Department or School Site Healthy Start serves children, youth, their family members, and the community by providing comprehensive school-integrated services and activities to meet the desired results identified in the areas of Academics, Youth Development, Family Support, Basic Needs, and Physical and Mental Heath Care. Project Student and Family Resources Navigators play an essential role in supporting student success by providing referral and case management services within a Healthy Start Program, school-based health clinic,

2 Wellness Center and Wellness Network, or similar student and family support programs. This selection process will be used to fill permanent and/or substitute positions in the Healthy Start Program. Individuals who are selected to fill the substitute positions may become eligible for permanent employment, as these positions become permanent. Vacation: Two weeks of paid vacation to start. Three weeks after five years of year-round paid service. Paid Holidays: 12 days. NOTE: Positions in this job classification are currently offered on a 9-month basis - salary and paid time off for 9 month positions will be adjusted accordingly. Job Duties/Responsibilities The role of the Student and Family Resources Navigator is to foster community based partnerships while performing case management services within a local School-based Health Clinic, Wellness Center, Wellness Network or student support service program; conduct locally driven outreach and enrollment efforts for Medi-Cal and other health insurance programs; and may coordinate the overall support program at some school sites. Typical duties include, but are not limited to: -- Develops, coordinates, and manages the overall case management plan of care for a client with student parent or guardian support. -- Provides outreach and enrollment services for low and no-cost health insurance programs such as Medi-Cal, and Covered California; and provides parents, students, and staff with resource information on and referrals to academic, medical, health education, mental health, and other support services. -- Refers students and/or family members to community services such as a medical, vision, or dental clinic, mental health, or other services designed to meet specific social/emotional, academic or health needs; and arranges for appointments and transportation for a student or student s family members to a service provider. -- Conducts intake and follow-up meetings with identified clients such as foster youth, children from families with low-income, and families whose language may act as a barrier to accessing school and community resources and services. -- Assists or leads the organization and implementation of local Healthy Start Program or other collaborative meetings such as school site

3 Student Success Teams (SST), Student Support and Progress Team (SSPT), Coordination of Services Team (COST), or other program related partner meetings. -- Attends meetings that include school administrators, providers from community-based partner organizations, health care providers, and other District staff to discuss and review the quality of services, trends, projections, and changes within the client populations for a health and student support service programs. -- Builds collaborative partnerships between District programs and social service programs available within the District perimeters. Minimum Requirements EDUCATION: Graduation from a recognized college or university with a degree in human services, social work, child development, community health, health services, or a closely related field. Additional qualifying experience may be substituted on a year-for-year basis for the required education provided that evidence of graduation from high school or equivalent educational proficiency is met. EXPERIENCE: Two years of experience in a social service agency, foundation, or school district working with parents and community agencies providing social services. Experience as a Certified Application Counselor is preferred. SPECIAL: A valid California Driver License. Use of an automobile. Desirable Qualifications The ideal candidate will have relevant experience with health and social services case management within a school-based health and student support service program. In addition, they should demonstrate strength in the following areas: --Knowledge of issues related to families of diverse socio-economic and cultural backgrounds such as foster care youth and English Language Learners --Knowledge of Protection of Personally Identifiable Information (PII), Health Insurance Portability and Accountability Act (HIPPA), and Family Educational Rights and Privacy Act (FERPA) --Familiarity with youth service agencies and services including medical, vision or dental, academic, mental health, and basic needs --Ability to support parent engagement and empowerment in a school community, and facilitate interdisciplinary team support --Ability to exercise tact and judgment in contacts with administrators, co-workers, employees, and others

4 --Ability to pursue relevant facts and data as needed when investigating or researching issues for assigned cases. --Initiative and innovativeness --Strong customer service skills --Effective skills in managing time, priorities, and resources to meet strict deadlines --Ability to present oneself as a positive representative of the District --Exceptional oral and written communication skills --Fluency with a variety of computer software and systems, including Microsoft Office Employment Selection Process The selection process tentatively consists of a pass/fail Alphanumeric Data Entry test, Microsoft Word & Outlook skill check; a pass/fail Self-Scored Questionnaire (Training and Experience Evaluation) that will be completed as part of your application; and an Interview and Technical Project, which will be worth 100% of candidates' overall assessment scores.
