1 TEMPORARY DEMOUNTABLE STRUCTURES (TDS) QUESTIONNAIRE This questionnaire must be completed by the person responsible for the procurement of any of the following temporary demountable structures at the forthcoming event: Grandstands Platforms Stage Structures Towers Masts Canopies, Tents or Marquees The completion of this questionnaire may also be required for other TDS s not listed above. It should be appreciated that it is not practicable to produce a generic checklist to cover all TDS s and some questions might be less appropriate in some cases than in others. 2 Question 1 Name and address and contact telephone number of person responsible for procuring the temporary demountable structures at the event: Note: The primary responsibility for the safety of people attending this event and the users of any temporary demountable structures lies with this person. For the purpose of this questionnaire this person will be referred to as the client and must sign and date the last page upon its completion. The client must ensure that competent persons are employed to design, supply, erect, monitor and dismantle any TDS at this event. A competent person shall be regarded as having had sufficient training and with enough experience to take responsibility for an identified task. The purpose of this questionnaire is to enable Durham County Council Safety Advisory Group to be reasonably sure that the client is fulfilling his/her role and that all TDS s will be safe for use at the event. Question 2 Who is the land/venue owner of where this event is to take place? 2

3 Question 3 Who is the event organiser? Please be aware that in most cases, clients, venue owners and event organisers, rather than the suppliers of the TDS s are principally responsible, in law, for complying with public safety legislation while the structure is in use. It should also be borne in mind that penalties for non compliance can be severe. Question 4 Please describe briefly all TDS(s) proposed: 3

4 Question 5 The client must ensure that competent persons are employed for the design, construction, inspection, monitoring and dismantling of any proposed TDS s. Please provide details of the following to support this requirement: a) Names and addresses of proposed Competent Persons b) Qualifications of Competent Persons c) Design drawings and structural calculations which must comply with relevant standards, guidance documents and codes of practice d) A specific TDS Risk Assessment showing consideration of hazards in design, construction, inspection, monitoring and dismantling e) A Safety Plan based upon the risk assessment but which must include dealing with inclement weather during the event f) Method Statements for erection and dismantling Question 6 Please confirm that after construction of each TDS and before opening the event, inspections will be carried out by Competent Persons to ensure they are safe for use. These inspections will be documented and will be carried out by the person named below: Note: In the event of larger or more technical TDS s it may be necessary for the person(s) named in Question 6 to be independent Competent Persons not involved in the actual design, construction or monitoring of the structure. 4

5 Question 7 Please confirm that during the event regular periodic inspections will be carried out by Competent Person(s) and state proposed frequency of these inspections. Frequency: If, why not? Question 8 Please provide details of the Competent Person carrying out periodic inspections referred to in Question 7: Question 9 Please confirm how the needs and convenience of disabled people have been taken into account for them to use the proposed TDS s. 6 Question 10 Please confirm that investigations have been carried out to ensure suitable ground conditions are available and that the positions of existing underground cables or drains will not affect or be affected by the TDS s proposed. Question 11 Please confirm that you have been provided with emergency out of hours contact details for all Competent Persons involved in the provision of the TDS s and provide these contact details below: Question 12 Please confirm that you have provided the landowner with a copy of all information provided here in respect of TDS s and that the landowner fully supports your appointment of all proposed Competent Persons. 6
