4 1. PRESENTATION OF MODEL The Ophthalmologic Chair ERU-5200 is available in just one model, to meet the diverse needs of medical professionals; to suit your requirements. Here are all the necessary information for correct installation and use of the product. 2. OPHTHALMOLOGIC CHAIR The Ophthalmologic Chair consists of the following model: ERU PRODUCT IDENTIFICATION: Technical Name: Ophthalmologic Chair Trade Name: ELIEZER Business Model: ERU-5200 CHAIR 2.2. PRODUCT DESCRIPTION The Ophthalmologic Chair ERU-5200, is a motorized chair, controlled by the operator through the control panel and footswitch. It has an elevation system for the seat, backrest and legrest, driven by electric gear motor, which works oil-free, and extremely quiet. Its operation allows various settings, including sitting, full tilt (lying) and intermediate positions. Its internal structure is made of carbon steel SAE 1020 with surface treatment and epoxy painting; with Polystyrene finishing for easy cleaning. It has padded injected foam, with anti-deforming system; its density is controlled; and its upholstery fabric is known as curvin, coated antimold, with rounded corners. 6 2.8. DIRECTIONS FOR PROPER USE OF THE PRODUCT: OVERVIEW OF THE EQUIPMENT: POSITIONS TO USE THE CHAIR: Normal position: The position set for the entry of patients and start exams. Position the patient sitting. Lying Position: It is the position that tilts with electric drive through gear motor. Position the patient in supine. Opening and closing armrest: It has articulated movements to facilitate patient entry and exit; with movements of the backrest. Chair seat: It has movement up and down controlled by the control panel. Footrest: Has articulated movement. Headrest: Has articulated movement with height adjustment; and foam padding with injected anti-deforming COMMANDS FOR CONTROLLING MOVEMENT OF THE CHAIR: The commands to control Ophthalmic Chair ERU-5200 is triggered by the operator via the control panel. It has electric controls for adjustable seat height, backrest chest and leggings and simultaneously back to the zero position, in which the chair returns to its normal position (sitting) to facilitate the exit of the patient. The chair control operations are performed just by a simple touch command buttons. 6

7 WARNING! The control panel and footswitch has no protection against liquids, therefore cannot be considered to be waterproof. Layout of the controls located on the Panel. PANEL COMMAND DESCRIPTION Power ON/OFF Turning on/off the chair. Up / Down Elevation/Drop of the seat and the whole set. Incline / Recline Descent of the backrest and lift the legrest, simultaneously. Zero Position Automated return to initial position, called zero: sitting position. WARNING! It is strictly unadvisable to use the equipment by persons not trained, and we are not responsible for misuse of the same. WARNING! Read the following instructions before beginning installation of equipment for the first time. 3. INSTALLATION OF EQUIPMENT - Before starting the installation of the equipment, make sure there is no bad component inside the package. If this happens, immediately contact the company to resolve the problem. - Install the equipment in the appropriate location with sufficient space so that their movements can be made freely and allowing access to all parts. 7

8 - Make sure the electrical supply network has the local tensions of 127 or 220 and that the same support at least 200 Watts of power. - The equipment should be plugged into a specially installed for this purpose and grounding as ABNT force in place to ensure total safety and proper operation of equipment. - Do not connect receptacles or other devices in the same outlet where the equipment is installed. - When connecting the power cord into the outlet, make sure it is not pierced in places where prolonged movement of people to prevent accidental disconnection and possible damage to the cable or outlet. - Level the machine using it for leveling feet that are at the base of the equipment and in contact with the floor. - Attach the headrest rod into the hole on top of the back of the chest. - If necessary, the manufacturer has technical staff to perform the installation of the equipment. WARNING! The equipment has a manual selector voltage, so before you connect you must adjust it to your outlet voltage. BEFORE INSTALLATION: 4. SPECIAL STORAGE, CONSERVATION AND/OR HANDLING PACKING: All information on precautions to be taken during transport and storage of the product are listed in their own packaging through standard symbols as described below: SYMBOL DESCRIPTION Fragile. TRANSPORTATION: - During transportation, avoid vibration, falls and impacts directly on the packaging. - When handling the equipment out of its original packaging, proceed from the base and never by the arms or other piece of equipment. - During transport and storage, ensure that environmental conditions are within the specified ranges, as described below: Ambient temperature for storage and transportation... 0ºC to +55ºC Relative humidity for storage and transportation... 0% to 90% Pressure to 795 mmhg AFTER INSTALLATION: 4.4. PACKAGING: - Keep product in a place protected from moisture and excessive heat, including incision of sunlight on it. - During the accommodation and operation of the product, verify that the environmental conditions are within the specified ranges, as described below: Ambient temperature for storage and transportation ºC to +35ºC Relative humidity for storage and transportation... 30% to 75% Pressure to 795 mmhg 4.5. CONSERVATION: - During use, clean equipment with a damp cloth and mild soap on the upholstery and alcohol in the other parts. - After each use, keep the equipment clean and disinfected. - Do not enter or spill liquid inside the machine. - Do not use organic solvents (thinners) or products that release chlorine to clean the equipment. - Keep the equipment in a clean and away from dust. 5. WARNINGS AND / OR PRECAUTIONS TO BE TAKEN SPECIAL WARNINGS AND / OR PRECAUTIONS TO USERS - This equipment should be used only by trained personnel and under the direction of qualified medical staff familiar with the known risks and benefits on the use of ophthalmic chair / Otorrinológica SPECIAL WARNINGS AND / OR PRECAUTIONS WITH WEIGHT LIMITS - Never exceed the weight limits specified by the manufacturer. - Do not sit in the back leggings or chest of the equipment. PRODUCT PERFORMANCE 6.1. INDICATION AND INTENDED USE OF THE PRODUCT Note: Designed for use in medical clinics, and hospitals to assist health professionals to position the patient in special positions for diagnostics, providing comfort to the patient and professional. If used according to instructions described in this manual, the equipment will not cause any problems and will not lose or change their physical characteristics and dimensions. 10

11 7. INSTALLATION OR CONNECTION WITH OTHER PRODUCTS The Ophthalmologic Chair was not intended use as a requirement for direct connection to other equipment or products. WARNING! Use in conjunction with this equipment in any part, accessory or equipment other than those specified by the manufacturer is the sole responsibility of the user. 8. PROCEDURES FOR RE-USE PRODUCT 9.1. CLEANING - The cleaning of the equipment should be performed before and after receiving each patient. - Clean the upholstery, metal parts with a damp cloth and mild soap. - Weakly alkaline cleaning products can be used to clean the padding. NOTE: Do not use any cleaning product chlorine or acid! 9.2. DISINFECTION - Held the usual plus a cleaning product or disinfectant germicidal broad spectrum, taking special care not to waste that may cause any toxic or allergic reaction on contact with the patient's body. - Alcohol-based disinfectant can be purchased in the market with the most diverse concentrations. SENSITIVITY TO ENVIRONMENTAL CONDITIONS IN NORMAL USE SITUATIONS The Ophthalmologic Cahir ERU-5200 has been specially designed and developed to avoid the sensitivity to electromagnetic interference, external electrical influences, electrostatic discharge and variations in pressure and temperature.
