Permssble Changes Durng the open season, elgble employees not enrolled n a Federal health plan may enroll. Employees such as casuals and substtute rural carrers are not elgble to enroll. (See , Old Postal Manual.) Employees already enro'ied may change plans, optons, from self only to self and famly, or any combnaton of these changes. New enrollments (new hres) and changes n enrollment permtted at tmes other than durng an open season may also be made n the usual manner between November 15 and November 30, However, make sure these new enrollments or enrollment changes are not dentfed as an open season change. Instead, show the event permttng the change n Part D of Standard Form 2809, Health Benefts Regstraton Form. The Table of Permssble Changes on the reverse sde of page 2, carrer copy of Standard Form 2809, lsts the events whch permt enrollment changes. The classfcaton of the event determnes the effectve date of the enrollment or change n enrollment. B. Automatc Dstrbuton to Installatons The supply centers wll make an automatc dstrbuton of the followng health benefts materal to all nstallatons: 1. Pamphlet BR , Open Season Instructons brochures of the two Government-wde plans brochures of employee organzaton plans. 4. FormBRl (P.S.), 1977 Federal Employee Bweekly Health Benefts Rates. Form BR (P.S.) shows the U.S. Postal Servce's health benefts schedule. The automatc dstrbuton of the materal lsted above wll be made to all nstallatons n suffcent tme for redstrbuton to employees for the open season. Do not send requstons or make nqures to area supply centers concernng ths materal. Prompt dstrbuton wll be made when the materal s receved. C. Comprehensve Medcal Plan Brochures Each postal nstallaton located n an area served by a comprehensve plan(s) s responsble for obtanng a suffcent supply of the plan's brochures drectly from the approprate plan(s). POSTAL BULLETIN contans a lst of comprehensve plans wth the name, address, and phone number of the person to contact for requestng brochures. A supplemental lst wth nformaton relatng to seven new comprehensve plans s contaned n BULLETTN 210%. Retan both lsts for future use n requestng comprehensve medcal plan brochures. D. Dstrbuton of Materal to Employees /. To All Elgble Employees Installaton heads must gve each elgble employee the followng open season materal as soon as t s receved from the supply centers. (Comprehensve medcal plan brochures, tem c below, must be requested drectly from the plan): a. BRI , Open Season Instructons. Ths pumphlet must be ssued to each elgble employee to nform hm or her of the open season opportunty to enroll or change enrollment. b. BRI (P.S.), 1977 Federal Employee Bweekly Health Benefts Rates. c brochure of the comprehensve plan, f any, servng the partcular geographc area n whch the employee lves. d. BRI 41-25, 1977 brochure of the Governmentwde Servce Beneft Plan (Blue Gross-Blue Sheld). e. BRI 41-24, 1977 brochure of the Governmentwde Indemnty Beneft Plan (Aetna). 2. To Employees Upon Request a. Employee Organzaton Brochures. Do not make a general dstrbuton of employee organzaton plan brochures. However, such a brochure must be furnshed f an employee asks for t. The Cvl Servce Commsson, wth assstance from employee organzatons, s responsble for malng employee organzaton plan brochures to each employee who s already a member of an employee organzaton that sponsors a health plan, whether or not the employee s enrolled n the partcular plan. b. Standard Forms 2809 and A. (1) SF 2809, Health Benefts Regstraton Form, should be gven to an elgble employee who asks for t to enroll or change enrollment. (2) SF 2809-A, The Federal Employees Health Benefts Program, provdes nformaton about the program and should be gven to employees who request t. c. BRI , Informaton to Consder n Choosng a Health Plan, contans nformaton to assst an employee n selectng a health plan. It should be avalable to employees upon request. E. Open Season Procedures /. Tmely Regstraton To be consdered tmely fled, an open season enrollment or change n enrollment (SF 2809, Health Benefts Regstraton Form) must be receved n the employng nstallaton no later than November 30, Installaton heads have authorty to accept and process a late regstraton f they determne under old Postal Manual that the employee was unable to tmely enroll or change enrollment for cause beyond hs or her control. Falure to receve the open season materal durng the open season s cause beyond control f the employee apples to change regstraton wthn a reasonable tme after the materal becomes avalable. See old Postal Manual , Late Regstraton. 2. Late Regstraton Belated open season changes accepted by nstallaton heads should be so dentfed under Remarks on SF 2809 and properly documented n a memorandum attached to the offcal personnel folder copy of the SF 2809

A new open season enrollment (from not enrolled to enrolled) s effectve January 1, 1977, f the employee was n a pay status n any part of the precedng pay perod. If the employee was not n a pay status durng the pay perod before January 1, 1977, the enrollment becomes effectve on the frst day of the frst pay perod whch follows one n whch he or she was n a pay status. The enrollment of a new or newly elgble employee (such as a substtute rural carrer who s apponted as a regular rural carrer) who happens to regster durng the open season s effectve the same as for all new employees. b. An open season change n enrollment s effectve January 1, 1977, regardless of whether or not the employee was n pay status durng the precedng pay perod. c. An open season change belatedly fled and accepted s effectve on the frst day of the frst pay perod whch begns after January 1, 1977, and after the SF 2809 s receved n the employng postal nstallaton. The requrement of havng been n pay status durng the precedng pay perod also apples to belatedly fled changes from not enrolled to enrolled. 4. Separatng Employees a. If t s known that an employee wll transfer, retre, or separate before the effectve date of the open season change, do not process the open season SF Rather, the nstallaton head (or desgnee) should wrte hs or her ntals and the date the SF 2809 was receved n part F of the form to show that t was tmely fled. (1) In retrement cases, attach the unprocessed SF 2809 to other health benefts documents and process n the usual manner. (2) In transfer cases, return the form to the employee and nstruct hm or her to gve t to the new agency promptly when he or she enten on duty there. (3) In both retrement and transfer cases, the losng and ganng nstallatons must prepare transfer-out and transfer-n SF 2810's as usual, transferrng the old enrollment n effect at the tme of the employee's transfer or retrement b. If an open season change has been processed but the employee unexpectedly transfers or retres before the effectve date of that change, the losng nstallaton should (1) Vod all open season forms and transfer the exstng enrollment (f any) to the ganng offce. (2) Tell the employee that the open season change has been voded, and, f possble, have the employee complete a new SF 2809 and handle t as stated n paragraph 4a above. (3) If t s mpossble to take ths acton quckly, notfy the ganng offce that the employee's open season change whch was tmely fled, has been voded, and that a new open season SF 2809 must be accepted by the ganng agency. c. If an open season change has been processed, but the employee separates (ncludng separaton because of death) before the effectve date of the change: (1) Note n "Remarks," f the change n plans and the enrollment must be termnated, on all copes of the SF 2810 termnatng the enrollment "Separated (or ded) Open Season SF 2809 vod." In the case of separaton by death where the famly enrollment must be transferred to the retrement system because of a survvor enttled to annuty, vod the open season SF 2809 and transfer the old enrollment n effect at the tme of the employee's death. (2) Vod the open season SF 2809 f the change nvolved a change n plans. If the enrollment must be termnated, prepare SF 2810 termnatng the old enrollment, and note n "Remarks" on all copes of the SF 2810 termnatng the enrollment "Termnaton supersedes 'Change n Plan' SF 2810 (gve date)." If the famly enrollment s transferred to the retrement system because of survvors enttled to annuty, note n "Remarks" on all copes of the SF 2810 transferrng the enrollment "Transfer supersedes 'Change n Plan' SF 2810 (gve date)." Also, on the carrer's copy of termnaton or transfer SF 2810, gve the SF 2811 report number by whch the "Change n Plan" SF 2810 was sent. 5. Processng Open Season Forms Employees who wsh to make an open season change should do so as soon as they have the nformatonal lterature. Installatons should quckly and accurately process open season changes on a daly bass to approprate postal data centers so the data centers may send the changes to the nsurance carrers. The carrers can then ssue dentfcaton cards and verfy ther lablty to doctors and hosptals. Open season SFs 2809 generally should be processed the same as regstratons handled at other tmes. However, observance of the followng wll elmnate many of the mstakes that usually take place durng the open season. a. Show 1 (one) as the number of the event permttng the open season change n Part D of SF Enter Nov as the date of event whch permts change. b. If there s a change from one plan or opton to another, show the old enrollment code number n the approprate secton of Part D. c. If the employee changes optons or from self only to famly, but remans wthn the same plan, the employng offce should strke through the carrer's control number preprnted n the upper-rght comer of SF 2809 and nsert, n the space below that number, the old carrer's control number. Ths number may be obtaned from the most recent health benefts form n the employee's offcal personnel folder. No. SF 2810, Notce of Change n Health Benefts Enrollment, should be prepared n these cases. d. If the employee changes to a dfferent plan durng the open season, he or she acqures a new carrer's control number. The preprnted number n the upper-rght corner of the newly submtted SF 2809 becomes the new carrer's control number, and the old number should not be nserted n the space below the preprnted carrer's control number. In such cases, SF 2810 must be prepared by the employng offce to notfy the losng carrer that the employee has changed to another plan. In completng the SF 2810, the old carrer's control number must be used. Also, gve the effectve date of the acton. (Part A, Item 7) as the day on whch the new enrollment becomes effectve, and check Part C on the SF Do not send the orgnal of ths SF 2810 to the employee. Contnued on p. 4

Advce to Employees 1. Postal nstallatons are responsble for gvng nformaton to employees who ask for help on health benefts matters. Advce should be lmted to answerng questons about the health benefts program and the applcaton of health benefts law and regulatons to partcular crcumstances. The employee should be told to contact the local offce or representatve of the health plan on techncal questons relatve to benefts or converson (ndvdual) contracts. 2. If an employee submts SF 2809 to cancel an enrollment, the nstallaton should contact the ndvdual to make sure that he or she s sure of ths decson. The opportunty to enroll agan and the requrements for contnung an enrollment after retrement should be ponted out to the employees before the cancellaton s processed. 3. An employee who does not want to change plans or enrollment does not have to take any acton durng the open season and the current enrollment wll contnue. If the employee s enrolled and does not change enrollment, any changes n benefts and rates made by the plan automatcally apply effectve January 1, 1977, for postal employees. 4. Effectve January 1, 1977, The Western Clnc Plan, Code 561, 562, Tacoma, WA 98405, wll no longer be a partcpant n the Federal Employees Health Benefts Program. Employees enrolled n ths plan who desre contnued health benefts coverage are urged to enroll n another plan durng the open season. H. Provsons to capture hours of servce at the old and new rate wthn the pay perod are outlned n the followng paragraphs. PAY INCREASE NATIONAL AGREEMENT 1975 A. Processng Personnel Actons Personnel Servce Centers. Personnel actons effectve on or after November 21 must reflect the new salary rates. Personnel change actons affectng salares that are effectve November 21 thru December 3 and processed n PP must be submtted as a PDC Hold Out No Tape acton. The change wll be processed by the PDC as an adjustment n PP B. Tmekeepng Procedures /. Tmecards. The postal data center wll ssue a st>ecal tmecard, color coded and dentfed as PP 41-76, for all elgble Barganng Unt employees. Ths specal tmecard wll be used to adjust the salary for employees n a pay status (work or leave) on November 20. The specal tmecard wll be maled for recept at postal nstallatons no later than November 19. All daly entres for leave, work, Sunday, nght work, and overtme hours for the servce day November 20 wll be transcrbed from the normal PP 25 tmecard to the specal tmecard. The specal tmecard wll be approprately totaled and submtted to the respectve servcng PDC. The specal tmecards must be submtted for recept by November 24, Unused specal tmecards, PP should not be returned to the PDC. Do not alter the regular PP tmecard; these tmecards wll be completed for the entre perod and submtted n the usual manner. 2. Hgher Level Servce. Elgble Barganng Unt employees servng hgher level n a non-barganng (PES-PMS-PTAC Schedule) poston may requre submsson of two Forms 1640, Hgher Level Certfcate. One Form 1640 wll be requred to reflect hgher level servce on November 20 only. Ths Form 1640 must be dentfed n bold prnt as PP November 20,1976. The Form! Use Form 4056, Your Malbox Needs Attenton, and Notce 38, Snow Removal by Rural Safety Alert Steel Typst Chars The Postal Servce has been advsed by GSA that some steel typst chars dentfed by the followng natonal stock numbers consttute a potental safety hazard: O The hazards occur when the spndles of the rotary mechansm of the chars are threaded full length. Spndles elevated full length cause extreme pressure on the plastc hub at the base of the char. Ths causes the plastc bushng on the char to snap, and the char to collapse. All chars of the above NSN's and further dentfed wth the name Inter Royal on the char (bottom or back) should be removed from servce and the followng correctve acton taken: The spndle should be removed from the base, and the handwheel spun upward to clear the bottom thread. A thread n the spndle located 2 y 2 nches from the bottom of the spndle should be mpacted wth a blow from a hammer and drftpn (blunt chsel or punch). Ths upset thread wll create the necessary adjustment range stop for the handwheel. The char can then be returned to servce.
