3 1. Purpose Seqwater is committed to the health and safety of all people at the workplace. The purpose of this procedure is to outline the minimum requirements and processes used at Seqwater operational sites to become a pre-approved worker and undertake pre-approved work, which removes the requirement to complete a permit to work. This procedure forms part of the Site Access and Works Control System (SAWCS) and supports Work Health and Safety (WHS) requirements outlined in Element 9 Operational control of Seqwater s WHS Management System Framework (MAN ). This procedure adopts and is consistent with the requirements of: Work Health and Safety Act 2011 (Qld) Work Health and Safety Regulation 2011 (Qld) 2. Scope This procedure applies to all Seqwater workers and operational business groups, including all contractors, who wish to undertake pre-approved work at operational sites. 3. Roles and responsibilities Role Responsibilities Manager Ensure all personnel working within their area of responsibility have completed SAWCS training where identified in the WHS Training Needs Analysis (TNA). Ensure compliance with this procedure within their area of responsibility. Establish a team authorised to identify, assess and approve pre-approved work. Operational Coordinator Coordinate groups and teams undertaking pre-approved work. Endorse pre-approved workers after successful completion of the Pre-approved Worker Authorisation Assessment Checklist (FRM-00625) and based on the recommendation of the assessing line supervisor. Notify the local learning administrator of people endorsed as a pre-approved worker for recording in Our Learning, including the details of the sites they have been preapproved to undertake work at. Ensure all pre-approved work assessments are forwarded to Tactical Maintenance Team for input into CIS. Line Supervisor Abide by the requirements of this procedure at all times when identifying, assessing and approving pre-approved work. Consult Our Learning to monitor pre-approved workers prior to allocation of pre-approved work. Update CIS with details of work that has been approved as pre-approved work. Access Officer Abide by the Operational Site Access Authority Procedure (PRO-01487). Monitor workers undertaking work at operational sites that they are responsible. Maintain the access log for operational sites they are responsible for. Discuss the nature and impact of the job plan for the preapproved work with the pre-approved worker to ensure the proposed work does not produce conflicts in operational and maintenance requirements for the site. Identify area specific hazards that the pre-approved worker may not be aware of (e.g. other workgroups in the area, chemical storage facilities, changed conditions, etc.). Pre-approved worker Abide by the requirements of this procedure at all times when performing pre-approved work at Seqwater operational sites. Undertake the relevant authorisation assessments with their Seqwater line supervisor that are required for the preapproved work they wish to complete. Ensure site access has been approved and granted prior to work commencing. Undertake a review of the relevant pre-approved job plan and Job Safety and Environment Analysis (JSEA) with workers, adding additional risk controls prior to commencing work if required. Perform isolations by securing each point with an isolation lock and tag as per the job plan/isolation instruction and JSEA. Review and sign the JSEA with the work team prior to commencing work. Contact the access officer to notify them of the status of equipment and/or completion of work when leaving the site. Worker (not preapproved) Abide by the requirements of this procedure at all times when performing work at Seqwater operational sites. Participate in the review of the job plan for the work and develop/review a JSEA prior to commencing work. Abide by all standard Permit to Work (PTW) obligations. If a work-order is identified as pre-approved work and the worker has not been assessed as an authorised preapproved worker, then that worker is to obtain all required permits as per the point above. 5 4. Procedure 4.1 Determination of pre-approved work eligibility Work is determined to be pre-approved work by a team authorised by the relevant manager. The following steps are taken to assess work for eligibility as pre-approved work: 1. Compile a list of all routine/scheduled work undertaken at operational workplaces. 2. Assess the work identified to determine if it: - is a routine or scheduled task with a residual risk rating of low. - does not impact on the safety of others or the ability to supply water to customers - is supported by a task specific job plan - previously developed JSEAs confirm that that all activities associated with the work have a residual risk rating of low - is supported by a verified and tested isolations instruction (where required) - is supported by appropriate training and authorisation assessments. If the authorised team is satisfied that the work meets the criteria listed above, then the team may authorise the work for inclusion as pre-approved work. If the above criteria cannot be met, the work defaults to the Permit to Work (PTW) process. Once the authorised team have identified work to be included as pre-approved work, the team will: develop a JSEA for this work utilising the Pre-approved Work JSEA (FRM-00616) notify the Tactical Maintenance Team in writing, which can occur by way of , for input into CIS. 4.2 Issuing of pre-approved work Pre-approved work is issued through the CIS work order system with the relevant job plan and pre-approved work JSEA attached. The person delegating pre-approved work (i.e. line supervisor) must ensure that at least one of the workers assigned to the job is an authorised pre-approved worker for the work to be undertaken. It is the responsibility of the pre-approved worker to ensure that non-preapproved workers understand the requirements and restrictions of the pre-approved work procedure and each party sign the JSEA confirming this. 4.3 Conducting pre-approved work The following requirements are mandatory to conduct pre-approved work: 1. Pre-approved work is identified on the front of the work-order template. 2. Work will only maintain its status as pre-approved work if it is performed or directly supervised by an authorised pre-approved worker. 6 3. Pre-approved work requires site access as outlined in the Operational Site Access Authority Procedure (PRO-01487). 4. The job plan and JSEA must be reviewed by the authorised pre-approved worker prior to commencing the work to ensure that any additional hazards are identified based on any potential changes to the equipment or work environment 5. Any additional hazards must be documented on the JSEA and risk control measures implemented to mitigate the risk to low. 6. If the additional hazards are unable to be mitigated to a low risk, then the preapproved status of the work at that point of time is cancelled. The pre-approved worker must then: a. contact the relevant line supervisor to discuss the change b. complete a permit to work as per Permit to Work Procedure (PRO-00900) to undertake the work. The line supervisor must then review the work to determine if the changes to equipment or work environment will affect the pre-approved status of the work upon its next scheduling. 4.4 Pre-approved workers The steps to become a pre-approved worker are: 1. The line supervisor undertakes an assessment of a worker using the Preapproved Worker Authorisation Assessment Checklist (FRM-00625) 2. Once the line supervisor is satisfied that the worker should become authorised to undertake the pre-approved work, a written recommendation, which can occur by way of , will be made to the operational coordinator to endorse the person as a pre-approved worker. 3. If the operational coordinator endorses the person, the coordinator will notify the local learning administrator to enter the details into Our Learning and save the completed checklist in TRIM. Note a pre-approved worker is only approved to undertake pre-approved work for sites and disciplines that they have been assessed for. Pre-approved workers will be required to be re-assessed against the relevant sections of the Worker Authorisation Assessment Checklist (FRM-00625) when any one of the following occurs at the discretion of the operational coordinator: change to site plant and equipment change to site specific hazards (e.g. introduction of new chemical) change to plant process. 5. Training requirements Training will be provided in accordance with the WHS Training, Competency & Behaviour Management Procedure (PRO-01574). 7 6. Monitoring and audit The requirements of this procedure shall be audited in accordance with the WHS Internal Audit Schedule and the Internal Audit Procedure (PRO-00002). Reporting to management will be conducted in accordance with the WHS Reporting Procedure (PRO-01334). 7. Record keeping All records created relating to this procedure are to be stored in TRIM. All records are to be retained, archived and disposed of in accordance with the Queensland State Archives General Retention and Disposal Schedule for Administrative Records. Additional guidance regarding mandatory record keeping requirements is provided in the WHS Record Keeping Matrix. 8. The access officer is responsible for assessing and approving/rejecting access requests and Permit to Works. They must ensure the proposed work does not conflict with operational and maintenance requirements for the site and identify area specific and process hazards that the requestor may not be aware of. A team of workers established by a manager who have been authorised to identify and assess work within the manager s area of responsibility that will be included as pre-approved work, then develop a pre-approved JSEA for the work. A situation that has the potential to harm a person and/or the environment and/or damage property. The job plan is a detailed step-by-step instruction of how to perform the work and is to be attached to the work order. A step-by-step method of identifying hazards, evaluating the risk, implementing control measures and providing a safe system of work. A Line Supervisor is a person with day-to-day supervisory responsibilities for workers within a functional area of the business. A Line Supervisor includes, but is not limited to, Team Leaders, Coordinators and Level 4 or 5 Supervisors. A Line Supervisor is also considered a worker, but has additional responsibilities for the implementation of the WHS Management System as identified in the WHS Management System and/or position description. An Seqwater worker who has additional access to Our Learning which allows them to update and maintain information about other workers within their team or group. A person with the responsibilities for managing a functional area of the business including the workers within the relevant functional area. 9 Term Operational Coordinator Definitions manager is also considered a worker, however managers may have additional responsibilities for implementation of the WHS Management System as well as any additional responsibilities as an officer of the business. This is a Level Four Manager on an operational site. It includes, but is not limited to, the following roles: Maintenance Coordinator Operations Coordinator Catchment Operations Coordinator Water Source Operations Coordinator. Operational site Any Seqwater owned and/or operated site that is not used solely for administrative purposes. Operational sites include, but are not limited to: water treatment plants pump stations dams catchment and recreational areas. Our Learning Permit to Work (PTW) Pre-approved work Seqwater s online learning management system. A formal document used to identify the controls to be implemented to provide a safe workplace and safe systems of work. Pre-approved work is work that has been assessed and approved by a team authorised by a manager as work as set out in this procedure that satisfies the following criteria: Is routine or scheduled with a residual risk rating of low. Does not impact on the safety of others or the ability to supply water to customers. Is supported by a job plan and JSEA for work that has been verified and reviewed by an authorised team as defined in this procedure. Is supported by appropriate training and authorisation assessments. Pre-approved worker Risk control Real Time Hazard Assessment (RTHA) A worker who is authorised and site inducted to carry out preapproved work at an operational site and has been assessed as a pre-approved worker for that site. Means taking action to eliminate health and safety risks so far as is reasonably practicable, and if that is not possible, minimising the risks so far as is reasonably practicable. Eliminating a hazard will also eliminate any risks associated with that hazard. A documented hazard assessment, or infield review, performed at the intended place of work immediately prior to undertaking the work, or during work where there is a change in conditions. 10 Term Training Needs Analysis (TNA) Work Work order Worker Workplace Definitions The identification of all training needs required by workers at Seqwater. For the purpose of this procedure, work is defined as any task that is conducted on an operational site outside of an administration, meeting or office environment. The term work excludes courier drivers, mail delivery, waste collection or security personnel. A document, generated by CIS, providing authority for costs to be incurred to undertake specified work by an identified person or team. Worker means a person who carries out work in any capacity for Seqwater, including work as: an employee a contractor or subcontractor an employee of a contractor or subcontractor an employee of a labour hire company who has been assigned to work at Seqwater an outworker an apprentice or trainee a student gaining work experience a volunteer a worker of a prescribed class. A place where work is carried out by Seqwater and includes any place where a worker goes, or is likely to be, while at work. This includes a vehicle, vessel or other mobile structure.
