1 AP-B APPENDIX Superintendent Performance Review Survey Administrative Procedures Revised November 17, 2012 Superintendent Performance Review Survey Board of Directors The following survey is to be completed by Board members of FFCA for the purposes of reviewing the performance of the superintendent and providing useful feedback. Note: Other referent panels will be consulted with respect to the performance of the superintendent as well. Please answer all questions by indicating whether you strongly agree (SA), agree (A), disagree (D) or strongly disagree (SD) with each statement, and provide comments as requested.
2 Leadership The Superintendent provides vision and direction to the school, including encouragement of innovation and continuous improvement. The Superintendent works with others in ways which enable them to take appropriate action. The Superintendent advocates for ongoing professional learning for staff. The Superintendent ensures administrative activities support the implementation of the Annual Education Plan and Charter mandates. SA A D SD Leadership is a process whereby an individual influences others to achieve common goals. Strong and effective educational leadership includes the following: - Projects a positive leadership image - Facilitates FFCA strategy development and evolution - Demonstrates knowledge of roles and responsibilities of position - Maintains a sharp focus on student success - Facilitates development of short and long term educational goals Please provide written comments to the Superintendent by completing the following statements: The Superintendent demonstrates strong and effective leadership in FFCA by:
