Classification Remuneration Service Area/division/state Effective date Community Mental Health Practitioner (CMHP) Level 4/1 72, per annum plus 9.5% superannuation and salary packaging Community Services Melbourne East/South East /Victoria February 2017 Under the direction of the Service Manager, provide strong leadership to the team according to the requirements of Mind, coordinate aspects of the operation of the team on a day to day basis, and provide services to clients, families and carers in line with ASDFMind s Model of Recovery Oriented Practice. The service type is outreach community mental health, which provides recovery oriented services to people experiencing significant mental health problems with complex needs. Programs are provided by the Community Team: PHaMs (Personal Helpers & Mentors service) is an Australian Government initiative, funded by the Commonwealth Department of Social Services. By using a strengths based-recovery focused approach, provides assistance to people aged 16 years and over whose ability to manage their daily activities and live independently in the community is impacted because of a functional limitation resulting from a mental illness. MHAPD (Mental Health Accommodation Pathways at Discharge) is designed to provide services to those people aged who are identified as homeless or at risk of homelessness prior to discharge from an inpatient mental health facility in the inner east. The Yandina program assists clients who may experience significant mental health challenges with complex issues and be homeless or at risk of homelessness. Employees work collaboratively with a variety of mental health clinicians in order to engage clients and address their homelessness situation FFS (Fee for Service) Mind East currently provides service to NDIS and TAC clients to support their recovery journey through both direct support and care coordination functions.

3 Provide day to day supervision and coaching to staff Co-ordinate day to day operations Provide direct support to individual customers Provide support to families and carers Undertake group work Work with other local service providers Working in the customer s community Other Lead and model for staff effective practice in the four main components of their work: working individually with customers; providing support to families and carers; undertaking planning and delivery of group work; and working with local service providers Coach staff in a variety of practice techniques such as: brief intervention; motivational interviewing and coaching; family inclusive practice; trauma informed practice; conflict resolution; behaviour support for dual diagnosis Actively support the use and awareness of Mind policies, procedures, tools and systems through staff development and modelling Assist staff to problem solve, develop creative solutions and remain motivated in meeting customer s goals Support staff in understanding and managing risks that are particular to working solo in the community, including in the customer s own home, and action as appropriate Motivate staff to work in a self-directed way to action, monitor and review the recovery needs of multiple customers living in their own community Positively influence and contribute to a service and team culture that focusses on meeting the customer s goals using evidence informed practice Provide practice supervision to staff who are providing direct support to individual customers as well as supervision to other staff Operate within delegated authority and undertake a range of administrative tasks that support efficient and effective service delivery including: Rostering Unplanned leave coverage Actively plan budget expenditure with guidance from the Service Manager Ensure accuracy of client file data Support Service Manager to ensure performance targets are met in line with funding and organisational requirements Provide direct support to individual customers to enable them to return to their place of residence and a meaningful life through supporting them through a staged approach to recovery, using agreed practice techniques and approaches Support family and carer roles including the re-engagement and maintenance of family and carer relationships Plan, deliver, evaluate and review group work programs that will assist customers to build their skills, focus on their recovery and remain in their natural community Engage with customers to fully understand their need for assistance from local service providers with clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation and social connections Make linkages and build relationships and referral pathways to maintain or create a range of local supports for customers that facilitate them living the life of their choosing in their own community Build an understanding and awareness of risks that are particular to staff working solo in the community, including in the customer s own home, and action as appropriate Oversee and provide guidance to staff working in managing case load to action, monitor and review the recovery needs of multiple customers living in their own community. Documents all activities using Mind s ICT system and processes Actively participate, contributing to your team and wider organisational initiatives Undertake relevant training and professional development, including regular supervision. Take personal responsibility for the quality and safety of work undertaken Encourage feedback and contributions to service delivery improvements Other duties as delegated.

4 Reports directly to the Service Manager Core Requirements Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind Current valid Driver s License. Current National Police Record Check. Current Working With Children Check (Employee). 5 This position is full time fixed term working 38 per week until 30 June Working days will generally be Monday to Friday, worked between the hours of 8am to 6pm as required. Please note that the work pattern may be subject to change to meet the needs of the service. You are able to salary package a tax free amount, which is currently up to $15,900 per FBT year, plus other items including meal and entertainment and accommodation. The amount available for salary packaging will be subject to Commonwealth legislation as it applies from time to time and on the basis that Mind will not incur any liabilities for FBT. Mind offers salary packaging through an outsourced provider (currently RACV Salary Solutions). Other conditions are as per Mind s industrial agreement. Employer superannuation contribution will be paid according to the legislative requirement. Appointment is subject to a six month probationary period of employment.
