Yu will play a key rle in creating and sustaining a psitive culture by persnally rle mdelling the Hme Grup values and creating ways f wrking that enables thers t d the same. Yu will specifically: Design a prfitable, high quality, distinctive service ffer that prmtes independence and supprt t peple with cmplex care needs including, intermediate care, reablement, dementia and/r frailty. Supprt the delivery f grwth targets fr cmplex care and assciated services. Wrk acrss the business t ensure a cnsistent apprach t the delivery f ur new re-ablement Service ffer. Ensure alignment f ur service mdel with NHS 5 year frward view, cmmissining and husing agendas. Lead the implementatin, planning and handver f new business int peratinal delivery in relatin t cmplex care services. Ensure ur service ffer meets cmmissiner requirements. Be an Ambassadr acrss the business and externally fr ur distinctive ffers in relatin t (but nt exclusively) intermediate care, reablement, dementia, frailty. Develp custmer utcmes fr cmplex care services. Ensure cmmissined business is in line with care and clinical requirements, natinal standards and prfessinal regulatin (where necessary). Supprt the transitin plans t retain and develp current business (apprpriate t this rle) and t adpt new service mdels as required. Ensure best practice is develped and delivered at rganisatinal and peratinal levels. Challenge ways f wrking and persuade, mtivate and influence ther senir managers t realign their practice where necessary.

T lead and perfrmance manage the cmplex care team. 4 ORGANISATION CHART Head f Clinical Practice Intermediate Care Learning Disabilities Mental Health 5 KNOWLEDGE, SKILLS AND EXPERIENCE REQUIRED Master s Degree level educatin r equivalent experience desirable. Degree level educatin essential. Clinical Qualificatin and Prfessinal Registratin desirable. Significant experience in an NHS, r public services setting. Senir management ability t lead cmplex care services related t bth the public and private sectr in a custmer fcussed multi-site rganisatin. Detailed understanding and experience f: Wrking with peple wh have dementia and/r frailty.

3 Wrking cllabratively acrss a wide range f stakehlders. Business readiness and implementatin planning. Accmmdatin and supprt requirements fr re-ablement. Clinical r alternative interventins and their impact in supprting peple with cmplex care. Persn centred planning. Digital slutins that supprt independence fr peple with cmplex care needs. Well-develped interpersnal and negtiating / influencing skills. Track recrd in managing teams prviding effective and engaging cmmunicatins. Experience f wrking in a service rientated sectr where service excellence is paramunt. In particular in relatin t the develpment f services in alternative settings and new mdels f care as set ut in the NHS 5 year Frward View. Delivery f new business grwth in cmplex care services. Successful transitin f current schemes t a new sustainable mdel. Implementatin planning fr new mdels int peratinal delivery.

4 Ensuring custmers are invlved in service design. Prvide and implement qualitative and quantitative measures t determine perfrmance against the rganisatinal strategy. Reprt prgress against the strategy thrugh persnal representatin at apprpriate frums and by written reprts. Apart frm a respnsibility fr yur wn health, safety and welfare, ensure that staff under yur supervisin perfrm their duties with due regard t the Grup s Health and Safety Plicy, Prcedures and Safe Systems f Wrk, in a safe wrking envirnment.
