1 WORKSITE INFORMATION PACKET Take this packet with you on Day One. 1. Mentor Papers Give the following to your Mentor when you first arrive: a. Informational sheet from Mr. Gibson... p. 2 b. Mentor Agreement (to be mailed to Mr. Gibson at beginning of Mentorship). p. 3 c. Evaluation of Student (to be mailed to Mr. Gibson at end of Mentorship)... p Goals of the Mentorship Form. p. 5 Complete this with your Mentor as early as possible, preferably on your first day. 3. Time Sheet / Daily Journal Form.. p. 6,7 EVERY DAY: record hours, make notes, and get Mentor s initials. Both Mentor and student signatures are required at end of mentorship. 4. Worksite Report Form. p. 8 Interview your Mentor or business owner to complete form. 5. Mentorship Excuse Absence Form.. p.9 Complete form and get Mentor signature each school day you miss, turn into office upon return. NOTE: Time Sheet, Goals of Mentorship Form, and Worksite Report are to be included in your portfolio when you make your Senior Presentation. Don t lose them! Read the following and know what is expected of you. Student Responsibilities: 1. Define with your Mentor the goals and expectations for the mentorship. 2. Demonstrate respect for your Mentor and others in the workplace environment. 3. Learn and follow worksite safety guidelines. 4. Interview the Mentor and complete the Work Site Report. 5. Observe rules and policies at worksite, and note that all school rules apply. 6. Be on time, attentive, and responsible. (Note: 15 minutes early is on time.) 7. Follow the schedule established. 8. Notify Mentor and PCHS facilitator if unable to be present at the appointed time and place. 9. Document activities by taking photographs and saving work samples. 10. Record hours, keep daily notes of activities on time sheet, and get Mentor initials. 11. During school hours, accept no pay or benefits other than the learning experience. 12. Devote a minimum 40 hours at the worksite with Mentor; may work up to 57 hours (recommended). 13. Notify Facilitator or school administration immediately if a problem develops with the mentorship. 14. Sign the time sheet at the end of the mentorship, verifying attendance and completion. 15. Send a thank you note to your Mentor and/or business location. 16. Deliver time sheets to PCHS Facilitator when mentorship is complete. DON T FORGET! Dress appropriately for the location. When in doubt, dress similarly to your Mentor and/or the business employees. Be prompt, helpful, focused, and appreciative. Learn as much as you can. 1

Gibson, Facilitator, PCHS Mentorship Program Thank you for your participation in the Pocahontas County High School Mentorship Program, designed to provide PCHS seniors the opportunity to experience the real world of work within their chosen fields. This program reflects the belief that people in government, business, health, the arts, media, social services, education, and related fields have something significant to teach young people. Time spent with individuals in a working environment provides a valuable awareness and often results in decisive action regarding student career and educational choices. Each Mentor and Student will collaboratively establish goals for the Mentorship and define roles and expectations of the student. This one-to-one relationship between is crucial to the success of the experience. Please use the form supplied by the student. Student and Mentor responsibilities are specified below. After review, please complete the attached Mentorship Agreement and return to PCHS. When the mentorship has concluded, please complete the Evaluation of Student Form and also return to PCHS. Mentor Responsibilities: 1. Provide an internship experience (min. 40 hr.) with an in-depth study of the career or field. 2. With the student, establish with goals for the mentorship. Also, inform him/her of your specific expectations. 3. Provide a safe working environment for student. 4. Require the student to comply with company rules, including workplace safety rules. 5. Remind the student that all school rules apply (as indicated in his/her student handbook). 6. Accept full responsibility for the student's learning and behavior (assuming he/she follows established rules) while under your supervision. 7. Know the provisions of the Child Labor laws relating to the types of work children less than 18 years of age are permitted to perform. 8. Understand that during the hours of the school instructional day, students cannot be paid or receive any other benefits. If so, he/she is not covered by the school insurance policy. If the student works before or after the school instructional day and is paid for the time, the mentor assumes full responsibility for the student and his/her welfare. In the case of an accident during compensated hours, the mentor is responsible for costs incurred. 9. Communicate with the PCHS Mentorship facilitator as necessary. Notify immediately if a problem arises. 10. Sign the time sheet at the end of the mentorship, verifying attendance /completion. 11. Submit by mail an evaluation of the student at the end of the mentorship period. Student Responsibilities: 1. Define with Mentor/s goals and expectations for the mentorship. 2.
