2 GUIDELINES Response to this questionnaire shall be completed in English by a senior member of your company. The requirements of each question shall be addressed comprehensively with all supporting documentation provided in order for this Pre-Qualification to be deemed completed. 3 1.0 INTRODUCTION Oman LNG LLC intends shortly to issue a tender for Switchboard Inspection, Service & Testing of HV Circuit Breakers contract. Oman LNG LLC wishes to pre-qualify contractor capable of managing and executing this service. You are invited to submit pre-qualification documentation for the above mentioned service. Further details are given below. 1.0 BACKGROUND OMAN LNG LLC located in Qalhat in Sur, has its plant electrical power supply distributed to various substations from substation 1 and 13 respectively, through numbers of switchboards at following voltage levels 11kv and 6.6kv feeding plant loads. These boards are therefore one of the critical equipment s at site. These switchboards are located in various Substations and are due for maintenance. The COMPANY is now looking for a qualified CONTRACTOR with professional expertise and experience in relevant system and equipment; to provide maintenance services of all its switch boards, all three phase bus-bars, will be cleaned and the connections re-tightened and tested. All outgoing and incoming circuit breakers will be serviced and tested. To carry out all tests to ensure that the board is safe and operationally fit to be energised after the work has been completed in line with Oman LNG detailed scope of work, procedures and Permit to Work system (PTW). 2.0 BRIEF DESCRIPTION OF SCOPE OF WORK The intended Switchboard Inspection, Service & Testing of HV Circuit Breakers contract shall be executed by competent CONTRACTOR s approved personnel who shall carry out preventative and corrective maintenances on switch boards and circuit breakers as stated above and in the detailed scope of work. The contractor shall be required to provide experienced staff based in Sur to carry out these services on site in a safe and professional manner. The contract duration shall be for two to three years depending on plant site turnarounds (shutdowns). The switchboards details are as follows: Brand Name: Holec/Eaton Type: Holec Unitole UP4 The intention is to have one contract, however, the Company at its own discretion may split the service and award two contracts: - 11kV & 6.6kV Switchboard Inspection - Service & Testing of HV Circuit Breakers 3.0 OBJECTIVE OF PRE-QUALIFICATION The objective of this contract pre-qualification exercise is to invite and identify capable contractors who shall be short listed to participate in subsequent tendering of OMAN LNG Switchboard Inspection, Service & Testing of HV Circuit Breakers Contract. Suitable Contractors will be selected based on information received that shall demonstrate the capability of performing the required services in line with manufacturer recommendations, company procedures and HSE requirement. 4 3.1 GENERAL COMMERCIAL INFORMATION Registration: Submit copies of commercial registration in Oman. Give the name of the Owner(s) and their percentage of ownership. Give the name(s) & position (s) of your authorized representatives. Address and contacts numbers Financial: Submit a copy of your last 2-years Audited Annual Reports and most recent published Accounts. Approximate Turnover. Total Capitalization Current Structure of Tenderers organisation listing: Numbers of personnel directly employed by your company Number of personnel and personnel working in the field that are involved in Switch Boards and Circuit Breakers Maintenance Services (give job description of each person) Main core business Available Engineering and technical back-up. Also list if sister companies who will be involved in back-up support & how? Key Personnel Submit CV s of the key personnel: o Project Manager. o Testing Engineer. o Switchboard service engineer / supervisor Existence: Give below the number of years you have been established as a Switch Boards and Circuit Breakers maintenance services Company. a) In Oman Years b) In the Middle East Years c) Elsewhere Years Provide type of Switch Boards and Circuit Breakers products (brand) you represent/serviced/maintained in Oman or Middle East. 3.2 Experience: 1. Experience in the Particular Field being tendered. Provide details of your experience in dealing with Switch Boards and Circuit Breakers maintenance services, particularly those involving international companies and Oil and Gas facilities (if any). 2. List of Switch Boards and Circuit Breakers related Projects/Contracts completed.

5 Details to be submitted should include list of similar project undertaken in the past 3 years, which at a minimum should include the following detailed information: - Project and / or Contract name - Client name & Country - Project/contract value - Project/contract duration [start and completion date] - Switch board brand name and voltage level. In addition to the completed projects/contract please provide similar details of current Switch Boards and Circuit Breakers projects/contracts. 3.3 HEALTH, SAFETY & ENVIRONMENT (HSE) INFORMATION Health safety and Environment based on industrial and oil and gas Plant standards and environment HSE Policy: What is your company s Policy on HSE? If your Company s HSE policy exists as a formal document please submit a copy. Submit copy of your organization chart. Identify who has the HSE role / responsibility and briefly describe their HSE related job / works. If no policy exists, explain how you intend on Handling HSE related issues HSE Training Submit list of training programmers and records of HSE training attended by your staff and clearly indicate if these staff will be working in this contract Safety Records Submit 2 examples of your safety records for the last 2 years 4.4 QUALITY ASSURANCE Submit one copy of the following.
