2 About the role Journey Enterprise s Job Coaching & Life-Skills teams work together to support young adults/adults with additional needs to live and work independently. Through skills development and work experience placements we aim to enable clients to achieve real work outcomes, better health and active participation in their local communities. This role is funded initially for two years by urham County Council and covers the Local Authority area. The Job Coach will ensure clients with a learning disability develop the skills and confidence to travel, volunteer and achieve employment, reducing their need for day care and assisted transport provision. Job coaching is provided on a 1:1 basis. It is tailored to the skills, interests and aspirations of the client. Job Coaches work with local employers, training providers, voluntary sector partners and with Job Centres, to ensure clients secure appropriate volunteering placements and employment. The team is also able to support clients who may wish to explore enterprise initiatives. Work is delivered both from our operating sites and in a range of community settings across County urham. Working within a person-centred approach our County urham Job Coach will: Liaise with the Local Authority s Transitions & Learning isability teams to identify clients attending day services who would like to receive Job Coaching; Work collaboratively with Job Centre Plus & Connexions, Schools/Colleges and training providers and with local employers to access suitable workexperience and employment opportunities; Work collaboratively with Voluntary Sector, Local Authority and NHS partners to secure transition from day-care to volunteering & work placements; Advise on training, work placements and employment opportunities; Work with clients to develop skills, confidence and self-esteem; Prepare clients through job search, application and interview stages; Provide mentoring & support for clients who are volunteering; Provide in-work support for clients who have progressed to employment; Provide information and guidance on WP s in-work benefits and services to support clients and hosting employers (e.g. Universal Credit, Access to Work, isability Confident); Provide pre-employment & retention support for hosting employers;

The successful candidate will need to work as part of a person-centred team, who are professional, committed and have our clients' best interests at the centre of all they do. You will have the chance to develop and grow the service, using your creativity, people skills and good judgement. Job Coaches are the public face of Journey, representing our vision and values whilst working with community partners, local businesses, potential clients, their families and Care Managers. We are, therefore, looking for people that embody and promote our values. Core responsibilities 1. Provide evidence of clients progression through appropriate record keeping and outcomes measures. 2. To adhere to the National Occupational Standards for Supported Employment. 3. To recruit suitable candidates for work placements/ volunteering & employment through appropriate referral channels. 4. To develop a Person-Centred Profile and Individual Employment Plan, identifying a route into employment. 5. To work with the whole circle of support (family, professionals and colleagues) to raise aspirations and maximize the chances of success of any work placement or paid employment. 6. Working within a social model of disability, to champion the value to individuals, employers and wider society, of employing people with disabilities. 7. To work creatively with employers to enable work opportunities for people with disabilities. 8. To actively network with employers to increase the number and variety of placements and jobs we can offer. 9. To prepare candidates for interview, placements and employment by addressing individual barriers to work readiness. This may include travel training, confidence building etc. 10. To prepare and support the employer prior to the placement / job, and as required afterwards. 11. To undertake job analysis and risk assessments in the work place.

4 12. To train the person to succeed in the job using systematic instruction. 13. To evidence and record each individual s progress on their journey to employment. 14. To support people to travel independently wherever possible. 15. Work flexibly as required, during evenings and weekends if required, to support people into employment. 16. To provide ongoing support in the work place at regular agreed intervals. 17. To carry out other duties as required commensurate to the post. What you can bring to the role using your creativity & judgement We want staff to bring enthusiasm and use their unique skills, developed through their work and life, to support adults with learning difficulties achieve their aspirations. We are particularly interested in people who are outgoing and can sense opportunity. The people we support have a variety of employment aspirations and the Job Coach will need to work creatively to match appropriate opportunities which offer long-term outcomes. We are also interested in people who are passionate about closing the disability employment gap, and who can offer just enough support to enable people to be as independent as possible. 5 To undertake continuous professional development (CP), including attending appropriate training and appraisals to support best practice. To undertake occasional unsocial hours if required by the role. Any other job related duties as directed by your line manager that are commensurate with the role.
