2 Dear IMTA Member, Iowa carriers have demonstrated their commitment to professionalism throughout the years and the ongoing campaign towards excellence continues to be a top priority among many carriers of all sizes. The Iowa Motor Truck Association offers many different opportunities for carriers to gain further recognition for their tremendous dedication in the areas of safety and maintenance and I hope that you will consider entering our awards program. This packet contains the forms necessary to enter the different award contests that provide an opportunity for recognition. The Iowa Motor Truck Association offers award and recognition opportunities in several different categories. The annual fleet safety awards, sponsored by Great West Casualty recognizes Iowa fleets for their safety efforts. The Iowa Council of Safety Management will again name a Safety Director of the Year and oversees the Professional Driver Awards program as well. Iowa Motor Truck Association also offers recognition in the area of maintenance. Two maintenance categories are available for recognition. These awards, sponsored by Midwest Wheel Companies, recognize the individual who excels in maintenance operations and awards companies that have shown a commitment to their maintenance activities. And finally, IMTA will be recognizing the men and women who serve in the trenches in the area of dispatching and operations. State and Federal regulatory officials, lawmakers, business leaders and IMTA members will be on hand to show their support of these award winners. It will be a special event that will further illustrate our commitment to excellence in every aspect of the word. We invite you to review the award nominations and to take a moment to get involved in this very important recognition effort. 4 Fleet Safety Award Program Rules Part I - The Contest Program The Iowa Motor Truck Association and Great West Casualty Company invite all IMTA members to participate in this important safety awards contest. Participation in this contest creates competitive safety motivation. DEADLINE: Tuesday, July 3, Part II - Awards Awards in the Iowa Motor Truck Association Safety Contest shall be made on the basis of accident frequency in terms of the number of accidents per 1,000,000 vehicle miles driven in Iowa. A panel of judges appointed by the association selects the winner. 1. Grand Trophy for the one fleet in the contest judged to have the most effective safety activities. Only fleets which are first place are considered for the Grand Trophy. 2. Plaques for each fleet with the lowest accidents frequency rate in its class or division of competition, the committee encourages participation from all companies, large and small. More awards will be issued as participation increases. Separate and/or lower mileage classes will be added as participation warrants. In all cases, decision by the judges shall be final. In the event of a tie, the participation and support of both companies in the activities of the Iowa Motor Truck Association and the Iowa Council of Safety Management will be taken into consideration in determining the winner. Part III - Eligibility and Entry 1. IMTA Membership: a contestant must be a member of the Iowa Motor Truck Association 2. Iowa participation: The IMTA Truck Safety Contest shall be limited to fleets owning or leasing terminals in the state of Iowa and occupying such premises under the fleet title. Occupancy of joint terminal facilities with other motor carriers shall be considered as meeting the requirements of the rule provided the fleet shall have at least one fulltime employee based location. Relay stations, call stations, agency stations or the use of the considered terminals for purposes of this rule. A contestant shall consist of three or more vehicles or combinations. 3. Subsidiary fleet; with two or more truck fleets under common management or control are entered in the truck safety contest, each such fleet operating under its own identity shall comply with section A or part II of these rules. 4. Number of Entries. When a fleet performs two or more distinctly different types of service, the vehicles performing each type of service shall be entered as separate fleets and the mileage and accident experiences of each group shall be reported separately. Part IV - Division of Classes Contestants in the State truck safety Contest shall be grouped for competition between fleets of similar size, type of operation, and exposure of accidents. A. Major contest division: based on type of trucking service performed, shall be established, typical divisions include but are not limited to: tank trucks dry, tank trucks liquid, private carriers, household movers, exempt commodity carriers. IMTA judges have the right to revise the major contest divisions each year. A miscellaneous fleet division may be included for those contestants who cannot be placed in either division. B. Within each major division, as referred above, are subdivisions for local and inter-city operations in accordance with the following definitions: Local Operations The transportation of freight wholly within a municipality or contiguous municipality or wholly within a commercial zone as established by a state or federal regulatory agency, or in compliance with the definition of local operation as otherwise established by a state regulatory agency, shall be entered and reported at local operation, whither or not performed incident, to inter-city transportation of the freight. Inter-city Operation The transportation of freight outside a local area between a local area and points outside that local area; or between distant local area, including the pick-up and delivery of way freight enroute or the performance of local pick-up and delivery by the same driver and the same vehicle performing the inter-city transportation, shall be entered and reported as inter-city operation. For contest purposes, peddle tun and short haul operation, otherwise meeting the definition set forth in this section shall be considered inter-city operations. Mixed Operations A contestant using the same group of drivers and vehicles intermittently in both local and inter-city service, and who cannot or does not, maintain separate mileage and accident reports for each shall enter and report the entire operation in the subdivision which accounts for the greatest share of the annual mileage operated. A. Minimum size of division or class: There shall be at least three truck fleets entered in their respective division or fewer than three entries in their respective division, shall be entered in the miscellaneous fleets division. Part V - Reporting and Statistics A. Basic requirements: the reporting of all mileage driven within the State of Iowa and the reporting of all accidents that occurred in the state of Iowa, shall be the basis for the contest. B. The contest year: The contest year begins April 1 st and ends March 31 st. Final report must be received in the IMTA office by the deadline agreed to by the committee on this basis the contest year will be completed and winners can be announced at the annual Best of the Best Awards Luncheon held in conjunction with the annual state truck driving championships. A. Report requirements: Do not report alleged accidents, accidents involving cargo damage only, or accidents involving company owned or operated passenger cars, service trucks, wreckers, or similar miscellaneous vehicles. The accident rates of contestants shall lie determined on the basis of the number of accidents per 1,000,000 vehicle miles determined on the basis of the number of accidents as derived by the following formula: Frequency rate = Number of accidents x 1,000,000 vehicle miles. The same formula applies to carriers reporting less than 1,000,000 miles. Part VI - Reportability of Accidents - an accident shall be defined as aggregate damage over $1,000 private and public property, unless it involves an injury or a fatality. 1.1 Motor vehicle is any mechanically or electronically powered device, not operated on built, which any person or property may be transported or drawn upon a land highway. The load on a motor vehicle is considered part of the vehicle. 1.2 Mileage is the total number of miles operated by motor vehicles under their own power. Miles operated on public roads or private property are included. Odometer or recorded trip mileages are acceptable. Mileages computed by the fuel consumption, hours of service, average speeds, or any other means are not acceptable. 1.3 Motor vehicle fleet accident is any occurrence involving a fleet motor vehicle which results in death, injury, or property damage, property was damaged and to what extent, where the accident occurred, or who was responsible is not a factor (exemption see part VI) 1.4 Properly parked motor vehicle is on e that is completely stopped and parked where it is legal to park such a vehicle or to stop for the purposes of loading or unloading persons or property. Insofar as possible, parking on private property shall be governed by the same regulations as apply on public streets and highways. A vehicle stopped in traffic in response to a sign, signal, or police signal is not considered parked. When submitting your annual report form we would appreciate a brief history of your company s safety program. In the case of a tie with statistical data, a review of the company s safety program will be the determining factor.

Carriers that have on-site fleet maintenance programs are eligible for this award. Nominations may be made by a fleet maintenance director, manager, or officer of the company.
