2 Aligning an audit program to IOSA Why did we choose to align our audit program to IOSA: IOSA is based on ICAO SMS Framework. IOSA is widely used and accepted. Updated regularly. Emphasizes safety management as a fundamental business process. Facilitates Safety Performance Monitoring & Measurement within our audit program. 2

3 Evolution of SMS Safety Management Systems (SMSs) are the product of a continuing evolution in aviation safety. Early aviation pioneers had little safety regulation, practical experience, or engineering knowledge to guide them. 3

4 Evolution of SMS Initially, significant gains in safety were derived through: Careful regulation of aviation activities. Operational experience. Improvements in technology. Focusing on individual and crew performance or "Human Factors" further reduced accidents. 4

5 Evolution of SMS Each approach has led to significant gains in safety. Even with these significant advances, we still had opportunities to take preventative action. The question was, "what is the next step?" 5

6 Evolution of SMS The term "organizational accident" was developed to describe accidents that have causal factors related to organizational decisions and attitudes. SMS is an approach to improving safety at the organizational level. 6

8 The Organisational Accident ORGANIZATIONAL PROCESSES. These are activities over which any organization has a reasonable degree of direct control. Typical examples include: policy making, planning, communication, allocation of resources, supervision and so forth. 8

9 The Organisational Accident In generic terms, latent conditions can be grouped into two large clusters. 1. Inadequate hazard identification and safety risk management. 2. Normalization of deviance. 9

10 SMS is all about safety decision-making throughout the organization. Emphasizes safety management as a fundamental business process to be considered in the same manner as other aspects of business management. This continues as organizations evolve from reactive to proactive and predictive phases. 10

14 IOSA Checklist Safety Management System ORG A The Operator should have an SMS that is implemented and integrated throughout the organization to ensure management of the safety risks associated with aircraft operations. [SMS] (GM) 14

15 ORG A Note: Conformity with this provision is possible only when the Operator has achieved conformity with all standards and recommended practices that are identified by the [SMS] symbol. Note: Effective 1 September 2016, this recommended practice will be upgraded to a standard and superseded by ORG B. 15

16 IOSA Checklist Requires the organization to examine its operations and the decisions around those operations. Allows an organization to adapt to change, increasing complexity, and limited resources. Promotes continuous improvement of safety through specific methods to predict hazards from employee reports and data collection. 16

17 IOSA Checklist The SMS elements of the IOSA checklist require compliance with existing regulations while predicting the need for future action by sharing knowledge and information. Includes requirements that will enhance the safety attitudes of an organization by changing the safety culture of leadership, management, and employees. 17

22 Independent Audit and evaluation program Internal audits are an important tool for managers to use to obtain information with which to make decisions and to keep operational activities on track. The primary responsibility for safety management rests with those who own the organization s technical activities supporting the delivery of services. It is here where hazards are most directly encountered, where deficiencies in activities contribute to safety risks, and where direct supervisory control and resource allocation can mitigate the safety risks to ALARP. 22
