5 May 19, 2015 Page: 5 of 8 GUIDE TO ORIENTATION CHECKLIST The orientation checklist is designed to ensure that an acceptable level of basic safety information is given to the employee upon arrival at a new work assignment prior to beginning work. The supervisor should conduct the orientation by physically showing and demonstrating whenever possible. It is important to ensure the employee understands the information presented and the supervisor should ask the employee questions to ensure comprehension. A short explanation of each item is provided for convenience. Further information is available from the Health and Safety Administrator. Corporate Health and Safety Training List: #A1 Commitment to Health and Safety: Explain the Corporation s commitment to safety and how you are personally committed to the safety of the employee. You may want to include training, awareness days, safety talks and any other practices you do within your department. #A2 & #A3 Safety Policy and IRS: Review the corporate Safety Policy with the employee. Answer any questions they may have. Review their basic responsibilities and rights. The three basic rights of all employees are the right to be informed about the hazards in the workplace and the protective measures to be used, the right to participate in ensuring the safety of the workplace primarily through the JHSC, and the right to refuse work they believe is dangerous or unsafe. Briefly explain the roles and responsibility of management. #A4 & #A5 Health and Safety Manual and OHSA: Where it is located and what Procedures, Policies, Forms and Guidelines are found within the binder. You can refer to the Table of Contents. Provide the location of where they can obtain a copy of the Act and briefly explain what can be found within the regulations. #A6 Ergonomic/MSD Orientation Package: Provide the new employee with the Ergonomic/MSD package and explain the various parts of the Township s program. You may want to do an assessment if applicable.

6 May 19, 2015 Page: 6 of 8 #A7 Accidents/Incident Near Misses Reporting: Explain to the employee that they are required to report any accident or occurrence that has or may have resulted in injury to their supervisor immediately. Show employee accident report form and where they are kept. Explain that if they are off work for more than the day of the accident or seek medical attention they must have the doctor complete the functional abilities form. If they are off work for a period of time they can expect to hear from the Claims Management/Return to Work Representative with an offer of accommodated work. #A8 Fire and Emergency Safety: Indicate to the employee the location of spill kit, eyewash and fire extinguishers. Show the employee the evacuation route and location of the evacuation plan. Show or tell the employee where the rally or meeting location for those personnel employed in their area. Explain what a Person Requiring Assistance means under the Accessibility Act and to advise if they feel they require special assistance. #A9 First Aid: Ensure the employee is aware of the location of the nearest first aid kits and where the names of the designated First Aiders are posted. Review First Aid Requirements Procedure as it relates to the Employee. #A10 Health and Safety Board: Show location of Board and discuss contents. #A11 Review Procedures listed: Briefly review Procedures that are applicable to the employee to ensure they understand and are aware of the protocols and processes that are to be followed within their work area. #A12 Workplace Inspections: Explain workplace inspection program and stress the need to maintain an organized and tidy work area. #A13 Joint Health and Safety Committee (JHSC): Explain to the employee that the JHSC is a committee made up of at least half worker members. Their role is to ensure that the workplace is safe and to work with management to resolve any Health and Safety concerns, complaints, problems, or hazards. Certified members have received special training and are knowledgeable in many areas. Provide the names of the Committee members and what areas they represent.

7 May 19, 2015 Page: 7 of 8 #A14 Review Violence and Harassment Program: Explain to the employee that they have to report incidents of violence and harassment and how the reporting process works. Provide information about violence and what the municipality will do in the event they are involved in a violent situation as per the corporate procedure. Include information about domestic violence. #A15 Employee Rights: Summarize the employee rights based on the information that has already been provided above as it applies to right to know, right to participate and right to refuse. #A16 Health and Safety Quiz New Employees HS-QUIZ-001: Provide new employee(s) with the quiz and a Health and Safety Manual as a reference guide, only if all of the training listed was administered by the supervisor. Mandatory and/or Job Specific Training: #B1 Review of safe operating procedures: The supervisor must indicate if any special qualifications or licences are required to operate equipment and identify to the employee what equipment they are authorized to operate. The supervisor must ensure that the employee knows and is able to apply the safe operating procedures for all the equipment that employee will be required to operate. Review items on the checklist if applicable. (Please note that vehicles are considered equipment) #B2 Site Specific Hazards: Each workplace may have its own particular hazards such as chemicals, specialized machinery or equipment, and special safety measures. Where chemicals are involved the employee must be made aware of emergency procedures, spill procedures, and MSDS. (If a Job Hazard Analysis has been conducted, review with employee) #B3 WHMIS and MSDS: If the employee will be working with chemicals ensure they have a WHMIS course. If they do not have training, make arrangements to have the training. If the employee will have only occasional contact with chemical products or none at all provide a brief overview of the WHMIS system covering labeling and Material Safety Data Sheets (MSDS).

8 May 19, 2015 Page: 8 of 8 Ensure employee understands where to find MSDS and how to locate hazard and protection information. #B5 Working Alone: Discuss protocols when working alone as per the specific procedures for the job task. #B6 Personal Protective Equipment: The supervisor must ensure that the employee is provided with the appropriate PPE. The employee must know how to properly wear and adjust the PPE and how to inspect it to ensure it is still fit for service. #B7 Safety Training: The supervisor must be aware of the safety training already taken by the employee and schedule the employee for any required training such as WHMIS, first aid, fire extinguisher training.
