1 JOB DESCRIPTION Job Title: Location: Reports To: Team Leader - Domiciliary Care Field based in Waterside, Cityside/Shantallow or Limavady & Strabane areas as required. Office base in Gortfoyle House, Spencer Road, Derry~Londonderry, BT47 6AG. Flexibility for occasional travel and work across all Bryson Care sites in NI. Service Managers - Domiciliary Care Overall purpose of the job To assist and support Service Managers with the effective management and the delivery of the Bryson Care Domiciliary Care service in accordance with agreed aims and objectives, best practice principles and statutory and regulatory requirements. Specific Responsibilities 1. To provide all relevant information to new service users and arrange initial visits to service user homes and completion of Service User Agreements. 2. To contribute to the on-going assessment and review of need of each service user in conjunction with the service user, relevant professional and where appropriate the service user s family / carers and develop a person-centred care / support plan 3. To undertake and complete Quality Annual Monitoring visits with each service user. 4. To ensure the completion of risk assessments for and with service users 5. To check service user satisfaction by regular telephone contact 6. In the absence of the Service Manager: Deputise with the management of the day to day operations of the service to meet the regulatory requirements of the Regulation and Quality Improvement Authority and the Northern Ireland Social Care Council Accept and process referrals based on the eligibility criteria for the service and allocate work to Care Workers Assist with the development and management of rotas and deploy staff efficiently and effectively to ensure that the service user receives the level of care and support appropriate to their individual needs 7. To participate in the out of hours on-call support to Care Workers on a rota basis in exceptional circumstances only. 8. To implement Improvement Plans and to contribute to a culture of continuous improvement in the delivery of the service

2 9. To ensure compliance with all relevant care standards and maintain quality control systems 10. To participate in the training, mentoring and coaching of new care staff 11. To carry out spot checks with staff and report outcomes to service manager 12. To adhere to and implement all Organisational Policies and Procedures to include all HR and Health & Safety Policies 13. To maintain clear, accurate and timely written reports and records 14. To actively participate in supervision and appraisal with Line Manager 15. To undertake and participate positively in mandatory and appropriate training and take ownership for continuous professional development 16. To maintain effective systems relating to the running of the service to include sending s, reports and inputting data to a computerised system and completing computerised reports 17. To work within the Vision, Mission & Values of the Organisation in all aspects of the work and be responsible for the demonstration of these in the course of daily work 18 To undertake any reasonable area of work as required by the Service Manager in line with the job responsibilities

3 Terms and Conditions Length of Contract: Salary: Permanent 8.90 per hour plus mileage Hours: 30 Work Pattern: Probationary Period: Notice: To include days, evenings and weekends. 6 months 4 weeks
