3 Disability Advice Service Lambeth is an independent charity which provides a range of services to disabled and older people and carers in Lambeth. It is working on this project in partnership with the Stroke Association s Stroke Care Advisory Service in Lambeth. Stroke Support Network Coordinator 25 hours per week. Salary 19,961 pa (inclusive of Inner London Weighting) for 25 hours This is an exciting opportunity to work on the next stages of our project to empower stroke survivors in Lambeth and Southwark to become more independent and to be included in local networks and community activities. The project is funded until March 2014 by Guy s and St Thomas Charity. This post will be employed by DASL; the Coordinator will also have access to advice from the Stroke Association. Initially working mainly in Lambeth, the Coordinator will recruit, train and support volunteers who will offer stroke survivors access to a range of activities such as travel mentoring, accessing arts and cultural venues, other social activities, befriending and advocacy. build links with a wide range of voluntary organisations and other services to enable stroke survivors and their carers to access social, leisure, educational and employment opportunities in the community promote and advise user led support groups including a peer support network for information sharing and mutual support facilitate and support advisory groups and involvement panels where stroke survivors and carers can play an active part in the planning and development of local services. Two of its major services, Lambeth Resolve and Carers Hub Lambeth, are provided as part of consortia partnerships with other local charities. Its services include: Advice and Advocacy for Disabled and Older People, part of the Lambeth Resolve service led by Age UK Lambeth A specialist advice service for people with mental health problems Enquiry Line and information Services as part of the Carers Hub Lambeth service led by South Thames Crossroads The Direct Payments Support Service for Lambeth More information about DASL s services are at and The Stroke Association The Stroke Association provides a Stroke Care Advisory Service (SCAS) for residents of Lambeth who have been affected by a stroke. The purpose of the service is to enable the stroke survivor and their family to re-engage with their life after stroke. The service provides a link between the hospital and transfer of care into the community, working with intermediate care teams, rehabilitation units, GP practices and nursing teams, social care and community services to ensure that their clients needs are identified and addressed. DASL works closely with the Lambeth Stroke Association. DASL will lead and have overall responsibility for the project and employ the Coordinator. The Stroke Association will collaborate with DASL on the project and will be available to provide advice and support to this post.
5 JOB DESCRIPTION Job Title Hours STROKE SUPPORT NETWORK CO-ORDINATOR 25 hours per week Salary 19,961 pa inc of Inner London Weighting for 25 hours (Spinal Column Point 29) Responsible to DASL Director Responsible for Volunteers including peer supporters. Location Accord Centre, 336 Brixton Road, London SW9 7AA Main Purpose of Post To empower stroke survivors in Lambeth and Southwark to become more independent and to be included in local networks and community activities. Job summary To recruit, train and support volunteers who will offer stroke survivors access to a range of activities such as travel mentoring, accessing arts and cultural venues, other social activities, befriending and advocacy. 6 JOB DESCRIPTION 1. To develop a range of volunteering opportunities for volunteers. 2. To design, develop and support the delivery of activities with stroke survivors and their carers including travel mentoring, accessing arts and cultural venues, other social activities, befriending and advocacy etc. 3. To recruit, train and support volunteers, including stroke survivors as peer supporters, who will provide a range of activities such as such as travel mentoring, befriending and advocacy. 4. To assess clients needs and match these to volunteers skills and experience. 5. To organise/provide accessible training, advice, support and supervision for peer supporters and other volunteers. 6. To develop a Lambeth stroke survivors support group ensuring that members are fully involved in the planning and running of the group and to support the group to become sustainable and independent. 7. To encourage referrals to the group and to support members/users in accessing services and activities. 8. To promote user led support groups including a peer support network for information sharing and mutual support. 9. To ensure stroke survivors and their carers are involved in advisory groups and involvement panels where they are empowered in the planning, consultation and development of local services. 10. To maintain an up to date knowledge of services and resources in Lambeth and Southwark, developing good contacts with key service providers and with other peer support and volunteering projects with which ideas and learning can be shared. 11. To produce accurate monitoring information on the services and activities and evaluate the service from a user led perspective. 12. To attend relevant external meetings, conferences and events including the Lambeth & Southwark Community Stroke Steering Group. 13. To attend relevant team meetings of DASL and the Lambeth Stroke Association, and to participate fully in personal supervision, training and appraisal. 14. To work within DASL s Equal Opportunities, Health & Safety, Vulnerable Adults, Confidentiality and other key policies and procedures. 15. To undertake any other duties as may reasonably be requested by DASL s Director. Occasional evening or weekend work may be required for which Time Off In Lieu will be given in accordance with DASL s current policies.
