1 CONCEPT NOTE FOR STROMME FOUNDATION PARTNERSHIP Please note that your concept note application must be submitted by 20 th January 2018 at 4:30 pm. Submission could be by post on the address: Strømme Foundation, P.O.BOX 27200, Kampala Or on Your Concept note should not exceed 8 pages (without the appendices) and please submit with your concept, the last three annual Audit reports and copy of current registration certificate and Memorandum and articles and three references. Check the guidelines from the newspaper advert, to guide you in filling in this Concept Note. 1. METHODOLOGY / PROPOSED IMPLEMENTATION a) What Methodology will you use to deliver the project? b) In what way(s) is your project innovative? c) How will your proposed project achieve sustainable change for the participants beyond the project period This should outline a high-level sustainability plan for the proposed project If the proposed project will not be fully sustainable by the end of the programme, highlight components of the project that could be sustainable. d) How will your proposed intervention align with the SDGs and which ones? e) How does your intervention align with existing or planned government or other donor activities, if at all (not duplicating existing intervention)? f) What long term changes will your proposed intervention support (like attitude, systems, cultural norms or social norms or any others) g) What is the organisation s ability to conduct local or national level advocacy? Give examples of how this has been done in the past, if any. h) What will be the indicators of success for your project? In particular, what evidence do you have that this project is likely to be successful in achieving the planned outcomes. i) What are the key challenges and operational risks (internal and external) that you anticipate in delivery of this project and how do you plan to mitigate? j) What are your plans to monitor and evaluate your project? k) What are your key organisational sterngths, weaknesses and capacity building required? l) Please add anything additional that is critical to this concept note. 5
