3 Benefit Eligibility Rules All full-time and benefits-eligible part-time employees who are regularly scheduled to work at least 26 hours per week for 8 months of the year. Those who meet these requirements are eligible for all employee benefits. The hours each employee works per week needs to be entered into IOI. 3

19 Flexible Spending Account Health Care Account limit remains unchanged at $2,500 per plan year Dependent Account limit remains unchanged at $5,000 per plan year You MUST re-enroll for by logging on to the Ceridian website Current elections will not carry forward to the next year. Access the Ceridian website to submit claims, access forms check balances and much more 19

20 Qualifying Life Events Employee Employee completes enrollment form Attaches Documentation Submits to Human Resources Human Resources Reviews the documentation If documentation is in compliance with plan provisions, the change is entered in MyEnroll by HR Rep If documentation is NOT in compliance with plan provisions, the forms with be returned to employee In either case HR will notify employee of the decision 20

21 Evidence of Insurability Employee enrolls in supplemental life insurance on MyEnroll If the amount for life insurance elected exceeds the guaranteed issue amount: ONLY the amount up to the guaranteed issue limit will be accepted Employee will complete Evidence of Insurability Form and send to insurance carrier If carrier determines that the Evidence of Insurability is satisfactory, they will approve the amounts of coverage that were elected above the guaranteed issue level according to plan provisions The carrier will notify Human Resources of their decision Additional coverage amounts will be entered into MyEnroll by HR Rep Human Resources will notify employee of the approval or denial of coverage 21

22 Benefit Sweep Effective July 2011 Employee contributions for medical and dental benefit coverages swept directly from bank accounts of parishes and schools Effective October 2013 The location cost (blended rate) will also be swept directly from the bank accounts of parishes and schools The sweep process of blended rate will follow that of the employee contribution 22

23 Wellness Recap CHC Wellness is an on-site corporate and community provider of health and wellness programs Programs will include health screenings, walking challenges and weight management A total of 1,046 employees participated in the wellness screenings 23

24 Walking Program A total of 339 employees participated 42 met the 10,000 step goal 38 met the 8,000 step goal 95 met the 5,000 step goal 24

25 Exempt vs. Non-Exempt Non-Exempt or Hourly Employees Based on duties performed and manner of compensation, will be subject to all minimum wage and overtime provisions of FLSA FLSA requires that all covered, nonexempt employees be paid at least minimum wage for all hours worked Must receive one and one-half times regular rate for hours worked over 40 in a workweek 25

26 Exempt vs. Non-Exempt Exempt or Salaried Employees FLSA provides an exemption from both minimum wage and overtime pay for employees who are employed in a bona fide white collar position: Executive Administrative Professional Outside Sales capacity Certain computer employees Highly compensated individuals 26

27 Exempt vs. Non-Exempt Three Tests to Determine Exempt Status Salary Level Minimum salary level required for exemption is $455 per week Annual salary equivalent of $23,660 Salary Basis Must generally be paid a predetermined amount each pay period Amount paid may not be reduced because of a variation in the quality or quantity of the work performed Must be paid full salary for any week that any work was performed Job Duties Employees must perform certain executive, administrative, professional, outside sales, or computer professional job duties 27

28 IOI Data Terminate employees in a timely manner Ensure benefit plans for employees that transfer between locations are consistent Ensure date of hire and date of benefit eligibility are correct Perform data audit on a quarterly on monthly basis Encourage employees to update personal information IOI data is principle source of employee data for the Archdiocese; incorrect data can effect: Employee benefits including health, life, FSA, disability, 403(b) and share plan contributions (employee & employer) Don t hesitate to call HR or IOI directly HR: IOI:
