By focusing on Whanau and their wellbeing Turanga Health removes the silo delivery approach to health services. Our mission provides and allows for collaboration or integration of other services outside the core business of health, e.g. housing, social services, employment, education and training. We will continue to find ways to "do what the others don't" for Whanau. 3 Job Description Position Title Specialist Area Location Reports To Employment Term Hours of Employment Salary Whanau Ora Primary Health Care Kaiawhina Community Health Worker Whanau Ora Turanga Health Premises Manager Service Delivery or his / her delegate Full Time Position 40 Hours per Week Times of work to be negotiated dependant on projects / events / programs and other Turanga Health activity Negotiable Position Overview The Kaiawhina / Community Health Worker can be the first point of contact for whanau into the health system and with health services in the community. They are an essential part of primary, community health services providing appropriate acceptable and affordable support to whanau. The following are key components of service delivery: Disease Prevention, Health Protection, Health Education / Health Promotion The Kaiawhina / Community Health Worker is responsible for providing services to the community require essential skills focused in communication with whanau, established networks in the community, established track record and rapport with whanau. Scope of Kaiawhina/Community Health Worker Role Work closely along aside the nurse/s and other colleagues within the organisation Develop and maintain a profile of key health services in the area. Develop and maintain a profile of all community groups and organisations in the area. Assist in planning and implementing health programmes for target groups in the area which addresses disease prevention, health promotion/education and healthy lifestyles. Work as a member of the health team to service the community of the area. Complete appropriate needs assessment(s), health management plans as per the service area. Assisting and facilitating consumer satisfaction, consumer evaluation and consumer feedback. Working Relationships The Kaiawhina / Community Health Worker works closely along aside the nurse/s and other colleagues within the organisation and the ability to meet the needs of our community, clients, colleagues and management in regards to the Whanau Ora service is crucial. The Kaiawhina / Community Health Worker will have strong communication skills to ensure that appropriate action and prompt referrals are initiated and followed through in accordance with Turanga Health s referrals policy. Maori Awareness Te Hauora o Turanganui a Kiwa prides themselves on honoring their commitment to the Te Tiriti O Waitangi. It is essential that all staff members of Te Hauora o Turanganui a Kiwa have an understanding of issues affecting Maori, this will also include an awareness of traditional and contemporary Maori and Iwi structures, key Maori concepts, an awareness or understanding of the Te Tiriti O Waitangi. 7 Targets Ensure all clients are registered to Turanga Health, and have an active health management plan as appropriate At a minimum one face to face contact quarterly, either rural or urban with each registered client Integrated approach to service delivery ensuring that whanau are receiving an opportunity to access other services within Turanga Health when there is a need. Data is loaded to ensure that records are maintained Acknowledgement of referrals received of clients have the opportunity to be supported by other Turanga Health services by way referral (internal / external) Weekly meets the 48hrs / 10 days timeframe Non active clients are exited after 3 months Contract reporting is completed as per identified time frames (weekly, monthly, quarterly or annually as required) Evaluations / surveys are carried out on a quarterly basis or as requested Show enthusiasm, initiative and a high degree of flexibility and availability to work variable hours within the contracted weekly hours. Workforce development plans are completed annually and identified training is completed within the timeframe agreed of clients that have not engaged in the service are exited after 3 months of non-contact At least 4 surveys / evaluations carried out a year one survey per quarter / program whichever is appropriate as per Quality Plan. 75% Able to attend service delivery that may happen outside the 8am 5pm working day on Participate in Turanga Health Workplace wellness program Undertake other duties as the company may require from time to time that are of a reasonable, ethical and legal request Qualifications A formal qualification is not necessary but study in relevant areas such as Social work, facilitation of workshops / programs, health promotion and Maori health would be an advantage.
