APPALACHIAN POWER COMPANY STAFF S FIRST REQUEST FOR INFORMATION AND PRODUCTION OF DOCUMENTS TO APPALACHIAN POWER COMPANY In accordance with the provisions of Rule 13.6 of the Rules of Practice and Procedure of the Public Service Commission of West Virginia, the Commission s Staff, by counsel, hereby requests of Appalachian Power Company the following information and/or data to be served upon the Staff and upon the Executive Secretary as soon as possible, but no later than noon, Thursday, December 6,2018. In providing the requested information, each response should be on a single sheet of paper which contains the following information: a restatement of the request; the name and title of the person(s) who prepared the response; the date the response was prepared; and the response. Please file any objections to the requested information within fourteen (14) days of receipt, in accordance with Rule 13.6.d. of the Commission s Rules of Practice and Procedure. (W. Va. CSR , et seq, effective April 12,2010.)

3 CASE NO E-C Staff Request Nos. 1.1 through 1.3 The Staff requests that the Company provide additional information and documents related to this Complaint, as follows: Note: If any part of this request for information is unclear please contact the Engineering Division for clarification. All responses shall be submitted on separate sheets of paper (font size 12 or larger) and if requested, electronically submitted in Excel Table format. If any of the requested information cannot be supplied, please provide a justification in your response. Staff requests the following information from the Company regarding this complaint: 1.1) Complainant: Billy Joe Belcher Address: 185 Pinch Ridge Road, Pinch, WV The appropriate one-line circuit diagrams for the distribution circuit serving this location. The diagrams shall provide specific physical locations for each clearing device location listed in the Company s Outage Summary database. The diagram(s) shall be in hard copy (paper) form and contain the following information: a) Substation feeding the distribution circuit, all distribution branches, terminus of all distribution branches, and the location of the above noted address. b) IdentifL all distribution line equipment including, but not limited to, distribution transformers, cutouts, conductors, conductor size, lightning arrestors (indicate device(s) responsible for outage(s)), reclosers, capacitors, voltage regulators, current limiting devices, station power transformers, station breakers, sectionalizing fuses/switches, etc. 1.2) 1.3) A five year outage summary history for this distribution line both in readable hard copy and electronic Excel file formats. The response to this request shall include all information contained in the outage summary database and an explanation of all column headings (including outage cause codes and provide the definitions of each cause code) regarding the significance of the data found in each column. This shall include the column titled Crew Work Required as provided on Attachment 1 of the Company s Answer. A five year history of the data including an indication of Major Events used in the trending of SAIFI, SAID1 and CADI reliability indices, in readable hard copy and Excel formats. Additionally, include graphs reflecting historical trending with major events included and excluded for all three reliability indices. 3

4 1.4) Provide a summary of all major equipment maintenance work performed on this circuit for the past three years according to Company documentation, including the locations and descriptions of all work completed on a year by year basis. State if the equipment maintenance for this distribution circuit occurred due to routine general maintenance, updating obsolete equipment, andor replacement of major equipment failure (as noted on outage reports) for the described time period. 1.5) Provide a list of complaints concerning this distribution line for the past 24 months as required by Commission Electric Rule ) Provide any or all reliability improvement projects for this distribution circuit which have been completed in the past five years or are currently in progress. The details shall include, but are not limited to: 1.7) a) The start / end or proposed end dates. b) The needs assessment for the project. c) The location of the project. d) The equipment added or modified. e) Whether company or contract labor was used. f) Other pertinent work details. The Commission has on file, American Electric Power s System Forestry - Goals, Procedures & Guidelines for Distribution and Transmission Line Clearance Operations manual, Document #: TVMD-0 10, Effective Date: May 1, Verify that this is the most current version of your policy or provide the updated manual. 1.8) Provide a complete accounting of the vegetation management for all work completed on the distribution circuit serving the residence at or near 185 Pinch Ridge Road, Pinch, WV starting January 1, 2014 through current date. This information shall include, but is not limited to all data fields found in the CAMPS database including all financial and contract data. a) When was this circuit evaluated for VMP work? b) What is the total length in miles of this Circuit? c) How many miles of this circuit, if any, have been cleared and approved by a Company forester? When was this work completed? d) Provide pole numbers corresponding to the one-line diagram in DR 1.1 indicating the location where work has been completed. e) When will this entire circuit be completed to comply with the VMP? f) For this Circuit, identifj both the number of miles that have not yet been completed and percentage of the circuit that has not yet been completed. g) Has any payment been made to the subcontractor for work completed on this circuit? h) What is the name of the subcontractor receiving payment for work completed on this circuit? 4

5 1.9) Has this distribution circuit ever been reported as a Worst Performing Circuit in an Annual Reliability Report? If Yes, please indicate the year(s) and the Ranking. 1.10) Please provide a copy of questions and responses to any Data Request submitted to the Company by the Complainant ) Specifically, when will the Company complete the entire distribution circuit related to this complaint? 1.12) What is the total length of the entire distribution circuit related to this complaint and what percentage of this circuit will be cleared by December 3 1,2018? 1.13) Explain in detail the reason(s) this distribution circuit will not be completed at this time, given the resources are in place to do so and that vegetation clearing is apparently needed? 1.14) Where is the distribution circuit in this complaint geographically located to the subject distribution circuits in Case Number E-C? 1-15) Does this distribution circuit originate at the same substation as any of the subject distribution circuits in Case Number E-C? 1.16) Regarding the maintenance that will begin at the substation the fourth quarter of and completed by the end of 20 18; a) Please provide details of the equipment and the work that is planned. b) Please provide a detailed a timeline outlining the work schedule for this work. c) If there are no answers to parts a and b of this question, provide a detailed explanation why. 1.17) Regarding the July 5,20 18 Outage; a) What was the start and end times repair crews were at the scene of the damaged pole July 5, ? b) What was the start and end times repair crews were at the scene of the damaged pole July 6, 20 18? c) What equipment was present both days of the restoration efforts? d) Provide the number of personnel present and their functions both days of the restoration efforts? 5

6 1.18) For each of the four outages referenced by the Complainant and discussed in the Company s Answer; a) How did weather factor into the toppling of the trees which caused damage to the Company facilities for each outage? b) Explain the typical process from notification to complete restoration of an outage. Please include who determines the need for additional personnel and the factors used in making the type, amount and when to call-out additional workers. c) What is the approximate percentage of outages requiring additional personnel such as line workers, flaggers, tree trimmers and additional crews, when weather events likely played a significant part in the outage? 1.19) On any given shift, how many personnel (include their job fimctions) are available for outage restoration? Please include weekend coverage. 1.20) On a daily basis, does the Company employ outage restoration personnel, or are they contracted workers? Please provide details. 6
