2 I. General A. In order to provide a safe workplace for our employees and to comply with OSHA Standards under the General Duty Clause [Sect (5)(a)(1)], your employer is instituting a Workplace Violence Prevention Program (WVPP). Workplace violence has become one of the leading causes of fatal outcomes in the workplaces of the United States. Health care and social service workers are at a significantly higher risk of being injured in the workplace than other types of workers. However, the risks attendant to your work in the offices of UCH - Ophthalmology and the University of Tennessee, Department of Ophthalmology, are substantially lower than those in other health care settings. B. Your employer, UCH - Ophthalmology or University of Tennessee, Department of Ophthalmology, is committed to providing a safe workplace for its employees. To that end the management supports a "clear policy of zero-tolerance of workplace violence, verbal and non-verbal threats, and related actions." C. UCH - Ophthalmology and University of Tennessee, Department of ophthalmology, are instituting a comprehensive WVPP and are committed to educating and training its employees to, in as much as possible, prevent violence in our workplace. D. Employees will have a major role in effectively implementing this program. Prompt reporting by employees of incidents of workplace violence is encouraged, and the employer welcomes suggestions from employees concerning problem areas. II. Written Plan A. This written Workplace Violence Prevention Program (WVPP) is located in the red-orange notebooks in the Photography Room that contain the other OSHA plans. This written plan is available to the employee during regular working hours in the UCH Ophthalmology, Cordova Office. Access is also available at: B. The OSHA Coordinator, William R. Morris, MD, is responsible for the overall implementation of the WVPP. Questions or suggestions regarding the plan should be directed to your individual office OSHA deputy or to the OSHA Coordinator. Kathleen Cruzen has been designated the Deputy OSHA Coordinator for this office. UCH - Ophthalmology, Cordova Office Page 2 of 5

Locks are provided for every office, and adequate lighting of parking lots is present. B. Administrative 1.
