2 Please Read Carefully INSTRUCTIONS Your cooperation in completing the Goldman Sachs Employment Application is greatly appreciated. Please refer to the following guidelines that address common questions. GENERAL GUIDELINES Dates: Education/employment should be indicated by month and year. Current employment should be indicated by an end date or Present. Complete Names and Addresses: All schools and employers must be indicated by their full proper name and complete address. Do not use any abbreviations. If street address is not known, at a minimum, city and state location should be indicated. Resumes: You may submit a copy of your resume, but your resume will not be accepted in place of the completed application. EDUCATION If you began a degree program at one school but completed it at another, write Transferred Prior to Program Completion in the box asking if you graduated. The school to which you transferred should then be listed. Where indicated, state the overall, cumulative grade point average (GPA), letter grade or percentage, not your average in your major or last semester. The space provided for your academic standing should not be left empty or estimated. If a school you attended does not calculate an academic average, mark the box next to the statement School does not calculate GPAs. EMPLOYMENT HISTORY Before completing the section on your employment history, ask yourself the following questions so that you respond appropriately. In addition, the questions may prompt you to begin gathering any necessary documentation in preparation for the firm s verification process, to be conducted should you receive and accept an offer of employment. Note that in all cases, the term employer relates to your official employer of record. Are you a prior Goldman Sachs employee? If yes, please make sure to correctly state the time period that you worked at Goldman Sachs. Are you aware that any of your prior employers have gone out of business? If yes, you may be asked to submit copies of your W2 forms or pay stubs from the years in which you began and ended your employment. Rather than as a regular full-time employee, were you working as a consultant or temporary, placed at client sites by an agency? If yes, the placement agency is the official employer of record. Please give the name and address of each agency for which you worked in the space allotted. You may list the client site locations and supervisors in the space marked Duties. Were you working at any of the listed employers as a self-employed consultant? If yes, list the client site as the employer, but under Position, list yourself as a consultant. In addition, in the section marked Manager, please list the names and telephone numbers of three business references who will be able to verify that you provided business services to them during the relevant time period. In connection with this type of employment, you may be asked to submit tax statements (such as a 1099), copies of consulting contracts, and/or beginning and ending date pay stubs. Are any of the employers listed businesses in which you were a partner or had an ownership interest? If yes, please state the complete and exact name of the business on the application. Indicate your status as owner or partner in the section marked Position. In the section marked Manager, list three business references who can verify that you provided business services to them during the relevant time period. At least one reference should be a client serviced during the stated start period, and another should be a client serviced up until the stated end date. In connection with this type of employment, you may be asked to submit documentation of self-incorporation and/or business dissolution, or tax records indicating self-employment from the stated start to the stated end date.
