The questions are about best practices that affect key health behaviors like tobacco use, physical activity, and nutrition. There are six sections in the survey: Worker/Worksite Characteristics, Health Insurance Benefits, Worksite Wellness Committees, Worksite Policies, Employee Programs, and Communication.
