2 The Bidder must provide the rate and the amount for each tender item, the total for each part / subsection, the grand total, HST amount and the total amount of tender on the forms in the ensuing pages. Bidders that do not fully complete these forms (such as leaving lines blank), or have unclear answers (such as "n/a", "-", "tba" or "included" etc.) will be declared non-compliant. Prices that are intended to be zero cost/no charge to the City are to be submitted in the space provided in the price schedule as "$0.00" or "zero". All spaces for the aforementioned information must be completed in ink ensuring the printing is clear and legible. Where included, the Supplementary List of Prices Required for Extra Work form must also be completed. General Notes: 1. Normal work hours will be 9.5 hours per day (0.5 hour unpaid lunch) on site. 2. If the Contractor arrives on site but work cannot commence for any reason other than that which is the Contractor's responsibility, the City will pay a minimum two (2) hours work time unless a City of Toronto representative cancels 12 hours prior to the scheduled commencement of work. 3. If the Contractor starts to work and ceases work for any reason other than that which is the Contractor's responsibility, the City will pay for a minimum four (4) hours work time. 4. The bidder must understand that not all the labour and/or equipment will be used at the same time, rather those will be chosen as per requirement. The payment will be made based on actual usage. Notes to Labour Rates: 1. Non-Working Project Coordinator hours to be paid for only when present on the job unless otherwise authorized in writing by the Contract Administrator. 2. Premium time to be paid for Night, Weekend, Statutory Holiday work and during work on expressway weekend closures only. 3. The total for Labour Premium Time Hourly Rates must exceed the total for Labour Regular Time Hourly Rates. 4. There is no premium rate for equipment and material. 5. The cost of traffic control as per current Traffic Control Manual for Roadway Work Operations Field Edition, Book 7 shall be included in the labour rates except for payment for police services or where additional traffic control person is required. 3 Notes to Equipment Items: Section 3 Tender Submission Package 1. Ancillary Tools for Emergency Repairs ( Item 26) The Emergency Repairs will be necessary at single or multiple locations in any day of work under this contract. In order to carry out all works efficiently, the contractor must supply the set of following tools/small equipment as part of the Utility Vehicle. Tool air hammer (7 kg. points and 15m hose included), electrical chipping hammer (8 kg., approximately 1300 watts ) 45 kg. sand blasting unit (including. pot, head and nozzles and hoses), generator (min. 5kW), concrete vibrator (rubber tipped), concrete vibrator (pencil type - rubber tipped), concrete saw (min m) including blades for concrete, concrete hammer drill including bits, hand mobile oxy-acetylene welder including gas and rods, water pump (centrifugal 75 mm) air cooled gasoline, including 6 m suction and 7.5 m discharge, concrete saw (min m) including blades (for steel), vibratory plate compactor (100 kg), chain saw (0.40 m), concrete vibrator (50 mm), oxy/acetylene torch (hand mobile heavy duty), generator (5 kw, air cooled, gasoline) including one (1) 110 V and one (1) 220 V outlets, concrete saw, self propelled, blades included (45 kw), air compressor (silenced 25 m 3 /min), tag-a-long trailer (19,000 kg), DC arc welder 200 amp including rods and one (1) 110 V outlet. Laser level for level survey The set of Tools as indicated above shall include other hand tools necessary to carry out all required works and the payment will be made as a single unit per hour under the appropriate Utility Vehicle Item in the. The Utility Vehicle item ( with all the Ancillary Tools ) will be paid in full for the time the vehicle actually remains in operation with the engine running and shall include travel time from one work location to another. Payment for the time, the Utility Vehicle remains on site but not in use, shall be made at 35% of the rate quoted by the contractor in the pricing form. 3. Any other equipment will be paid at current OPSS 127 rates or if rented paid as per General Conditions. The Contractor shall note that payment for any equipment shall be based on actual time used only. Materials: 1. Payment for materials under this contract will be in accordance with the Special Specifications, SS Salvageable/Reusable materials will be paid at 50% of the invoice cost, or moved to District Yard at the Contract Administrator's discretion. 3. All materials removed during the course of the work are to be disposed of at the Contractor s expense and included in the appropriate unit bid price. Method of Payment for the Scope of Work: 1. Except for SS.79 (payment for material) the payment for completed works under this contract will be based on the prices quoted by the contractor for labour and equipment with no markup.
