Community Mental Health Practitioner (CMHP) Level 3/1 $66, per annum plus 9.5% superannuation and salary packaging Partners in Recovery East/ South East /VIC June 2017 To provide specialist services to customers, families and carers and teams in line with Mind s Model of Recovery Oriented Practice and according to the values of Mind. The services type is Support Facilitation/ Care Coordination, which provides recovery oriented services to people in the community who experience severe persistent mental health issues resulting in a range of complex needs. Partners in Recovery: The Australian government funded Partners in Recovery (PIR) Program is a joint initiative with the Eastern Melbourne Primary Health Network (EMPHN) which is the commissioning organisation for this initiative in the outer Eastern catchment area. PIR works with individuals along with their families and carers who experience severe and persistent mental health issues resulting in a range of complex needs. Mind provides Support Facilitators who co-ordinate wrap around support to individuals and their families. The key aim of PIR is to work collaboratively with a broad range of services as they relate to customer need and to ensure a co-ordinated and integrated response to the customer s recovery goals. The PIR program is transitioning to the NDIS, with the focus of the transition phase to support the NDIS roll-out. The Mind PIR team works across the Maroondah, Knox and Yarra Ranges LGAs.

3 Provide direct support to individual customers Co-ordinate direct support to individual customers Provide a central point to support families and carers Work with customers to enable them return to their place of residence and a meaningful life through supporting them through a staged approach to recovery: Welcoming and engagement Strengths identification and individual recovery plan development Skill and capacity development Engagement and maintenance of natural supports Service exit and on-going self-management support Support customers in a range of areas including: Understanding and managing clients own mental health Developing daily living skills and capacity for self-care Crisis and incident management Addressing stigma Having an awareness of trauma Managing issues associated with drug and alcohol issues Support to maintain or create meaningful activity through participating in community life, including education and employment and utilising public transport Purposefully engage with customers using techniques including: Brief intervention Motivational interviewing and coaching Family inclusive practice Trauma informed practice Conflict resolution Behaviour support for dual diagnosis Provide direct specialised services beyond those of the Community Mental Health Practitioner. Ensure that clients are at the centre of service delivery and receive a timely, coordinated response to their support needs. Assess individual client needs using PIR assessment tools in accordance with established assessment guidelines and best practice Identify and engage appropriate service system support for clients to meet their assessed needs. In collaboration with clients and their care and support teams, develop and implement shared PIR Action Plans that facilitate a clear, understanding of specific roles and responsibilities and common goals to support individual clients recovery. Work collaboratively with a broad range of services as they relate to customer s needs to ensure a co-ordinated and integrated response to the customer s recovery goals. Promote strong interagency relationships and ensure timely and accurate information sharing between clients, their care teams and the wider service system as required Co-ordinate the development and ongoing review of support, risk management and crisis management plans within the client s individual recovery plan Convene and facilitate regular case conferences to develop, monitor and update PIR Action Plans. Utilise brokerage funds as appropriate to customer needs and PIR criteria Where a case manager is not in place for a client, and where authorised, carry out case management duties on a short term basis with a view to identifying a substantive case manager as soon as possible. Involve carers, family and friends as identified by the client in work to support their recovery. Support family and carer roles through understanding their concerns and the provision of information, education and referrals Facilitate, as appropriate, the re-engagement and maintenance of family and carer relationships Work intensively with families and carers at the time of transition back to community

4 Co-ordinate and work with other Mind team members to support the client Supporting on-going development of PIR services Ensure team have a shared understanding of the client s individual recovery plan and Coach and mentor team in area of specialty Work collaboratively with the team as to ensure a coordinated and integrated response to the customer s recovery goals. Facilitate PIR referrals to central PIR intake as required. Contribute to the ongoing development of collaborative working relationships and shared learning within the Partners in Recovery Organisation. Engage with stakeholders and partner agencies to promote the PIR service, and improve service system responses to meeting the needs of PIR clients. Promote systemic change by identifying opportunities for service system improvement from individual client work and strengthening community partnerships to encourage innovative and collective solutions. Contribute to service mapping and gap analysis activity. Contribute to evaluation of the effectiveness of PIR in consultation with clients and their support networks. Access flexible funding resources available through the PIR initiative in line with relevant guidelines. Be flexible, and adapt to change as necessary in line with the continued development of the PIR initiative.

5 Reports directly to the Service Manager Core Requirements Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind Current valid Driver s License. Current National Police Record Check. Current Working With Children Check (Employee). 6 This position is full time fixed term working 38 per week until 30 June Working days will generally be Monday to Friday, working between the hours of 8am to 6pm as required. Please note that the work pattern may be subject to change to meet the needs of the service. You are able to salary package a tax free amount, which is currently up to $15,900 per FBT year, plus other items including meal and entertainment and accommodation. The amount available for salary packaging will be subject to Commonwealth legislation as it applies from time to time and on the basis that Mind will not incur any liabilities for FBT. Mind offers salary packaging through an outsourced provider (currently RACV Salary Solutions). Other conditions are as per Mind s industrial agreement. Employer superannuation contribution will be paid according to the legislative requirement. Appointment is subject to a six month probationary period of employment.
