You should also have your Head of School/Unit place their signature in Column (c) of the table, confirming that the Supporting Examples of work / projects described are an accurate reflection of your involvement. Please note that each area submitted for assessment must be confirmed by the Head of School/Unit. Please note that you must only nominate the required number of criteria (including the mandatory criteria) against which you will be assessed. Applications that exceed the required number of criteria will be returned to the candidate for review. APPLICATION CHECKLIST Before submitting your application, please ensure that it includes the following: Fully completed application form (6 copies); Head of Unit signatures in section 6 of application form; Evidence Portfolio (6 copies). Maximum number of pages 20; Referee Reports. You should request your Head of Unit/senior member of staff who is familiar with your work to prepare referee reports on your behalf. Two Referee reports are required for candidates applying for promotion to ALIA. An additional referee report is required for candidates who have transferred from one library post to another in the 12 month period up to Friday, 29 August Completed report(s) should be forwarded directly by the referees to the UCD HR, Promotions Office (HR is scheduled to relocate in August The new address will be confirmed at a later date), no later than 4.00pm Friday 29 August It is the responsibility of the applicant to ensure that the referee report/s have been forwarded to the UCD HR, Promotions office by the closing date. 9.
