2 INVITATION TO BID BOARD OF EDUCATION OF LINCOLN COUNTY, KENTUCKY EQUAL OPPORTUNITY/AFFIRMATIVE ACTION EMPLOYER 305 Danville Avenue Stanford, KY DRUG TESTING SERVICES FOR August 1, 2018 July 31, 2019 The Lincoln County Board of Education invites you to submit a sealed bid on drug testing services in accordance with detailed specifications and general conditions of the bid. Please read the invitation carefully to bid. Failure to comply with these instructions may disqualify your bid. Bids should be calculated on random drug testing of 25 certified employees, 25 classified employees, monthly random testing of 7 school bus drivers and 10 students per month for 10 months. Bids must be mailed or delivered to Marsha Abel, Lincoln County Board of Education, 305 Danville Avenue, Stanford, KY 40484, in a sealed envelope marked DRUG TESTING SERVICES BID in the lower left hand corner. Copies of this invitation to bid may be obtained from the Finance Office, at the above address, between 8:00 a.m. and 4:00 p.m. Monday through Friday, prior to the time and dates specified for opening. PERIOD OF CONTRACT: The contract shall be for the period August 1, 2018 through July 31, 2019 TIME OF OPENING Proposals will be opened at 11:00 A.M. May 30, 2018 or as soon thereafter as possible. None will be considered thereafter. WE DO NOT ACCEPT FAX PROPOSALS. The Board of Education cannot assume the responsibility for any delay as a result of the mails to deliver bids on time. A recommendation will be made to the Board at the regular monthly meeting of the Lincoln County Board of Education, held on June 14, 2018, at which time the bid will be awarded to the appropriate bidder. 3 LEGAL NOTICE INVITATION TO BID DRUG TESTING The Lincoln County Board of Education will accept sealed bids for drug testing services for students, staff and bus drivers. Specifications and bid packets may be obtained from Marsha Abel, Finance Officer at Lincoln County Board of Education, 305 Danville Avenue, Stanford, Kentucky between 8:00 A.M. and 4:00 P.M. Monday through Friday. All bids must be received by 11:00 A.M. May 30, 2018 at which time they will be opened and read aloud. The Board reserves the right to accept or reject any and all bids. 4 LINCOLN COUNTY BOARD OF EDUCATION REQUIREMENTS/SPECIFICATIONS FOR DRUG TESTING VENDORS 1. Required specific services must include, but not limited to: Onsite urine specimen collections by trained and experienced collectors. All specimens are to be shipped overnight to a SAMSHA approved forensic laboratory. Each specimen will be submitted to a standard eleven (11) panel drug screen (for staff and students and randomly chosen bus drivers) Any positive laboratory screen will be automatically confirmed with a GC/MS confirmation test. Management of a computer generated random testing program for student testing. Certified Medical Review Officer (MRO) review of all positive laboratory tests before final reviewed results to designated school contact. Any Supervisor and Educational Training as requested and required by client. Management of a blind specimen program as a further ongoing quality check of laboratory testing performance and accuracy. Development of any summary reports, progress reports, or other reports requested by client. On going consulting with Board of Education and designated school administrators on ways to make the testing more effective. 2. Vendor Qualifications required, but not limited to: All personnel will be fully trained and familiar with federal workplace testing standards and DATIA training requirements. All specimens collected will be tested by a laboratory SAMSHA certified by Federal Department for Health and Human Services. Any positive laboratory results will be confirmed by GC/MS and fully reviewed by an experienced and certified Medical Review Officer before being reported to the designated school contact person. Results can be reported verbally, by fax, or electronically by from the MRO. Negative results will turnaround within 24 hours from the time of specimen collection. Positive results can take up to 48 hours to complete GC/MS lab confirmation procedures. In all cases, results will be expected to be fully reviewed by the MRO and reported back to the school system within 72 hours. PROCEDURES FOR URINE SPECIMEN COLLECTION, PROCESSING AND REPORTING OF RESULTS SECURE THE COLLECTION SITE: 1. Add bluing to the toilet bowl. 2. Secure the toilet tank lid with security tape. 3. Secure any other water sources (sink, urinal, etc) with security tape or shut off water to other sources. 4. Remove any materials that could be used to contaminate a urine sample (soap, disinfectant, cleaning products, etc). 4

5 COLLECTION STEPS: 1. The staff member will be asked to provide proof of identification with a photo ID. Students and their parents will be asked to sign a consent to test form prior to any testing. 2. The student will be identified by a staff member prior to giving a urine sample. 3. The Collector will initiate the Chain of Custody form: a. The name of the school system will be printed on the form. b. The account number and clinic number will be printed on the form. c. MRO Information should be pre-printed on the chain of custody form for staff. d. The form will list the individuals social security number or ID number. e. The form will indicate the reason for testing. f. The form will indicate which drug test is being performed. g. The collection site address will be indicated along with telephone and fax number. h. The form will indicate if there is a split specimen or not. 4. The individual will be asked to empty his or her pockets, remove unnecessary outer garments and leave them with the collector. Purses, briefcases and etc. are to left with the collector. 5. A sealed test kit will be unsealed and the specimen cup given to the individual. 6. The Collector will accompany the individual to the bathroom, where he/she will provide the specimen in the collection cup/bottle; and instruct the individual not to flush the toilet. 7. The Collector will remain outside the bathroom door. 8. The Collector receives the specimen from the donor. 9. The Collector will check specimen integrity for the following: 10. Temperature if not within an acceptable range, the donor will provide another specimen. 11. Volume 12. Signs of adulteration 13. The Collector will ask the donor to verify the bar-coded specimen ID number printed on specimen bottle matches the specimen ID number on the Chain of Custody form. 14. The collector will place tamper resistant security seals on the bottles. 15. The Collector will ask the donor to initial the bottle seals, indicating the sample is his/hers. 16. Staff will be asked to sign a certification statement that he/she provided the urine specimen that they witness it being sealed and the ID numbers are correct. They also will provide a day and evening phone number and birth date. 17. Students will be asked to verify that his/her name is correct and sign the donor certification. 18. The Collector will place the certifications and specimen in the plastic bag and seal the bag. 19. The student or staff member will receive a copy of the Chain of Custody form. ANALYSIS AND REPORTING OF RESULTS 1. All specimens will be shipped overnight to a SAMHSA certified laboratory. 2. Samples for drivers will be analyzed according to DOT procedures. 3. Negative results will be reported to the school contact person. 4. Confirmed positive results will be reviewed by a certified MRO for a possible medical explanation. 5. After review, all results will be reported to the school contact person. 6. Hard copies of all results will be mailed to the school contact person. PROCEDURES FOR TESTING 5

INTENT This bid is intended to award contract services for drug testing for randomly selected students, staff and bus drivers to a single vendor for the Lincoln County School System. A single vendor will be chosen and used for all drug testing throughout the district. 2. METHOD OF BIDDING/DETERMINATION OF LOW BIDDER The per test price for students, drivers and staff will be multiplied by the estimated number of tests expected to be performed for The following bid tabulation example shows how the Board will determine the overall lowest bid. 9 DRUG TESTING SERVICES BID FORM (Page 3 of 3) (Return this page completed) CONFLICT OF INTEREST It shall be a breach of ethical standards for any employee with procurement authority to participate directly in any proceeding or application; request for ruling or other determination; claim or controversy; or other particular matter pertaining to any contract or subcontract, and any solicitation or proposal therefore, in which to his knowledge: (a) He, or any member of his immediate family, has a financial interest herein; or (b) a business or organization which he or any member of his immediate family has a financial interest as an officer, director, trustee, partner or employee, is a part; or (c) any other person, business, or organization with whom he or any member of his immediate family is negotiating or had an arrangement concerning prospective employment is a party.
