1 Request For Information (RFI) Addendum #1 November 3, 2014 RFI Number: RFI Services: Comprehensive Evaluations UNSPSC Code: Subject: This Addendum contains questions submitted by interested parties and the Department s answers to all questions. Questions are presented in exact manner received. QUESTIONS FROM PSYCHOLOGICAL ASSESSMENT AND TREATMENT SERVICES, LLC RECEIVED 9/5/2014 Question # 1: After some internal discussion amongst ourselves, we are wondering, has the department considered the possibility of restructuring some of the aspects of the assessment? Answer #1: The Department will be restructuring the evaluation specifications and will release a revised scope of services. All answers are based on the present scope within the RFI. Question # 2: Answer # 2: Question # 3: Answer # 3 Question # 4: Answer # 4: Specifically, is there an interest in using the psychologists within the framework of what a psychologist does, that being testing and test interpretation. As indicated in the RFI, Exhibit 4, a licensed psychologist may be utilized for administration of Comprehensive Mental Health Evaluations, Comprehensive Substance Abuse Evaluations and Psychological Evaluations. A licensed psychologist meeting the requirements set forth in section , F.S., and Rule 64B F.A.C., may also be utilized for administration of Psychosexual Evaluations. Would the department consider revising the requirements of ancillary details such as various interviews, psychosocials, and collateral documentation to be performed by separate entity prior to the testing? The Department will be restructuring the evaluation specifications and will release a revised scope of services. Another issue is the consistent overhead of maintaining a compliment of rather costly test instruments. Would the department be willing to consider a centralized bank of these tools, purchased by the department for contractors to relieve evaluators from this cost? The Department cannot maintain a centralized bank of test instruments as Providers have several instruments that can be utilized. The cost of the tools should be considered when establishing prices for the services requested. Question # 5: Finally, we would like clarification regarding the cut off for IQ testing at 79 instead of 70 as per the DSM-IV Manual. We have been told that if a youth scores below 79 on FSIQ that we have to proceed with an assessment of adaptive functioning. Answer # 5: That is correct. The Department is revising the structure of services; however, youth with FSIQ of 79 or below are required to receive an assessment of adaptive behavior functioning to determine his/her eligibility and need for placement in a DJJ residential program designated to provide Developmental Disability (DD) Services for youth with intellectual disability (IQ < 70 with adaptive behavior deficits) or a residential program designated for Borderline Developmental Disability (BDD) Services for youths with an IQ of and adaptive behavior deficits. 2 Question # 6: Answer # 6: Question # 7: Are the services needed in a specific county or throughout the state? The purpose of the RFI is to determine the availability and interest of qualified vendors throughout the State; however, services are needed in a county or circuit basis. In addition, the RFI is requesting the Provider to detail any concerns with the specifications. QUESTION FROM TARCHA RENTZ, PH.D. RECEIVED 9/22/14 Who are the current/previous service providers? Answer # 7: Currently, the following Providers provide contracted Comprehensive Evaluation services to the Department: Citrus Health Network, Florida Therapy Services, Dr. Marie Hume-Guilford; Psychotherapeutic Resources, Inc; Juliana Gerena; Kimberly Haga; Shanlis Counseling And Assessment; Harmony Clinical Service, Inc; Dr. Lori Bourgard; Southwest Florida Psychological Services; Lee Barrow, LLC; First Coast Management Services, Inc.; Cordova Counseling Center; NCS Counseling & Development; and Daniel Kitzerow M.A. QUESTIONS FROM POSITIVE SOLUTIONS PSYCHOLOGICAL SERVICES RECEIVED 9/29/14 Question # 8: How many children are referred for comprehensive evaluations each week? Answer # 8: Utilization varies from Circuit to Circuit and there is no way for the Department to estimate the number to be referred. There is no guarantee that any specific level of evaluation will be requested even if a contract is executed. Question # 9: Answer # 9: Question # 10: Answer # 10: How many vendors conduct the comprehensive evaluations? If multiple vendors, how does the Department choose which vendor conducts the evaluation? The purpose of the RFI is to determine the availability and interest of vendors statewide and the Department will make a determination on the method of procurement to be utilized. If there is a need for multiple vendors within an area based on utilization, the determination is made based on the urgency of the completion of the evaluation(s), coupled with which prospective Provider is capable of providing the services most expeditiously. For example, Provider A may have no availability due to caseloads, while Provider B has availability to provide the services, resulting in the utilization of Provider B. Are all evaluations conducted at the Juvenile Detention Center? Please see Attachment I, Section III. D. 1., of the RFI. The Provider shall make its principal or qualified employees available to interview and assess youth in locations (within Circuit XX) convenient to the youth and family, including but not limited to the Provider s office(s), the youth s home/school, juvenile detention centers, hospitals, juvenile probation offices, juvenile assessment centers or community centers. Question # 11: Answer # 11: If chosen as a vendor, when will we begin providing services? The RFI is a request for information to determine availability and interest of prospective Providers statewide, not an offer for a contract. Question # 12: Do we have to re-apply each year to continue being a vendor? Answer # 12: No. Please note the RFI is not an application process. The RFI is a request for information only and does not constitute a contract offer or commitment by the Department for a future contract. QUESTIONS FROM POSITIVE NEWVISION BEHAVIORAL HEALTH RECEIVED 9/29/14 Question # 13: Would these contracts for comprehensive evaluations also be open to sole practitioners who are looking to do what I think you call a "rate agreement"? Answer # 13: A rate agreement is utilized when utilization for services is less than $34, annually, and is dependent upon a variety of factors. There is no difference in the service delivery between a rate agreement and a contract for services. All vendors who have qualified staff would be eligible to respond to any solicitation. 3 Question # 14: Answer # 14: Are providers also paid to conduct home studies for the offenders you serve? No. Home studies are not included in these evaluations. Question # 15: Answer # 15: Question # 16: Answer # 16: Question # 17: Answer # 17: Once a comprehensive evaluation is completed, are the licensed therapist also able to contract to provide psychotherapy to the individual if deemed appropriate and or feasible, or do you like to have different practitioners conduct the therapy? If so, how many sessions are usually authorized for therapy? The RFI is seeking information about the three levels of Comprehensive Evaluations only, not psychotherapy. Comprehensive Evaluation contracts do not authorize any psychotherapy to be provided in most circumstances. However, if the youth is to receive DJJ contracted community based services, the services should be provided by an entity other than the Comprehensive Evaluation Provider because of potential conflict of interest issues. Does DJJ contract for psychotherapy services alone, once an evaluation has been completed by another provider/agency? Comprehensive Evaluations are required under section , F.S., for any youth for whom a residential commitment disposition is anticipated or recommended. Thus, many of the youths receiving Comprehensive Evaluations will be receiving services in a DJJ residential program. If the youth remains in the community, the youth may receive DJJ contracted community based services. What are the needs of the offenders that you usually serve? Do you ever contract for vocational services for your youth offenders? Probation youth have a variety of needs to be provided within the community. Yes, the Department has recently completed a Request for Proposal for a Vocational Services Program in the Central Region for probation youth, which was awarded to Home Builders Institute, Inc. (HBI). Question # 18: How quickly does an offender need to be seen once a provider has been contacted for services? Answer # 18: Comprehensive Evaluations shall be completed and the Evaluation Report delivered to the Department requestor within ten (10) business days from receipt of the referral by the Provider. QUESTIONS FROM FLORIDA THERAPY SERVICES, INC. RECEIVED 9/30/14 Question # 19: Page 11 of 49 indicates that Level 1 assessments may include educational testing and review and in-depth vocational testing and analysis, but this is not a component of our current contract for level 1 evals. Current contract covers in a level 2 eval (vocational) and 3 eval (educational). In fact, our current vocational assessments are very brief and limited. Will Level 1 evaluation reimbursement rates increase to cover these components? Answer # 19: A Level I Evaluation is to include the Vocational Evaluation Protocol which requires administration of a Career Interest Inventory, and the Academic Evaluation Protocol which requires administration of a Learning Style Inventory. Comprehensive Evaluations. Upon finalization of updated specifications, the method by which rates will be established will be determined by the Department. Question # 20: Answer # 20: Also, on page 12 0f 49, the contract states that a Level 1 eval shall include a mental health and/or substance abuse evaluation per the protocol in attachment II. That attachment lists a number of required tests be completed that are normally only a part of our level 2 evals. Will reimbursement rates for Level 1 evals be increased to cover increased cost of testing? Comprehensive Evaluations. Upon finalization of updated specifications, the method by which rates will be established will be determined by the Department. Page 3 of 9

On pg. 11, proposal states may include. Pg. 12 states shall include. Thank you for pointing out the inconsistency. The Department is restructuring the services from the current specifications for Level I, II, and III Comprehensive Evaluations and this language will be corrected. Page 12 of 49 section b: Level 2 evaluation must include Psychological Evaluation. Can a school psychologist complete who are trained, however may not ne Licensed as a Psychologist? A licensed school psychologist could not administer the Psychological Evaluation set forth in the RFI on Attachment IV. However, section , F.S., indicates that a licensed school psychologist could administer academic, vocational, and psycho-educational assessments that relate to learning and educational needs. Please note, however, the Department is currently restructuring the services from the current specifications for Level I, II, and III Comprehensive Evaluation. Page 12 of 49: Level 2 evaluation shall include administration of most recent edition of the Wechsler. Standardized adaptive behavior measure We usually do this as part of a level 3 assessment. Will Level 2 reimbursement be increased to cover cost of this evaluation? Comprehensive Evaluations. Upon finalization of updated specifications, the method by which rates will be established will be determined by the Department. Page 15 of 49 section c: When the above information is not furnished to the Provider by the Department and is necessary to complete a thorough Comprehensive Evaluation for the youth, the Provider shall be responsible for collecting information from appropriate entities, the youth/parents, or the Department? This is problematic. This stipulation would allow for the burden of collecting the documents to be shifted from the JPO to the provider? Will additional time and reimbursement be allowed? Comprehensive Evaluations and will review this specification. The Provider should be communicating with the JPO and Circuit staff if all documents are not received. Additional time and reimbursement will not be authorized, as most court systems require the Comprehensive Evaluation Report within ten (10) business days. Page 16 of 49 section 2e: The Provider shall complete an evaluation report on academic performance and make recommendations related to classification and placement. This would seem to be beyond the scope of the Comp Eval? Are we to do ESE testing which is currently done by the School system? ESE testing is not included as part of service delivery of Comprehensive Evaluations. As indicated on page 14 of 49 of the Attachment I within the RFI, (Goal of Services). The Comprehensive Evaluation guides the Department to the right placement and treatment for the youth based on his/her unique characteristics and needs. Thus, the Educational Evaluation component of the Comprehensive Evaluation should provide recommendations regarding DJJ classification and placement of the youth as indicated by educational needs identified in this component of the evaluation. Page 17 of 49, section C: the Provider shall ensure its subcontractors make their principal or qualified employees available to interview and assess youth in locations convenient to the youth and family, including but not limited to, the home, schools, juvenile detention centers, hospitals, juvenile probation offices, juvenile assessment centers or community centers. Some of these locations are not conducive to testing, and may be out of the county and/or District geographically served by provider. If this is the case, will DJJ transport youth to provider? The Department will work with the Provider to ensure the youth is available for testing at a location convenient for the Providers. Such situations should be coordinated with the JPO Page 4 of 9

5 making the referral. Question # 27: Answer # 27: Question # 28: Answer # 28: Page 18 of 49 section 2a: Staffing Qualifications a..the term Florida licensed or certified professional as used above includes: a psychiatrist licensed under chapter 458 or 459, Florida Statutes who is Board Certified in child and adolescent psychiatry or psychiatry by the American Board of Psychiatry and Neurology; a licensed psychologist or school psychologist under chapter 490, Florida Statutes. What can school psychologist do versus licensed MH counselor versus licensed psychologist? Section , F.S., indicates that a licensed school psychologist could administer academic, vocational, and psycho-educational assessments which relate to learning and educational needs. Thus, a licensed school psychologist could administer the RFI Vocational Evaluation Protocol and Academic Evaluation Protocol including IQ and adaptive behavior testing related to learning and educational needs. Pages 22 and 23: List a number of new reports that must be made to DJJ, many of which are montlhly. Will there be any reimbursement for administrative cost included in contract? No, all prices are inclusive of all work performed. However, since this is a Request for Information, please identify in your RFI response any cost breakdowns that will allow the Department to receive the best pricing available for these services. Question # 29: Answer # 29: Page 24 of 49: Performance Measures/Evaluation 1. GOAL: 100% of the Comprehensive Evaluations shall be completed and the Report delivered to the Department requestor within ten (10) business days from receipt of the referral by the Provider consistent with the requirements of this Contract. MEASURE: This will be measured by reviewing the date of the referral and the date of the Comprehensive Evaluation Report. MINIMUM STANDARD: 100% of the Comprehensive Evaluations shall be completed and the Report delivered to the Department requestor within ten (10) business days from receipt of the referral by the Provider. What if our comprehensive assessments were not provided to DJJ within ten working days regardless of well documented extenuating circumstances beyond our control, such as absconded youth, repeated broken appointments by youth/ parent, youth refusal, inability to make contact with the parent, and lack of access to the youth? Is there process to negotiate language of this goal to include attempts made and issues out of providers control so provider is not deficient in report for outcome measures out of their control? The Department will reduce the minimum standard to 95% to allow for any extenuating circumstances. This will be set forth in the revised specifications in the future. Any contracted Provider should also document circumstances and reach out immediately to the JPO should a youth fail to keep a scheduled appointment. If a current Provider has this level in their contract, they should request a contract amendment from their Contract Manager. QUESTIONS FROM COMMUNITY PARTNERS IS HOME TO: HOUSING PARTNERSHIP, INC. / PARENT-CHILD CENTER, INC. RECEIVED 9/30/14 Question # 30: Are we allowed to see copies of other bids from the past? Answer # 30: This service was procured through an RFP # The records/files pertaining to this solicitation are public documents. Comprehensive Evaluations. Upon finalization of updated specifications, the method by which Page 5 of 9

6 rates are established will be determined by the Department. Question # 32: On page 10 of the RFI, under 2a, Level 1 Evaluation Midway through the paragraph it states This level evaluation shall include all of the components listed in paragraph 1 below, including an in-depth Mental Health Evaluation and/or Substance Abuse evaluation and MAY include educational testing and review and in-depth vocational testing and analysis. The question is: In the past this level 1 evaluation did not require IQ, educational and/or vocational testing. This meant that Master s level/licensed clinicians could perform this Level 1 evaluation. If every Level 1 evaluation needs this testing it must be performed by a psychologist as Master Levels/Licensed clinicians are not permitted to complete this testing. Can you let us know if all Level 1 evaluations will require this testing or just when requested.? My agency wants to be clear if we would be able to utilize Master s level/licensed clinicians to complete this level evaluation should we be chosen by the Dept. Answer # 32: Page 11 of 49 and on page 28 of 49 of the RFI, provides examples of abbreviated IQ tests which could be administered by a Master s level or licensed professional meeting the qualifications set forth in the RFI, Exhibit 4 for Comprehensive Mental Health Evaluations. QUESTIONS FROM COLLEEN D. CHARACTER, PH.D. RECEIVED 10/1/14 Question # 33: How are vendors selected? Answer # 33: Depending on the amount of utilization needed and dollars to be expended, Providers may be selected through a variety of methods. Question # 34: Answer # 34: Question # 35: Answer # 35: Question # 36: Answer # 36: Should a vendor submit a vita? No, not at this time. The Department will request any specific documentation when the decision to enter into a contract is made. The staffing documentation requirements are provided so vendors will understand the required documentation for each type of professional license conducting specific elements of an evaluation. How many vendors are selected per county? The Department has not made any determination regarding utilization needs and number of vendors at this time. This is just a request for available and interested vendors to maintain an interested parties list. Do vendors have to bid on contracts? Vendors will be notified as to any bidding or quote requirements at the time a decision regarding future service needs are finalized. Question # 37: How do we register to bid for a contract? Answer # 37: All vendors should be registered in MyFloridaMarketPlace (MFMP) at and for the Vendor Bid System notifications at In addition, the Department will put all vendors who indicated interest on an interested parties list for future notifications of contracting opportunities in the areas a vendor has identified they are able to offer services. Question # 38: Answer # 38: Question # 39: Answer # 39: When will vendors be notified if they are selected? This is a Request for Information only, not a solicitation document or offer of a contract. Who assigns the juveniles to be evaluated to the vendor? The Department refers the youth for evaluations and some evaluations are court ordered. Page 6 of 9

7 Question # 40: Answer # 40: Question # 41: Answer # 41: Question # 42: Answer # 42: When are reports due? Comprehensive Evaluations Reports are due within ten (10) business days of referral. Who should reports be submitted to? The Juvenile Probation Officer making the youth referral and the Contract Manager. What must be submitted for payment? A complete list of youth who were provided services during the service period detailed on the invoice shall be furnished. At a minimum, the Report of Youth Provided Services shall include the youth name, juvenile justice identification number, date of referral, date of evaluation, date report submitted, and the service required by the Contract that was provided. Question # 43: Who should payment be submitted to? Answer # 43: The Department s Contract Manager on a monthly basis. QUESTIONS FROM HELPING HAND COUNSELING AND CONSULTING SERVICES OF THE PALM BEACHES RECEIVED 10/8/14 Question #44: I know this is a RFI but would like to know if the department will be moving in the direction of contracting several provider in a circuit than the current system where one entity is contracted for all services in the area. Answer #44: The Department is considering their need for multiple Providers within a Circuit. If a Provider cannot handle the utilization needs or has other problems with delivering services, the Department must have back-up service delivery methods. Question #45: What is the likelihood that the Department will consider separate providers for the three evaluations? That is one provider will only be given one or two and another have the third. Answer #45: Comprehensive Evaluations. Providers are encouraged to subcontract for services they cannot provide directly through their staff. QUESTION FROM TAYLOR COUNTY RECOVERY CENTER, INC. - RECEIVED 10/8/14 Question #46: I want to know if the GAIN (Global Assessment of Individual Needs) is an acceptable assessment tool for Level 1 & 2 substance abuse. Answer #46: Based on the present scope, Attachment II, Substance Abuse Evaluation Protocol noted on G3, within the RFI provides for the Substance Abuse Evaluation to include administration of appropriate standardized assessment instruments. The Global Appraisal of Individual Needs (GAIN) could be utilized as a standardized assessment instrument to supplement the Substance Abuse Evaluation. QUESTION FROM CORDOVA COUNSELING CENTER - RECEIVED 10/9/14 Question #47: Awards in certain districts have been made based on criteria in the original RFP. Now that the criteria may be changed, will there be an opportunity to submit new bids based on the revised criteria in districts where contracts have already been awarded? Answer #47: This is a Request for Information (RFI) not a competitive bid solicitation. QUESTIONS FROM GULF COAST YOUTH SERVICES - RECEIVED 10/9/14 Question #48: As currently written, does a provider need a licensed psychologist on staff or on contract, to perform Level II and Level III evaluations? Answer #48: Yes, as currently written; however, the Department is restructuring the services. Question #49: Answer #49: What are the current rates for Level I,II and III evaluations in Circuits I and 14? Level I at $ (All Providers), Level II at $ (All Providers) and Level III at $ and $ for (three (3) Providers) respectively. QUESTIONS FROM BRIDGEWAY CENTER INC. - RECEIVED 10/9/14 Page 7 of 9

8 Question #50: Answer #50: What are the requirements as a vendor? The Department doesn t fully understand this question. A Provider must comply with all the specifications of the Department (currently being revised). In addition, all Providers must be registered in MyFloridaMarketPlace at Question #51: Answer #51: Question #52: Answer #52: Question #53: Answer #53: Question #54: Answer #54: Is it for a designated area or all of one circuit? The Department is seeking services throughout the State. Providers are asked to identify their service areas, by county or Judicial Circuit. Can we use our own template? The Department does not understand the question. How many hours are authorized per assessment? The total number of hours to complete an assessment is not specified. Do the vendors have specific hours or do they make their own hours? Services shall be provided to youth during traditional and non-traditional business hours, including weekends. The Provider shall maintain an administrative office to receive Department referrals between the hours of 8:00 a.m. and 5:00 p.m., Monday through Friday, excluding State holidays. Question #55: Is there any additional paperwork required in addition to the Comp. assessments? Answer #55: There is additional reporting and invoice requirements outlined in the Attachment I of the RFI. QUESTIONS FROM MENTAL HEALTH CENTER OF FLORIDA - RECEIVED 10/9/14 Question #56: Do you have a writeable template providers can use to submit the RFI or should we develop our own? Answer #56: You can provide your response to the questions posed in the RFI in any format you choose. You can request a word version of the RFI template from the Procurement Manager, Cheryl Beasley at Cheryl.Beasley@djj.state.fl.us Question #57: Answer #57: Question #58: Answer #58: Question #59: Answer #59: In the area of service locations. Do you want service areas (city/county) where we have physical office/ building locations, or service areas where we have staff who are willing to travel to sites and provide the services. All of the above. We are looking for service areas for related services. If your office is in one county/circuit, but you are willing to provide services in another county/circuit, please provide that information in your RFI response. What is the expect start date for service provision. The RFI is a request for information to determine availability and interest of Providers statewide, not an offer for a contract. Will the Department be open to considering revising the requirement of having the Licensed Psychologist present when the assessments are being conducted by Doctoral Level Interns and Doctoral Residents as long as interpretation and report development is directly supervised by the Licensed Psychologist. It is the norm in our field for Doctoral level interns and residents to competently perform these types of services under the supervision of a Licensed Psychologist without having to have the Licensed Psychologist be present during the clinical interview and/or administration of the assessments. The RFI, Attachment II Psychological Evaluation Protocol provides that the licensed psychologist Page 8 of 9

9 will conduct a face-to-face interview with the youth, interpret test results and sign the psychological evaluation. Answers are based on the presented scope and the Department is reviewing and considering alternatives to restructure the service delivery. Question #60: Answer #60: Will the Department be open to considering revising the 10 business day turn around to what is more of the norm and practice in the field of 20 days. A 10 day window period seems to be setting up a negative impact on the outcomes from the onset as it doesn t seem feasible to accomplish and attain all requirements within a 10 day period. Unfortunately, the Department is required by Judges in the Juvenile Court System to receive these evaluations as quickly as possible, so the ten (10) business day turn-around will not be changed. If a Provider has difficulty meeting this ten (10) day requirement, they should be working directly with the Circuit and JPO making the referral to document the circumstances and see what can be done to assist the Provider in accomplishing this goal. Question #61: Will the department consider creating a master checklist list of minimum requirements/documents/collateral information that must be attained by Probation Officer to be submitted as a referral packet (ex: school records, physical, copy of PACT and/or SAMH-2, previous evaluations, legal records, health records etc). Which then would make the ten (10) business day turn around more feasible. Answer #61: See Answer to Question # 4. Question #62: Must each provider have on staff a Medical Doctor/Physician/ARNP? Will each child be required to receive a physical at the time of the referral? And does this physical need to be performed by a physician made available by the service provider? (ex: a medical doctor that needs to go out to the site of the child and conduct a physical/medical assessment). Answer #62: Attachment I, Staffing Qualifications provides that the Provider must have the capability to provide a physician, ARNP or physician assistant to complete the Medical Evaluation component of the Comprehensive Evaluation, if needed. Providers are encouraged to subcontract for any staffing they do not currently have, and still provide the services as specified. QUESTIONS FROM PSYCHOLOGICAL ASSESSMENT AND TREATMENT SERVICES, LLC - RECEIVED 10/9/14 Question #63: Would the Department please clarify what is meant in the RFI indicating that the psychologist conducts the face to face interview.? Answer #63: Psychological Evaluation Protocol provides that the licensed psychologist will conduct a face-toface interview with the youth. Thus, the licensed psychologist will meet with the youth to observe, question and interact with the youth for the purpose of obtaining information regarding the youth s functioning as noted in Attachment II of the current RFI.
