1 Third Party Report for TLIF2010A Apply Fatigue Management Strategies Instructions to Candidate and Supervisor 1. AVELING is required to gather and assess evidence of the candidate s skills and knowledge as part of the assessment process for this Unit of Competency; this Third Party Report will assist to achieve this. 2. The Third Party Report should be completed by a supervisor, employer or someone with appropriate industry experience who can comment on the candidate s ability to perform the relevant skills or have the relevant knowledge noted in this Report. In this form, such a person is called the Supervisor. 3. The Supervisor should not be a relative, close friend or someone who could have a conflict of interest. 4. If the Supervisor believes the candidate needs to further develop their skills, please make the appropriate comments on this form. 5. AVELING may contact the Supervisor (by phone) to clarify the contents of the Third Party Report. Where this is necessary, the Unit of Competency will not be issued until this has occurred. 6. AVELING will use the information in this form as well as online course and assessment questions (which candidate has already completed on their own) to assess whether the Candidate is competent for this Unit. AVELING cannot finalise completion of this course nor award the Statement of Attainment until the Third Party Report is received and assessed by a Qualified Assessor. Submitting this form does not guarantee issuance of the Statement of Attainment for this Unit. A Qualified Assessor at AVELING will review it to deem competence. Describe at least 2 different situations where the candidate has applied fatigue management strategies to manage their fatigue at work. The situations should demonstrate the person can do this in different work conditions over a time period. Examples: night-time driving, poor-weather driving and long shifts. 2. Provide examples demonstrating the candidate can identify conditions that lead to workplace fatigue. Examples: they take rest breaks and understand why to take them, they stay hydrated, they recognise the difference between working at night-time and daytime, they recognise that personal factors such as stress, an unhealthy lifestyle and poor sleep hygiene can lead to fatigue. Give examples where the candidate demonstrated they could work effectively with colleagues (including schedulers, managers, team members, clients and/or others) to manage and minimise the effects of fatigue whilst working. I.e. they communicate with appropriate persons when they re having fatigue-related issues or concerns. 4. Give examples that demonstrate the candidate understands how to adjust their lifestyle patterns to ensure they ll be able to manage their fatigue at work. Examples: resting prior to coming onto shift, getting a good night s sleep before work, adopting a healthy lifestyle e.g. eating healthy, not smoking, doing sport. 5. Describe situations when the candidate recognised symptoms of fatigue and took actions to minimise these effects, in accordance with fatigue management regulations and workplace procedures. Examples: stopping for a rest break if feeling tired, getting fresh air, staying hydrated, stretching, declining tasks that may put them at risk due to fatigue. 6. Identify occasions where the candidate demonstrated their knowledge of the relevant codes, regulations, permits and licence requirements related to fatigue management. Examples: identifying maximum work/rest times, understanding their responsibilities for working safely and to others, making sure they filled in work diaries and kept them in accordance with regulations.
