We group our programs into: community, education, family services, foster and kinship care, residential care, therapeutic services and youth services. Band: (A-F) A Date Completed: 5/6/2013 Berry Street s Intensive Case Management Service (ICMS) is funded by Department of Human Services to provide an intensive level of support to young people between the ages of 12 and 17 who are most at risk in the community. The service operates across the Lower Hume Region. PRIMARY OBJECTIVES OF THE ROLE The primary objective of the role is to provide intensive case management to young people, between the ages of 12 and 17, who are most at risk in the community in the Lower Hume Region. It is part of the broader Adolescent Specialist Services Team which also includes Residential Services. This role reports to the Manager, Adolescent Specialist Care Services who will provide supervision and review. This role has no direct reports and works in conjunction with other team members.

2 KEY SELECTION CRITERIA KNOWLEDGE, SKILLS AND ABILITIES REQUIRED TO FULFIL THE ROLE A capacity and ability to demonstrate Berry Street s Core Capabilities is required to be successful in this role. Knowledge and relevant experience in the provision of support for young people with complex needs and/or experience in the provision of specialist services. Demonstrated ability to achieve positive outcomes for young people through a case management approach, including the ability to determine and access post care pathways appropriate to the needs of the client. Excellent communication skills able to converse comfortably and effectively. Capacity to engage with challenging young people and their families. Knowledge of services available to support young people and demonstrated experience in the provision of support and services to young people facing barriers to employment and education. QUALIFICATIONS AND OTHER REQUIREMENTS Diploma of Community Services or equivalent. Staff must hold a valid WWCC, current drivers licence at all times and undergo a Criminal Records Check prior to employment. Subsequently, staff must report any criminal charges or court appearances. ACCOUNTABILITY ACCOUNTABILITY Direct Service Delivery Case Planning and Case Management Demonstrated ability to develop relationships and linkages with local providers, such as DHS, Youth Justice and the Homelessness sector. Able to think clearly, making decisions as required, often in a crisis. Well-developed negotiation skills able to manage young people whom display strong emotions and challenging behaviours. Good written and comprehension skills demonstrated ability to write reports and take case notes. Ability to work comfortably in a team assisting colleagues as required. Desire to want to provide support and care for children and young people. Knowledge of demonstrated understanding of and commitment to the principles of equity, diversity and occupational health and safety. DESIRABLE Tertiary qualification in Social Work, Psychology or related discipline. KEY ACCOUNTABILITIES AND RESPONSIBILITIES describe the key duties that will indicate success in the role. SPECIFIC RESPONSIBILITY SPECIFIC RESPONSIBILITY Carry a case load of up to 8 young people in the target group. Provide individual support through a direct casework service aimed at minimising the protective concerns and maximising personal development through a range of intervention techniques. Develop ways of engaging at risk young people who may not have responded to traditional intervention. Liaise with relevant services and advocate on behalf of at risk young people to ensure access to resources and facilities where necessary. Work with young people and their families and other significant people in their lives to facilitate contact, negotiate secure alternative accommodation options and provide assistance with conflict resolution as appropriate. Develop, in conjunction with clients, referring DHS worker, Manager Adolescent Specialist Services, and others as appropriate, individual Best Interest Plans which specify objectives of intervention, anticipated timelines, and tasks and responsibilities necessary for their achievement. Negotiate with DHS and other service providers to ensure co-ordinated and co-operative service delivery. Maintain adequate case records, and prepare all reports and documents as required by DHS, BS, Children s Court or

3 other relevant person or service. Other Prepare Critical Incident Reports for DHS as required. Be prepared to carry a mobile telephone as part of a roster providing 24 hour cover in order to respond to crises when required. Also be prepared to be recalled after hours as part of this roster. Represent ICMS and/or Berry Street Hume Region as required. Provide reports to the Manager Adolescent Specialist Services, and the Hume Regional Director, as requested and in the agreed format. Be prepared to manage a specialist portfolio (eg. ICMS program development) as required. Other duties as directed. CERTIFICATION

5 CONDITIONS OF EMPLOYMENT 1. The hours for this position are 76 hours per fortnight. It is strongly preferred that these hours are worked over a 3 to 5 day working week. There is no paid overtime, but any extra hours worked will be accrued as Time-in-Lieu according to the Berry Street Victoria Enterprise Agreement. 2. You will initially be employed at Shepparton. You may be required to work from other Berry Street sites (on a temporary or permanent basis) as directed from time to time. 3. Terms and conditions of employment are in accordance with the Berry Street Victoria Enterprise Agreement, which includes above Award payments and eligibility for remuneration packaging. Salary packaging is available to full-time and 0.8 staff to a maximum value of $16,050 and a maximum value of $12,000 to all other permanent part-time staff, in line with our Salary Packaging policy. 4. The base salary for this position is based on SCHADS Level 5 Pay Point 1 ($53,404.56) under the Berry Street Victoria Enterprise Agreement. The value of the salary can be increased through salary packaging. 5. Superannuation will be paid according to Superannuation Guarantee into a compliant fund of your choice or into HESTA Superannuation Fund. 6. The successful applicant will be required to undergo satisfactory pre-employment checks, including 3 professional referees, a pre-existing injury/disease declaration, a criminal records check and proof of identify and qualifications. 7. The successful applicant will initially be employed for a probationary period of 3 months. During this period, either party can terminate employment with one week's notice. A probationary review before 3 months will be undertaken. 8. Agency vehicles are available for authorised use and these should be used at all times for work-related purposes, in accordance with the Berry Street Motor Vehicle Policy. However, if you are authorised to use your own vehicle for work-related purposes, you will be paid an allowance per kilometre, which includes provision for comprehensive insurance and other running costs. Berry Street is unable to insure private vehicles owned by staff and will not accept any liability for damage to any staff vehicles incurred while being used for workrelated purposes. 9. Under Victorian WorkCover legislation, it is the applicant s duty to advise Berry Street of any pre-existing medical condition, which could be aggravated by the type of employment for which they are applying. The existence of a medical condition will not preclude you from employment, unless you are unable to perform the inherent requirements of the position. However failure to disclose any relevant injury or disease will jeopardise any entitlement you may otherwise have for a work-related aggravation of that non-disclosed pre-existing condition. 10. 6 INHERENT REQUIREMENTS OF WORK ACTIVITIES / ENVIRONMENT Following is a table that outlines the main physical and psychological requirements of the position.
