The Institute for Urban Indigenous Health (IUIH) was established to provide a coordinated and integrated approach to the planning, development and delivery of primary health care services to Aboriginal and Torres Strait Islander populations within the South East Qld Region. The IUIH is a lead agency working in partnership with key stakeholders to support the effective implementation of the Council of Australian Governments Closing the Gap initiatives. Join this dynamic organization and drive your career forward Competitive Salary Package arrangements are available This role Reporting to the Youth Program Manager, this position will take a lead in delivering the young women s preventative health education program in a school or community setting, to develop awareness and capacity for participants social, emotional, cultural and wellbeing; and to strengthen, build and provide access to support to identified young women at risk from within the local Aboriginal and Torres Strait Islander community. In doing this, you will: In conjunction with the Manager, develop an annual plan of school based activities in line with the Regional Youth and Young Persons Action Plan Facilitate and deliver the Deadly Choices Healthy Lifestyle Program (including Tobacco Cessation) Assist in the planning, delivery and evaluation of DC Sistas education program modules Distribute health information/program materials, promote clinic services and refer participants for Aboriginal Health Checks Provide culturally appropriate support services to identified participants of the Program and support participation and involvement in activities, projects and initiatives Participate in the continuous quality improvement framework to ensure that program content and information is relevant, up to date, topical and engaging To be successful in this Youth Programs Officer role, you will have: Diploma of Aboriginal/Torres Strait Islander Primary Health, or equivalent qualification, and/or relevant equivalent experience Demonstrated work experience in Education and/or similar Community Preventative Health programs, is desirable Mental Health First Aid Training (or the ability to undertake the training) to support youth within the Aboriginal and Torres Strait Islander community This means you will need: Demonstrated organisation skills, including the ability to coordinate a range or tasks, negotiate timeframes, and handle competing priorities in order to meet tight deadlines. Delegation of Authority As per the Finance & HR Delegations and Authorities accessible on Logic QC - Doc 1448_IUIH Delegations of Authority Date of Approval December 2017 Our Organisation The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the South East Qld Region. IUIH established the Moreton Aboriginal and Torres Strait Islander Community Health Service (MATSICHS) in 2011 as a strategic response to the significant growth and geographic dispersion of the Aboriginal and Torres Strait Islander peoples in the Moreton Bay area. IUIH has developed a number of youth based programs to support young men and young women in building individual capacity and resilience in the areas of culture, leadership, role modelling, healthy relationships, health and fitness. The MomenTIM (Tomorrow s Indigenous Men) Program, funded by the Movember Foundation, is directed at year-old boys/men. Deadly Sisters I a school based program for young women. Role Purpose The Program Officer is responsible for the delivery of school and community based health education, promotion and prevention activities in line with the Regional Youth and Young Persons Action Plan within the framework of the Deadly Choices Program. DC Sistas, Deadly Choices and Smoking Cessation Procedures Manuals. The role will take a lead in delivering the young women s preventative health education program in a school or community setting, to develop awareness and capacity for participants social, emotional, cultural and wellbeing; and to

3 Role Responsibilities strengthen, build and provide access to support to identified young women at risk from within the local Aboriginal and Torres Strait Islander community. School and Community Based Program Facilitation School Based In conjunction with the Manager, develop an annual plan of school based activities in line with the Regional Youth and Young Persons Action Plan Facilitation and delivery of the Deadly Choices Healthy Lifestyle Program (including Tobacco Cessation) o Assist in the planning, delivery and evaluation of the 6-week smoking cessation program; the Good Quick Tukka Program; the Physical Activity Programs/sessions (e.g. Traditional Indigenous Games, Sporting Events/Sessions) and other identified programs from time to time Facilitation and delivery of the Deadly Choices (DC) Sistas o Assist in the planning, delivery and evaluation of DC Sistas education program modules, including: foundation topics (values, identity, culture, mental health awareness, self-care, healthy relationships, positivity & potential and becoming a woman / adult), and; Deadly Choices topics (Leadership, chronic disease, physical activity, nutrition, harmful substances, tobacco and smoking cessation, Medicare access to Primary Health Care). Community Based In conjunction with the Manager, develop an annual plan of community based activities in line with the Regional Youth and Young Persons Action Plan Delivery of after-hours programs and youth-focused events, including such activities as camps and Community Days Support and provide education, instruction and role modelling of key health messages at community events; including o raising awareness around social, emotional and cultural wellbeing o promoting physical activity, healthy eating, tobacco cessation; o leading groups in appropriate physical or cultural activity sessions o promoting CCHS services and refer participants for Aboriginal Health Checks, to other health programs and clinical advice for chronic diseases, as appropriate Work with CCHS s to identify opportunities for further education and promotion, and linkages for clients General for school and community based programs Instruction and role modelling of key health messages within the Deadly Choices Healthy Lifestyle Program Distribute health information/program materials and support participants in making healthy lifestyle choices to develop and enhance their understanding of health and wellbeing and managing the risk factors Promote Clinic services and refer participants for Aboriginal Health Checks Promote services and ensure referral pathways to specialised health and wellbeing programs especially social health, and clinical advice, as appropriate Ensure appropriate records of all program activities are maintained and report on activities and participants, as required Support a range of health education, promotion and prevention activities for young people across the region Identify localised opportunities and develop support for linkages to programs that support in formal and informal education, further training and employment.

4 Provision of Specialised Support to Clients Where appropriate, assess participants and ensure referrals to Social Health Youth Engagement Officers o Identifying and assessing participants needs and interests through surveying/ interviewing/ observing. Provide culturally appropriate support services to identified participants of the Program and support participation and involvement in activities, projects and initiatives Provision of information, advice and advocacy (where needed) to other available services. Identify participants for referral onto further health and wellbeing programs and ensuring referral pathways are relevant and reflective of the needs of clients to appropriate treatment and supports, as required especially working in conjunction with o Youth Mentors to ensure links to the Social Health team; o Family Practitioners and Family Wellbeing Workers to ensure links to the Family Wellbeing Services; o Clinics and the range of available medical/health services o Other identified personnel and programs from time to time As part of the integrated model of care, participate in case conferencing for effective working with clients and their families Operational Processes and Administration Maintain effective planning and reporting documentation Collect data and feedback to assist in the evaluation of Program activities which may include survey distribution and collection. Participate in the continuous quality improvement framework to ensure that program content and information is relevant, up to date, topical and engaging. Provide feedback and report on workforce capacity and risks as required Adhere to policies and procedures in relation to purchases, expenses and expenditures for the program (with consideration of budget). Relationship Management Foster productive working relationships with other IUIH staff in the delivery of seamless support services Actively foster productive working relationships with local networks, government departments, other service providers and suppliers to promote IUIH and remain abreast of emerging issues. Encourage cross-functional collaboration to achieve the best outcome for the organisation. Champion IUIH s Ways Statement both internally and externally to the wider community. Information Management Undertake reporting activities together with other relevant documentation within the scope of your role. Maintain the information flow that supports reliable data and documentation in your area of responsibility. Participate in business planning processes and policy and procedure formulation and improvement within your area of responsibility.

Wear/use and maintain/care for personal protective clothing and/or equipment provided and report any defects. Use lifting equipment and assistive devices as required. Participate in rehabilitation process if injured or ill due to work. Demonstrate leadership in IUIH s Work Health Safety goals and objectives remaining complaint wuith WHS Legislation and IUIH s policies and procedures. Carry out your duties safely in accordance with your ability and competence. Other duties consistent with the position where required and/or requested by your Manager. IUIH can direct you to carry out duties which it considers are within your level of skill, competence and training at any time. Role Requirements Essential A Current Blue Card (working with children and young people) or willingness to apply for a Blue Card, must be attained and declaration made of any future charges or convictions that could potentially affect your role within IUIH. Current C Class Drivers Licence (Qld) Ability to travel across South East Queensland (SEQ) as required. Qualifications and Experience Diploma of Aboriginal/Torres Strait Islander Primary Health, or equivalent qualification, and/or relevant equivalent experience Demonstrated work experience in Education and/or similar Community Preventative Health programs, is desirable Mental Health First Aid Training (or the ability to undertake the training) to support youth within the Aboriginal and Torres Strait Islander community Desirable
