The Transition & Work Experience Co-ordinator will be part of a pro-active enthusiastic team helping to promote student achievements and to be responsible for the robust planning and execution of High Needs students smooth transition into and out of Telford College. The post holder will be involved in the writing of appropriate EHCP outcomes depending on students individual needs. They will attend students annual meetings prior to them starting at Telford College through to when they leave and will be important participants in helping students and families plan for the future. The responsibility of arranging high quality work experience, focusing on students individual interests and aspirations will also be part of this role. Responsible to the Leadership team of the provision, the post holder is required to maintain efficient working relationships with all students, staff, multi-professional team, L.A, parents/ carers, work placement staff, governors and all visitors. Main Duties and Responsibilities The successful applicant will be expected to: Actively participate in the interview, assessment and induction process for and with students. Attend and report at the student s annual reviews, including those held at the feeder school. 1
2 Liaise with external agencies to ensure a smooth transition into other provisions or settings at 19. Ensure minutes of reviews and Multi-Disciplinary Team (MDT) meetings are recorded. Ensure all goals, targets and objectives are set up with the students, parents and MDT s. Monitor the progress of students against their goals, targets and objectives and report concerns to senior staff. Act as a point of contact for key people such as parents, social worker, MDT s and internal staff. Work with students and their families and the MDT to plan transition and work experience for students. Develop proactive working systems with the MDT to support work experience activities. Create and maintain an accurate database of work experience contacts. Co-ordinate and prepare individual learners for their work experiences both internal and external. This will include supporting the completion of required paperwork in preparation for work placement visits. Actively find, nurture and maintain links with work experience providers. To develop further opportunities and contacts and distribute this information as effectively as possible for use by learners and staff. Work with MDT s to ensure that the learners person centred plans are used to inform work placements and transition plans and underpin the learners long term goals. Develop business and community contacts and networks and develop a range of placements within the local community in a range of different formats to meet the needs of individual students. Work in partnership with the internal staff. Maintain and update a clear and central monitoring and evaluation system of all student Work Experience and Transition activities. Be responsible for the menu of work experience activities and optimise a range of internal and external placements. Contribute to report processes for learners. Maximise opportunities for work experience placements with external businesses. Ensure all learners taking part in a work activity have been prepared for the activity, learning and recording opportunities explained and de-briefing exercises put in place. Provide assessment records for individual students. Provide feedback and reports as required on student achievement in line with provision policy requirements. Contribute within contractual hours, to meetings, discussions and management systems necessary to co-ordinate the work of the provision as a whole. Undertake professional development. Undertake outreach/inclusion work in other settings as required. Promote good relationships with parents/ carers other professionals and visitors. Carry out any reasonable administrative or supportive role in line with the post. Attend staff meetings and training as required. 2
