2 What is NJWELL? NJWELL is the new incentive based wellness program offered to actively employed members of the State Health Benefits Program (SHBP) and School Employees Health Benefits Program (SEHBP) NJWELL aims to cultivate healthy lifestyle choices and improve wellbeing among actively employed members of the SHBP and SEHBP. The program is a multi-year approach to raising awareness and improving the health of SHBP and SEHBP members and worksites. Eligible members and their covered spouses or partners who meet the program s Wellness Goals each year receive financial i rewards in the form of a Prepaid Visa gift card. 2

3 Eligible: Who is eligible to participate? Who is ineligible to participate? All active employees enrolled in a SHBP/SEHBP health plan through their employer. Spouses or partners covered under an employee s SHBP/SEHBP health plan. Not Eligible: Employees who have waived SEHBP/SEHBP health coverage. Anyone who is not eligible for SHBP/SEHBP coverage. Anyone enrolled in COBRA or retiree SHBP/SEHBP Coverage. Dependent Children, including those over the age of 18 and Chapter 375 subscribers. 3

4 Background on Wellness Programs Empower individuals to take steps to improve their health. Reduce risks for conditions such as: heart disease, diabetes, stroke, and obesity. Offer employees and their covered spouses or partners financial incentives for taking steps to know their numbers and improve their health. Offer a win-win by giving employees the support and structure they need to meet their health goals and allowing employers to enjoy the savings that result from a healthier workforce. 4

5 Employer Participation = Success When employees take an active role in managing and improving their health, absenteeism is reduced, attendance is improved, and the costs of providing health care are better maintained. Employees are a valuable asset to all employers. Taking all available opportunities to ensure employee health and wellbeing makes sense for everyone, employers and employees alike. By helping employees improve their health and wellbeing, employers also increase job satisfaction and employee retention. 5

6 Program Overview NJWELL Year 1 (2014) Know Your Numbers Biometric Screening 150 points Health Assessment 100 points 250 Points = $100 For both Employee and their covered spouse or partner. 6

7 Do the employee and their spouse or partner both have to participate pate to earn a reward? It is encouraged, but not required for both the employee and their covered spouse or partner to participate in NJWELL. If only one of the member, spouse or partner accrues 250 points, then that person would receive a $100 Prepaid Visa Gift card. If both the member and their covered spouse or partner accrue 250 points, then they would each receive a $100 Prepaid Visa Gift card, totaling $200 in rewards. 7

8 Biometric Screening: Biometric Screenings Measure of blood pressure, BMI (height & weight), glucose (sugar), cholesterol and triglycerides Onsite Schedule biometric screening events at regional meeting locations throughout the State Looking for Opportunities Are you hosting an event in early 2014 where people would be interested in signing up for a biometric screening? Physician Attestation Forms for participants who wish to have their doctor complete the screening, spouses, or those without an on-site option Health Assessment and Screening must be completed by October 31, 2014 in order for eligible participants to qualify for a gift card. 8

9 Health Assessment The on-line Health Assessment Questionnaire allows members to gauge their potential health risks by answering a comprehensive self-assessment. The assessment captures the following information: Current health conditions Vital statistics, including cholesterol and blood pressure measurements Lifestyle factors: alcohol consumption, tobacco use, stress, exercise and diet Life events: family, children, key relationships, job and finances Utilization of health care: doctor visits, prescriptions pt s and hospitalizations at o s NJWELL participants can earn points for completing a Health Assessment on the Horizon or Aetna/ActiveHealth online portals starting January 1, Employees must register for their health insurance carrier s portal in order to complete the Health Assessment. 9

10 NJWELL Year 2 (2015) Action and Engagement The focus of year two is on taking action and engaging employees to participate in healthy activities. Activities Employee / Spouse Incentive 2 Health lthassessment 150 points Biometric Screenings 100 points Age/gender appropriate screenings 25 points each for physical exam, mammogram, Pap test screening, colon cancer screening, prostate screening (if recommended by doctor) Flu Shots 75 points Online Coaching 1 Maximum of points points total for Online Activities 25 points each these 3 items Telephonic Wellness/Disease Management Coaching points points: $100 incentive points: $150 incentive 500 or more points: $200 incentive Per Employee and Per Spouse 1. For individuals identified as eligible. Members cannot get points for both online health coaching and telephonic wellness/disease management coaching. 2. 11 Year 2 Continued In year 2, participants will have additional opportunities to earn points in NJWELL by getting age appropriate, doctor recommended screenings, and by utilizing services available on their health plan s online portal. Points for Online Activities are earned when participants utilize tools on their health plan s online portal including (but not limited to) exercise logs and food journals. The number of points necessary to earn a financial reward increases. The potential value of the financial reward also increases. If both an employee and their covered spouse or partner earn 500 points, they each receive a $200 Visa Gift card, for financial rewards totaling $

12 NJWELL Year 3 (2016) Engagement g and Adherence The focus of year three is on continuous employee engagement and adherence to the program. 13 Year 3 Continued Year 3 again increases the opportunities to earn points by allowing participants to earn points by meeting goals related to their biometric screenings and health assessment questionnaires. Participants i t can earn points by lowering cholesterol, l body mass index, quitting smoking, or by lowering blood pressure or blood sugar counts. Healthy yparticipants p who start out with and maintain cholesterol, body mass index, blood pressure, and blood sugar counts at nationally recognized target levels will also receive points for achieving goals. Important note: Participants who are unable to meet the above goals due to clinical reasons can still earn their financial incentive by accruing points through the other available activities. 13

14 How can you help NJWELL as an employer? Employers serve a critical role in making NJWELL a success. You can help your employees improve their health and wellbeing. Enjoy the benefits of a healthier workforce. Offer your employees a choice to take part in a program that can benefit them for the rest of their lives. 14

15 Designate a Wellness Champion NJWELL is asking each Employer to designate a Wellness Champion as soon as possible. Designation form is available at click NJWELL in the Special Announcements Column. An ambassador to promote NJWELL and encourage healthy behaviors by employees of SHBP/SEHBP employers. Implements and coordinates wellness initiatives for the employer. Works with the Division of Pensions and Benefits designated NJWELL coordinator to implement on site health and wellness initiatives. Supports the goals of the program and the health goals of the Employer. Encouraged to form a Wellness Committee in locations with multiple worksites. 15

16 Wellness Champions Year 1 (2014) Wellness Champions will receive a toolkit from the SHBP/SEHBP that will include electronic and printed materials including promotional flyers and posters. They will need to disseminate and post this information around the worksite. At employers with multiple worksites, it is recommended that the Wellness Champion form a Wellness Committee to assist them in promoting NJWELL. In year 1 (2014) the Wellness ess Champion s focus will be on communicating cat the benefits, e required ed activities, financial incentives and deadlines of NJWELL. They can also get a headstart on starting healthy workplace initiatives to maximize participation in

17 Wellness Champions Year 2 (2015) and Beyond Starting in year 2 (2015) the Wellness Champion can help colleagues obtain points by encouraging participation in healthy workplace initiatives such as walk at lunch or healthy eating programs. Participants in the healthy workplace initiatives can log their participation into their health plan s online portal and receive points towards their financial reward for completing online activities. Wellness Champions will receive frequent suggestions for healthy workplace initiatives from the SHBP/SEHBP, Horizon BCBS, and Aetna. They will also receive periodic trainings in the form of webinars. They are encouraged to provide feedback and communicate successfully implemented initiatives to the NJWELL Coordinator at NJWELL@treas.state.nj.us 17

18 Wellness Champions Healthy Workplace Initiatives Healthy Workplace initiatives offer a sense of camaraderie among coworkers. Employees have a chance to participate in activities together that aren t related to work, through programs including walk at lunch clubs, or healthy eating groups. With improved health and fitness levels, employees experience increased stamina, lower levels of stress, and better levels of self image and self esteem. This leads to improved employee relations and morale, as well as increased productivity. 18

21 Employer Requirement: Taxation of Rewards Beginning in February 2014 and continuing throughout the year, members who meet the eligibility and participation requirements for year 1 of NJWELL will be issued a $100 Prepaid Visa gift card. As an Employer, you should know about the tax requirements that go along with NJWELL Prepaid Visa Gift cards are considered taxable income to the employee and their spouse/partner. Centralized Payroll will handle taxation for State Biweekly locations. State Monthly locations will receive a report listing employees who received rewards via EPIC. State Monthly locations should check with their payroll vendor for information on reporting and withholding taxes on the value of the financial rewards issued to your employees. 21
