4 1 Background The Diabetic Retinopathy Screening Service for Wales (DRSSW) is an all Wales service designed to detect sight threatening diabetic retinopathy at an early stage before visual loss occurs, ensuring early treatment and preventing loss of vision in 70-90% of people with sight threatening diabetic retinopathy. The Service was commissioned by the Welsh Assembly Government in July 2002 as part of a risk reduction programme and an important element of delivering the Diabetes National Service Framework. Diabetic Retinopathy Screening Service for Wales is hosted by Cardiff and Vale University Health Board. Welsh Government has initiated discussions around moving the service to be under the governance and management of. Screening Division,, manages all population based screening programmes in Wales. Cardiff and Vale University Health Board and have initiated a project to scope the potential opportunities and risks involved to move Diabetic Retinopathy Screening Service to the management and governance of. 2 Outline of Project 2.1 Initiation of Project Groups A Project Board has been established with representation from Welsh Government, DRSSW,, Cardiff and Vale University Health Board, other Health Boards and Diabetes UK Cymru. The Project Board is chaired by Dr Quentin Sandifer, Executive Director of Public Health Service and has met twice (12th September and 16 December). The Project Initiation Document and terms of reference were agreed at the first meeting. A project team has been established with representation from DRSSW and Screening Division and Ann-Marie Donaghy from Screening Division is the project manager. The work is lead by Dr Sharon Hillier, Deputy Director Screening Division working closely with Andrew Crowder, Head of Programme, DRSSW and Rhys Blake, General Manager of Screening Division. 5 2.2 Project Objectives The project is split into 3 phases. Phase 1, to undertake the initial scoping and information gathering to identify potential benefits and risks to DRSSW and Screening Division of move. Ensure appropriate governance in place while in project phase. Phase 2, the external quality assurance review of DRSSW to be undertaken; baseline assessments of cross programme functions including infrastructure, administration and IT; financial review of DRSSW and costing of programme for the future. Phase 3 (depending on recommendation made), to undertake the necessary work to ensure that there is a safe and effective transfer of governance and that DRSSW is supported by cross programme functions and fits into the Screening Division s governance structure. To complete transfer of programme delivery and associated governance to Screening Division, Pubic. The outline timelines for the project are detailed in Appendix 1 3 Progress The Project is currently in Phase External Quality Assurance Review The External Quality Assurance review was undertaken by the NHS Diabetic Eye Screening QA Team on the 10 th October 2013 and the final report was received by the board on the 16 th December. A consistent theme noted throughout the report was the professionalism of the DRSSW staff. The report noted that the uptake of the identified diabetic population to the screening programme was good at 81% and that the quality of the grading was high. There were also areas for improvement noted in the report around the identification of the entire eligible population, failsafe, sustainably providing an annual screening cycle to all patients and ensuring that there is evidence of good follow on care for at risk patients referred into health boards. The Board has tasked the project team to put together an action plan over the next six weeks to address the recommendations identified in the report. Date: 15 Jan 2014 Version: 1 Page: 5 of 10

6 3.2 Baseline report The baseline report is currently being developed and a proforma has been agreed and is being completed for the different aspects of the programme. The aim of the baseline report is to explore the detail around processes needed to inform the recommendation. The report will aim to describe the strengths, weaknesses, opportunity and risks of moving DRSSW. The full baseline report is aimed to be completed by the end of January. 3.3 Financial assessment The current budget for the programme is 6.3 million of which 2.45 million is allocated directly to Health Boards for ophthalmology referrals. An important aspect of this project is to assess the financial risk to Public if DRSSW was transferred. The assessment will need to include whether any additional funding would be required to implement the recommendations from the External Quality Assurance, and if so quantify the likely additional resource required initially and on a recurring basis. 3.4 Communication A communication plan has been developed and regular monthly bulletins have been sent out to staff of both screening division and DRSSW via . A wide stakeholder list has been developed and an initial bulletin has been sent out in Jan 2013 explaining the project. A webpage to host the bulletin and initial documentation is being developed. A generic address has been established to provide a mechanism for feedback from stakeholders. 3.5 Definition of Governance of the Programme At the Project Board on the 16 th December a paper was received which proposed the governance arrangements of the programme if DRSSW was to be managed by. The board approved in principal that the DRSSW screening programme would be responsible up to the point that a patient is identified with referable diabetic retinopathy and confirmation of acceptance of referral has been received from the relevant Health Board. This definition will be used to scope the project and it has been agreed that work to review the service level agreements for the hospital eye services will be lead by Welsh Government, with support from Public as appropriate.
