Reportability: Reports to: Case Managers Social Service Supervisor and/or Director My next authority step is the Agency Director Supervises: None Dimensions: Budget: As assigned up to $100,000. Clientele: Persons with serious and persistent mental health problems, serious mental health problems, severe emotional disturbance or emotional disturbance or families involved in child protection and child welfare services, senior services and the general public.

Provide ongoing paraprofessional services to SPMI, SMI, SED and ED clients. A. Instruct, train, assist and support clients in areas such as: interpersonal communication skills; community resource utilization and integration skills; crisis assistance; money management and shopping; healthy lifestyle skills and practices; cooking and nutrition; transportation skills; medication education and monitoring; mental illness symptom management skills; household management skills; employment related skills; personal care and hygiene, transition to community living skills; independent living skills; self-advocacy skills; coping skills; nurturing skills; discipline techniques; stress management skills; personal safety issues; and healthcare program issues. B. Assist clients in arranging needed crisis services that may include assessment, intervention and stabilization. C. Plan and develop group activities for clients, facilitate and/or assist with groups as assigned. D. Inform case manager, mental health practitioner, mental health professional, clinical supervisor, treatment director and /or social service supervisor of any change in client s status, symptoms of mental illness, physical health and other behaviors or conditions that may lead to abuse or neglect of the client. E. Document and report unusual events, statements and/or occurrences observed or discussed with clients and inform case managers and/or supervisor of these events. F. Assist clients in obtaining and maintaining living arrangements that are least restrictive and most appropriate. G. Document information in case files and prepares written reports per agency standards and timelines. H. Give testimony in court for agency and/or may accompany client to court to give support. I. Provides ongoing paraprofessional services to child protection and child welfare clients. J. Other duties as assigned by supervision.

3 35 Responsibilities in Conjunction with Senior Coordinator, assist with: 1. Coordinate services and programs to maximize efficiency and benefits to those served. a. Work with existing senior programs as needed. b. Receive inquiries; make contacts, correspondence, administration-daily. c. Attend meetings regularly d. Work with Traverse County committee on Aging quarterly e. Establish relationship with community organizations, service providers, and agencies. f. Assist in senior access to transportation program. 2. Conduct outreach activities in order to become acquainted, assess needs, and involve new people. a. Home visits regularly. b. Telephone and office contacts daily. c. Senior Center visits and meetings. d. Data collection annual. 3. Provide for public education and dissemination of information to increase knowledge and awareness of the public regarding aging issues. a. Newspaper columns monthly. b. Radio and public announcements as needed. c. County-wide publications and distributions as needed. d. Attend senior meetings throughout the County. 4. Coordinate activities with other agencies, service providers, and public bodies to increase effectiveness of programs, avoid duplication gaps in service, and contribute to mutual objectives. a. Participate in appropriate organizations, advisory committees: Association of County Coordinators. b. Regular contact with other service providers and public officials: Green Thumb, Rural Minnesota CEP, public health nurse, family services, community recreation, community education, RSVP, Area Agency on Aging, West Central Community Action Program, health care providers. 5. Maintain routine bookkeeping, record keeping, data collection and correspondence procedures for the purpose of planning, accountability, and reporting. a. Counts of participation. b. Purchasing. c. Referrals, applications. 6. Produce timely monthly, quarterly, and annual reports as requested or required. a. Area Agency on Aging quarterly. b. County commissioners annually with aging committee. 7. Serve as a focal point in the County for information requests from individuals and organizations and as a liaison and advocate on behalf of the interests of senior citizens for the purpose of assisting people in achieving their desired contact and results. a. 4 8. Assist in design and implementation of new programs which will enhance the quality of life for elderly persons, when appropriate. a. Direct services (nutrition, transportation, in home). b. Educational and self-help programs. c. Volunteer services. d. Caregiver Support Group. e. Vision Support Group. 9. Remain informed on policies, issues, and events pertaining to the older population, in order to maintain a working knowledge and to share information when needed. a. Workshops, conferences, trainings and meetings. b. Literature Forms Assistance: Assist clients with applications for public assistance programs e.g. cash assistance, food programs, subsidized housing, social security disability, medical assistance, etc. A. Educate clients regarding available public assistance programs. B. Assist clients with applications when appropriate. C. Accompany clients to appointments when necessary. D. Assist clients with appeals. E. Assist clients with budgeting. F. When agency is representative payee for benefits, assist clients with back payments, overpayments, recertification, monthly allocations, etc. G. Assist clients with employment issues related to benefit assistance. Administrative Responsibilities A. Attend and participate in required meetings with case managers, mental health practitioners, mental health professionals, clinical supervisor, treatment director, social service supervisor, clients, legal representatives and others to meet program requirements. B. Maintain documentation of services provided according to appropriate statue, rule and program requirements. C. Foster Care Licensing Forms D. Comply with all applicable laws and program rules. E. Work with clients in a professional and ethical manner. F. Meet agency standards for time reporting. G.Other duties as assigned by supervision.

5 FREEDOM TO ACT AND PROBLEM SOLVING Ability to problem solve and make decisions on a common sense level. The majority of activities is guided by the directives of case managers or supervisors, but does require the worker to act freely within those guidelines. Decisions regarding the delivery of social services must be reviewed with supervisors. JOB RELATIONSHIPS Internal interactions with: Supervisors, Social Workers and other Human Services Staff. External interactions with: Clients, Providers, and Volunteers Ability to deal with customers who are under stress, and who may be angry, hostile, verbally aggressive, and may use language that would be considered inappropriate in normal business interactions. The ability to diffuse these situations is highly desirable. QUALIFICATION REQUIREMENTS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND/OR EXPERIENCE Completion of two years of coursework in social work, psychology, sociology or closely related behavioral science area or two years of experience working with families or individuals teaching home management and other independent living skill areas. LANDUAGE SKILLS Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. MATHEMATICAL AND COMPUETER SKILLS Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to operate personal computer, word processing, and county/state computer systems. REASONING ABILITY Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
