youth in Transition SALARY: $42, START DATE: October 2018 Organization & Program Summary: For Youth Initiative is a non-profit organization that has served, inspired and advocated for thousands of teens and young adults in York South-Weston since Our multi-service agency provides year-round programs and services to local youth to meet their needs and empower them to reach their full potential. Every young person s journey is unique and their needs are too. Our case management model meets young people where they are and helps them get to where they want to be. Our programs and services are designed to support youth to graduate high school, enroll in post-secondary education, gain employment and prepare for adulthood. Youth in Transition - Employment serves teenagers and young adults up to age 29 as they make important decisions that will shape their adult lives. The program focuses on training and employment to equip participants with resources, expose them to new opportunities and support them in reaching their full potential. Case Leads work closely with participants to identify individual needs, goals and interests. Frequent group events include skill development workshops, career speaker series. Participants are connected to youth employment specialists, and can receive assistance with resumes, interviews and developing workplace skills. Position Summary: The Case Lead, Employment Youth in Transition (YiT) Program will support the Program Manager, Youth in Transition (YiT) to create a supportive network for youth in York South-Weston. In this role you will provide mentorship, opportunities and resources to nurture and build the capacity of youth and young adults between 14 and 29 years. The Case Lead, Employment must outreach to, recruit and support participants to achieve self-identified goals relevant to personal and professional development. Case management in collaboration with internal and external service providers/partner organizations is an essential part of this position to ensure that appropriate service delivery and timely supports are provided. Lastly, the Case Lead will complete the required monitoring and evaluation activities, work to improve programs based on feedback from youth, and support the Program Manager to author reports for our Board and funders.

2 Main Tasks & Responsibilities: Outreach (20%) Utilize innovative strategies for outreach, engaging in both traditional and creative methods to engage hard-to-reach youth Engage youth to identify issues, needs, barriers to accessing services in order to achieve goals and aspirations Provide youth with information, resources, contacts, referrals services and support as needed to facilitate access to programs, services and opportunities (i.e. employment, education, housing, health, including substance abuse, social abuse, social and emotional issues, etc.) Engage potential partner organizations, other service providers, community members and general public to promote the program, as well as develop resource and referral network Program Coordination (30%) Coordinate multiple components of YiT s program activities including monthly employment-based workshops and networking events with internal/external stakeholders, weekly resume clinics, on-site supports via external partners, paid training programs and case management Coordinate bi-weekly on-site supports with various external partners Identify and make referrals to internal and external supports Organize and mobilize participants to attend local networking events, career expos and job fairs with diverse community stakeholders and partners Support the maintenance and development of collaborative relationships, partnerships and networks among participants and community stakeholders, including employers, adult allies and peers, through a clear understanding of strengths and challenges Research opportunities and curate YiT Weekly eblast through MailChimp platform and send information to youth regarding upcoming job and training opportunities Support with supervising program spaces during FYI s program hours (3 to 6 pm) Case Management (40%) Conduct intake and needs assessment with youth to examine strengths and challenges Support participants to set self-identified goals to develop a comprehensive service plan Support and guide youth through necessary pre-employment steps including resume/cover letter writing, job searching, interview preparation, etc. Build and manage a healthy client caseload of active youth participants and provide ongoing one-on-one supports tailored to needs of each participant Ensure follow up on assigned tasks from service plans and continuously develop, re-evaluate and monitor participants progress on personal and professional goals Work collaboratively with internal/external stakeholders, service providers and

3 community partners with an aim of maximizing resources and increasing supports available to participants, to deliver effective case management plans that ensure that youth have access to programs, services and opportunities needed Identify and provide crisis resolution and solution-focused supports Facilitate connections between participants and community members Maintain comprehensive client files and case notes utilizing FYI s database and case management system Track and meet program outcomes (e.g. # of Youth Clients Employed, # of Youth On track to Employment etc.) Evaluation & Administrative Duties (10%) Implement and maintain strength-based and evidence-based professional standards of practice based on FYI s Program Manual and HR policy Work with stakeholders in developing and coordinating an evaluation plan to implement during check-ins with participants to provide feedback to ensure services are relevant, flexible and responsive, recognizing that recipients priorities and needs may change and evolve Input evaluation data into data management tools Provide weekly and monthly reports and updates on targets and progress to Manager Participate in staff and team meetings, trainings and professional development sessions Complete other necessary administrative work and documentation as required Required Qualifications: Education: Ideal candidate has a Bachelors or Masters in Social Work or relevant human services and social studies related education or equivalent employment experience Skills & Experience: Adept at coordinating programs and managing multiple priorities Experience working with youth in transition and/or youth at greater risk of being NEET (Not in Education, Employment or Training) Sound knowledge of current employment climate and understanding of labour market information Experience developing systems and processes to ensure the careful assessment, referral, and follow-up of complex youth cases Able to work from an outcome-based/evidence-based programming model Comfortable outreaching and building relationships/partnerships with community stakeholders, non-profit organizations and corporate entities A demonstrated ability to work in culturally diverse communities, with a particular understanding of the challenges facing youth from socio-economically marginalized communities Demonstrated understanding of strengths-based, client-centered, and

4 solution-focused approaches Has an understanding of community based, youth-led and youth-serving work, common service models in non-profit/social service sector Knowledge and experience working within an anti-oppression and anti-racism framework Ability to set priorities and manage deadlines with excellent administrative and time management skills Proficiency working with Google Suite (Gmail, Drive, Calendar, Docs, Sheets), client database software, etc, possessing a tech savvy attitude. Core Competencies: High level of professionalism when working with community partners (i.e. service providers, partner organizations, community members, etc.) Established high degree of good judgement, professional manner, empathy, and strong understanding and application of ethical principles Initiative and commitment to continuous learning Acceptance and use of constructive criticism for personal and professional growth Ability to work cooperatively with other professionals and community Capacity to work positively in a team environment Fantastic team player, with also a demonstrated ability to work independently with minimal supervision Physical Requirements: None Equipments Used: Experience and knowledge of using regular office equipments including computers, photocopiers, printers and fax Working Relationships: Works under supervision from the Program Manager, Youth in Transition (YiT) Will work with and support colleagues, including placement students, to complete tasks The position interacts with staff, supervisors, managers, volunteers, general public, partner organizations, corporate partners, and other organizations Location: This position works out of the 1652 Keele Street location, with some travel within York South-Weston required.

5 Working Conditions: Hours of work: Monday to Friday, 11:00am - 7:00pm, with some requirement to work extended evening hours and occasional weekends.
