1 Susquehanna Health Distinguished Servant Leader Award Nomination Form Susquehanna Health values putting patients first, sharing ownership and being servant leaders. Servant leaders call service partners to follow the mission of the organization: to extend Gods healing love by improving the health of those we serve. The Susquehanna Health Distinguished Servant Leader Award is designed to recognize, honor and celebrate those individuals who consistently demonstrate the highest levels of servant leadership. Individuals recognized with this award have fully integrated our mission, vision and values in their daily interactions within the organization and community. Servant leaders at Susquehanna Health are passionate about the work they do and the people they serve. In essence they have joyful spirits. Servant leaders create environments of C.A.R.E. 2 where teamwork and creative thinking flourish in their departments and service lines. Servant leaders are proactive change-agents and problem solvers, teaching and coaching to build teams that are action oriented and accountable. Servant leaders are agents of peace who find ways to reduce the fears and anxieties of patients, families and other service partners. Servant leaders focus energy through discernment, always evaluating and revising plans and approaches as the situation requires. Servant leaders are ambassadors for SH, always promoting the mission of the organization and valuing service partners who are Susquehanna Health. 2 Leadership Duties include: To our patients & customers, leaders must deliver products & services with an extraordinary regard for a positive service experience and safe, reliable and consistent quality resulting in an increased target market share. To our service partners (employees & physicians), leaders must deliver working conditions & opportunities that show up in professional competence & commitment and create conditions for high performance. To the organization, leaders must deliver a set of capabilities that become an enduring culture, not tied to any one person or practice. To investors, leaders must deliver financial performance today & intangible confidence about the future. Eligibility: The award is available to all leadership levels in the organization including: Supervisors, Coordinators, Managers, Directors, Medical Directors and Administrative Directors. Who can nominate a Distinguished Servant Leader: Nominations can be made by any Susquehanna Health Leader including Corporate Staff, Administrative Directors, Medical Directors, Directors, Managers, Coordinators and Supervisors. Criteria: To be eligible for a Distinguished Servant Leader Award the individual must: Be employed in a leadership position with Susquehanna Health for a minimum of two (2) years. Incorporate and demonstrate servant leader traits and behaviors on a consistent basis with all interactions within and outside of the organization. Consistently exceed the standards on the Management Competency Assessment and maintain Thompson Reuters benchmark scores within expected levels. Demonstrates transformational leadership traits by focusing on Accessibility, Reliability, Interoperability and Productivity in the implementation of operating plans for areas of responsibilities. Demonstrate commitment to living the Leadership Pledge Demonstrate C.A.R.E Behaviors treating fellow service partners, patients and visitors with dignity and respect. They are true Ambassadors for Susquehanna Health. Takes personal responsibility for his/her actions - role modeling accountability Be an effective coach, teacher and mentor

3 NOMINATION INSTRUCTIONS 1. Complete the nomination form and submit it to the Leader/Medical Director s immediate supervisor. 2. If approved by the immediate supervisor the nomination must be submitted to the Division Vice President for final approval. 3. If approved by the division Vice President, the nomination is then forwarded to the Human Resources Department for processing I have read the nomination criteria and respectfully nominate for consideration for the Distinguished Leader Award.
