4 This report shows Youth Job Connection Summer (YJCS) case participant activity and expenditures for the fiscal year. The report is grouped by case owner, and displays participant profile information, YJCS activities, expenditures, program dates and outcomes as per service plan. There is also a summary per case owner by status shown at the top of the report. Additionally, there is a breakdown of case activity by status at the bottom of the report, totaling the number of Placements, number of Referrals, Fundamental Job Readiness Skills Hours, Career Development/Career Exploration Hours, Personal Management (Life) Skills Hours, Job Attainment Skills Hours, Employment Related Skills Hours, Job Maintenance and Career Advancement, Total Pre-Employment Service Hours, Paid Pre-Employment Service Hours, Pre-Employment Service Stipend Expenditures, Job Coaching Expenditures, Training Support Expenditures, Training Incentive Committed, Training Incentive Expenditure and Total Expenditures. The report has one line per service plan. Access This report is available to service provider staff report user roles. Any changes made to data after a report run will be reflected in the following week s report. Format, Schedule and Retention This report is in Excel format to accommodate the ability to adjust font size, column widths, and sort and manipulate data and print based on service provider preferences. This report will run weekly, available on Monday mornings, with a total of 264 weeks of reports being retained on the EOIS SP Reporting Website. Data Filtering Participants who have a Youth Job Connection Summer service plan are filtered into the report. Participants are filtered into the report based upon their service plan being in an Open, Approved or Active status, or being Closed within the current or previous fiscal year. Service plans that are closed with a closure reason of Opened in Error are excluded from the report. Release 17.3 Version The Queen s Printer Page 4 of 22

5 Data Sources The information in this report is obtained from several different pages: Participant Information The participant information (Client Name, I Identify As, Age, Person with Disability (PDB), Inuit, First Nations, Métis, Visible Minority (VM), Newcomer (NC), Deaf, Deafblind, Francophone (Fr)) is drawn from the Person page. YJCS Case Activity Information The YJCS case activity information (Case Owner, Case ID, Self Service Initiated (SSI), Referred In, Goal, Number of Placements, Fundamental Job Readiness Skills Hours, Career Development/Career Exploration Hours, Personal Management (Life) Skills Hours, Job Attainment Skills Hours, Employment Related Skills Hours, Job Maintenance and Career Advancement Skills Hours, Total Pre-Employment Service Hours, Paid Pre-Employment Service Hours, Paid Pre-Employment Service Stipend Expenditure, Job Coaching Expenditures, Training Support Expenditures, Training Incentives Committed, Training Incentives Expended, Total Expenditure, Program Start Date, Program End Date, Program Duration Weeks, Closure Reason, Program Status) is drawn from the Service Plan pages (Service Plan, Sub-Goal, and Plan Items and Closure). Referral Information The referral information (Referral, Referred In, Referred Out) is drawn from Service Plan page (Service Plan, Sub-Goal and Plan Items). The Referral column displays as Yes if the participant has an In Progress or Completed sub-goal for either referral to Ministry Delivered Programs (MDP) or Referral to Other Programs and Services. The Referred In column is blank if no response is selected. The Referred Out column displays one of either the referral to a Ministry Delivered Program, Referral to Other Programs and Services type of referral or Referral to Other Community Resources type of referral. 6 Employment Information The employment information (Employment NOC, Employment NAICS, Employment Wage, Period, Hours/Week) is drawn from the most recent placement plan item of the Service Plan, where the plan item is either Placement with Incentive General, Placement with Incentive Job Trials, Placement SP Payroll General, Placement SP Payroll Job Trials. If there are multiple placement plan items within the Service Plan, the report will display the employment information for the placement plan item which has the most recent Actual Start Date and no Actual End Date. Participant Profile Information The participant profile information (School Factors, Essential Skills Reading, Essential Skills Writing, Essential Skills Document Use, Essential Skills Numeracy, Essential Skills Computer Use, Essential Skills Thinking, Essential Skills Oral Communication, Essential Skills Working with Others, Essential Skills Continuous Learning, Labor Force Attachment, History of Poor Work Retention, Language Proficiency English, Language Proficiency French, Language Skills, Source of Income, Validation of OW/ODSP, Family Household Circumstance, Socially Marginalized, Employment Experience, Job Search Skills, Employment Skills, Time out of School, Time out of Work, Education Country) is drawn from the Client Summary page.
