1 MAXWELL HEALTH FEATURES Maxwell Health Version 5 is a fully-featured and sophisticated Benefits Administration and Private Exchange technology platform. It is based upon an operating system design methodology that enables it to be flexible to implement any benefits strategy for employers of two employees to thousands. The core platform is based upon a rules-engine-driven workflow and temporal data structure that handles complex benefits scenarios and regulations. The innovative benefits selection process for employees is based upon a bundled approached to benefits. This shopping process empowers employees to make the best choice for their benefits package and increases the adoption of ancillary and wellness products. Flexible rates and contribution constructs allow for products to be priced and presented in a way that increases employee understanding and cost transparency. Frictionless user experience and beautiful design is central to the Maxwell experience, with employees placed at the center of every design decision. 2 The Platform Cloud-based - No installation is necessary for any technology at the Broker or Employer site Multi-tenant - The entire platform is multi-tenant allowing for multiple Brokers and Employers to co-exist and keep information separate and secure Encryption - All Employee information is encrypted and stored securely Permission-based - Each screen and function can be restricted based upon overall user permissions throughout the platform Electronic Data Interchange - HIPAA 834 supported EDI connections across multiple vendors and products - Multi-tenant and single tenant capable - Broker-aware The Marketplace Maxwell s marketplace supports enrollment in all employee benefits, including: - Medical, dental, vision - Life insurance, disability - HRA/HSA/FSAs - Work-site (cancer, critical illness, accident) - Voluntary (life insurance, short and long-term disability, EAP) - Other benefits like emergency child care, ID theft coverage, travel insurance, pet insurance, legal benefits, transit benefits, and more Maxwell can work with your TPA or, you can work with our partner, ConnectYourCare, and we can issue Maxwell-branded debit cards for groups HRA/HSA/FSAs Maxwell can work with any 401(K) provider but we have a deep integration with The Online 401(k) for a seamless enrollment experience 2

12 The Employee (Post-Enrollment) Web Interface Employees are able to review and actively approve HR documents Employees can view and download important company-wide notes Employees can review or update demographic information as well as dependent information, sometimes to start a QLE View specific product information and launch into the vendor s member websites from the Employee screen Mobile Interface The mobile app acts as a virtual health card, allowing employees to access plan details for each product in which they are enrolled - They can view breakdown of benefits by product, see their policy or ID number, and access or use carrier contact information The mobile app also allows employees to interact with the concierge service - It s easy for the employee to call the concierge with one tap - They can send a photo of a confusing bill or insurance claim through a secure connection for the concierge to review, explain, or adjudicate (if applicable) - The concierge can also help with: - Explanation of benefits - Creating a health summary - Explaining care options and estimating costs - Doctor recommendations - Scheduling appointments - Collection of medical records - Bill review - Communicating with carriers/providers about disability claims - Assisting with eldercare Employees can use the in-app, device-less tracking, or connect their own fitness device, to track healthy activities - They can link a supported device through the app 12

13 Benefits - The app will track their progress and allow them to participate in (or opt out of) a company-wide leaderboard - Employees earn points/coins for positive activity, which they can then redeem for rewards The portal enables employees to enroll in any available plans They are able to view key aspects of available plan in a clear, transparent way that makes the benefits decision more intuitive, and also view plan costs based on coverage tier Employees can view employer contribution (overall or per-product) and the plan cost per-month or per-pay period Employees can view and download plan documents (SBCs) which are attached to each product Definitions of each insurance or benefits term are easily viewable for employees during the selection process Employees can enter beneficiary information for each applicable product They can enter their primary care physician and access provider lookup to determine strength of network Employees can easily upload proof of prior coverage The confirmation page allows employees to review plan/coverage selection and total cost before confirming - This page also gives a breakdown benefit by benefit, with cost to the employee and the employer, and shows which benefits are pre- vs. post-tax. - They can also use this page to view one-time and recurring payroll deductions - They are able to send a copy of the confirmation statement to their address Employees can use the portal to initiate requests for QLE Employees can view or download previous confirmation statements Wellness Employees get a truly unique wellness experience that connects with some of the most innovative and effective health products available The wellness platform provides fun encouragement while getting healthier, through in-app challenges and competitive leaderboards 13

14 By pulling different data streams together to make conclusions about care, Maxwell is able to provide meaningful, timely feedback to employees that they can act on immediately Rewards Employees can view reward options in the mobile app or portal s rewards store and redeem rewards once they ve earned enough coins Rewards are electronically redeemed via the vendor s website with the use of exclusive promo codes Requests for Help Contact information for the Maxwell Health customer service team is readily available, and employees can also submit requests for help directly through the portal 14
