1 THE DEMING CUP FOR OPERATIONAL EXCELLENCE 2015 Nomination Form Awarded annually, the Deming Cup for Operational Excellence recognizes world leaders and practitioners who have made outstanding contributions in the area of operational excellence and who have fostered the culture of continuous improvement in an organization. Award Criteria and Eligibility Requirements: A global leader who has applied and demonstrated the Deming principles 1 An organization that exhibits multidimensional excellence (e.g. excellence in safety, environmental responsibility and a view towards inclusion) An organization known for continuous improvement An organization that has increased in shareholder value over a sustained period of time due to organic growth An organization where operational excellence has made a decisive impact on the competitive position of the firm Evidence of a culture in which all stakeholders in the organization are treated with dignity and respect A leader who requires data-driven analysis and has clear commitment to impactful training and skill development Nominations can be made through self or third-party recommendations. The nominee must demonstrate his/her contribution in the achievements of operational excellence through specific and measurable outcomes. Individuals can receive the award only once during their lifetime. Please submit the completed form and supporting documentation to deming@columbia.edu. 6 EXHIBIT 1 DEMING PRINCIPLES Create constancy of purpose for improvement of product and service. Adopt the new philosophy. Cease dependence on mass inspection. End the practice of awarding business on price tag alone. Constantly and forever improve the system of production and service. Institute modern methods of training on the job. Institute modern methods of supervision. Drive out fear. Break down barriers between staff areas. Eliminate numerical goals for the work force. Eliminate work standards and numerical quotas. Remove barriers that hinder the hourly worker. Institute a vigorous program of education and training.
