Employees in this position may also handle some Child Protection (Family Assessment) cases or Child Welfare (Truancy) cases in an overflow capacity. Reports to: Social Services Supervisor Supervises: None, but may provide work direction to Family Service Aide, Case Aide, or other staff as needed or directed. Dimensions: General Scope of Work: Under limited direction, provides social work services on cases involving complex social problems, develops treatment or service plans, performs responsible and difficult casework, directs service programs or provides work direction to paraprofessional staff, performs related work as assigned. Budget: Employee does not directly control any budget allocations, however case planning and service decisions will have an impact on various departmental allocations (waiver funds, foster care budget, etc.). Supervisor should be consulted prior to authorizing services that are normally outside of the function sof the specific person or are more costly than usual for that particular service item. Clientele: This position will work primarily with emotionally disturbed children and their parents, but any agency social worker may have contact with, and must have the skills to work with, a wide variety of highrisk, marginalized, underserved, or otherwise vulnerable populations, both adult and children.

2 II. Principal Responsibilities; Priority; Time; Discretion; Performance Indicators A. Initial Assessment/Eligibility Determination DHS-876c PZ Refer/Coordinate Diagnostic Assessment 2. Complete Functional Assessment 3. Determine eligibility and appropriateness for case management services 4. Complete all necessary forms and documentation in a timely manner per current Minnesota statute and policy 5. Coordinate with stakeholders, collaterals, referral sources as needed Discretion: B Time: 10% B. On-going Case Management 1. Assist client in maintaining stable mental health status and living in the least restrictive setting possible 2. Develop service plan (IFCSP) in conjunction with client/parents/guardians, within required timelines 3. Provide/coordinate services as outlined in plan; adapt plan and respond to client change in need in a flexible manner 4. Attend necessary client meetings 5. Conduct face-to-face visits with children at least monthly 6. Handle and respond to crisis situations as needed 7. Participate in CHIPS process when necessary; follow up on all required court documentation in required timeframes 8. Schedule/coordinate client mental health appointments as needed depending on client/parent level of independence 9. Insure that all necessary forms for public assistance are completed in a timely fashion 10. Follow all current state permanency timelines and IV-E procedures if child does end up in placement Time: 80% C. Back-Up for Child Protection Family Assessments 1. Conduct Family Assessments within designated timeframes.

3 2. Determine if on-going CPS services are needed 3. Transition case to on-going CP worker as needed Time: 5% Priority: B D. Truancy Case Management 1. Assess child/student situation to determine most effective course of intervention for each individual; coordinate and provide referrals as needed (ex: chemical dependency evaluation, mental health evaluation, etc.) 2. Develop service plan in conjunction with parents, truant teenager, and other appropriate stakeholders within required timeframes 3. Coordinate in an on-going capacity with stakeholders, collaterals and referral sources as needed. Schools are an especially important stakeholder in these cases. 4. Provide on-going case management; coordinate, arrange and monitor service provision; track and evaluate progress toward goals; adapt plan and respond to client change in need in a flexible manner 5. Conduct face-to-face visits with children at least monthly 6. Complete all necessary documentation (SDM tools, child well-being tools, mental health screening tools, placement entry, VPA form, case notes, etc.) as required by SSIS, Minnesota statute, and federal Children and Family Service Reviews within required timeframes. 7. Report to the court as needed in a timely manner 8. Follow all current state permanency timelines and IV-E procedures if a truant child does end up in placement 9. Handle and respond to crisis situations as needed Time: 5% E. General Employee Responsibilities 1. Abide by county policies; adhere to work rules, etc. 2. Attend staff meetings and approved trainings. 3. Provide coverage for other agency staff/cases as assigned 4. Stay updated on policy/procedure changes; implement changes as directed in bulletins, memos, staff meetings, s, etc. 5. Attend meetings and/or be a presenter for those meetings as may be assigned. 6. Accurately complete computer and/or paper documentation forms in required timeframes. This may include, but is not limited to data entry in the following programs: SSIS, MMIS, CCM. 7. Provide supervised visitation or transportation as necessary. 8. Contribute to a cooperative, supportive, teamwork environment. 9. Demonstrate on-going commitment to social work best practice guidelines.

4 10. Establish and maintain positive working relationships with clients and stakeholders. 11. Make thoughtful contributions to agency decision-making processes. 12. Participate in internal and external conflict resolution in a professional, respectful manner. 13. Other duties as assigned. 5 III. Employee s Name: DHS-876c PZ Machines or equipment used regularly in this position: Telephone, calculator, computer, printer, copy machine, fax, automobile or van, child car seats. Must be able to lift approximately 50 pounds (i.e. normal office equipment, client belongings, small children.) 2. Freedom to act and problem solving: Social Workers are given considerable latitude in dealing with complex social problems and meeting the service needs of clients within the general guidelines of state statute and rule, agency policy, and budgetary limitations. 3. Who reviews your work? Formal review is conducted by the Social Services Supervisor, but input is sought and expected from stakeholders in the service system (ex: agency social workers, county attorneys, service providers, guardians ad litem, treatment providers, clients, guardians, foster parents, etc.) 4. Knowledge, skills and abilities: Knowledge of the principles and techniques of interviewing and recording in social work and the ability to apply them. Knowledge of individual and group behavior. Knowledge of social casework objectives, principles, and methods and the ability to apply them. Knowledge of social-economic factors, which promote stable family life and understanding of the elements, which affect family security. Knowledge of physical and mental illness and their impact on personality. Knowledge of social welfare research methods. Knowledge of family systems and dynamics. Ability to work constructively with community agencies to help clients utilize appropriate resources and services. Ability to work constructively in the development and coordination of community resources to meet special needs. Ability to manage time productively. Ability to prioritize caseloads.
