1 1.0 QUALIFICATION BY ANALYSIS This EQPR is intended to be used when buying Keystone brand butterfly valves and spares that require thirdparty dedication by Pentair/Anderson Greenwood Crosby because the Original Equipment Manufacturer (OEM) is no longer in a position to supply the items to Ontario Power Generation (hereinafter OPG) for use throughout its nuclear sites in safety-related applications which require being environmentally qualified (EQ). Note: Keystone brand is currently part of Valves & Controls business unit of Pentair. The Manufacturer of Record is Anderson Greenwood Crosby (a division of Pentair). OPG Engineer (see the Contract Standard for definition of Engineer ) generally assesses and qualifies electrical equipment for use in EQ applications by conducting qualification tests (or referring to qualification tests performed by others) whereas mechanical items are generally qualified by analysis. Once it has been established that an item is environmental qualified, it is necessary to ensure that successive purchases of the item shall be equivalent to the qualified item. In order to do so, it is necessary to clearly establish the critical characteristics of the qualified item so that these can be replicated and verified. These critical characteristics may be shown on one or more of a number of different documents including the manufacturer s catalogue, manufacturer s drawing(s), test reports, bill(s) of material, specification(s) etc. So as not to limit the generality of the foregoing list, the document that describes the critical characteristics that need to be replicated shall be referred to herein as the primary controlled document. For EQ materials it is essential that the primary controlled document clearly identifies the materials of construction as well as other critical characteristics of the item. Refer to EPRI NP-5652 and TR for a more detailed explanation of critical characteristics. The primary controlled document will be selected by OPG Engineer and stipulated in the OPG purchase order (P.O) item description. In every instance, the content of the primary controlled document shall be under rigorous revision control. In instances when the primary controlled document defines most but not all critical characteristics, and in instances when the primary controlled document provides configuration or material selection options, the required characteristic or material option(s) shall be stipulated in the OPG P.O. item description. Pentair/Anderson Greenwood Crosby is advised that the item shown in the OPG P.O. has been assessed and qualified by the OPG Engineer for use in nuclear safety-related applications based on the primary controlled documents (drawings and associated bills of materials) referenced in the OPG P.O. item description. All the items and spare parts are to be supplied in accordance with the requirements on the approved drawing(s) and OPG Design/technical specifications referenced in the OPG P.O. item description. Note: If there is no drawing referenced in the item description Pentair/Anderson Greenwood Crosby is to contact the OPG Buyer for further direction. If there is a discrepancy between the approved drawing, specification data sheet and the required item description, Pentair/Anderson Greenwood Crosby shall contact the buyer for resolution, using the forms referenced in section CHANGES IN DESIGN AND SUBSTITUTION OF QUALIFIED MATERIALS Changes made to the Pentair/Anderson Greenwood Crosby items that were originally assessed and environmentally qualified by OPG are to be tracked. Pentair/Anderson Greenwood Crosby shall advise OPG in the event that changes take place in the following areas: design of the product, material/compounds used, manufacturing methods, non-destructive testing procedure, and test reports. Some changes, which may appear insignificant for the commercial applications and are not deemed to be reported to the client (since fit, form, and function are met), can invalidate the equipment qualification. Changes to non-metallic components are of particular interest to OPG. Pentair/Anderson Greenwood Crosby shall notify the OPG Buyer using the forms identified below, if any changes have or are to take place to the item shown in the P.O. item description which is assessed and qualified by OPG. This clause applies to purchase orders placed on or after March 20, 2018 Page 1 of 8

2 Pentair/Anderson Greenwood Crosby shall ensure contractual provisions and QA program requirements imposed on suppliers and sub-suppliers are such that Pentair/Anderson Greenwood Crosby shall be notified of any such changes that may occur within the supply chain. If any of these changes take place then Pentair/Anderson Greenwood Crosby shall notify the OPG Buyer using the Vendor Parts/ Material Substitution Control form N-Form during bidding or the Concession Application form N- Form subsequent to contract award. As an alternative to the preceding paragraph, Pentair/Anderson Greenwood Crosby shall notify the OPG Buyer, using the same forms, if for any reason it is unable to provide assurance that there have been no such changes since the item was originally evaluated/ tested for use in a nuclear safety application. The OPG Buyer should forward all such notifications to the Design Authority in OPG. The responsible Design Authority shall make all decisions relating to the need to re-evaluate and/or re-test the item. Although the Design Authority may solicit Pentair/Anderson Greenwood Crosby s input to the decision, the decision of the Design Authority shall be final. Pentair/Anderson Greenwood Crosby is advised that: The items will be installed in a nuclear safety system. That system shall be required to operate following a very unlikely but theoretically possible design basis accident. If such an accident occurred the items could be subjected to a harsh environment consisting of chemical and pressure transients, radiation, steam, and elevated temperature and pressure or submergence. It is possible that the design basis accident could occur at a time when the items have already been subjected to many years of normal operation and are nearing the end of their useful operating life. These items have been rigorously evaluated and/or tested, and the OPG Engineer expects that they shall perform their intended safety function following exposure to this harsh environment. OPG s Environmental Qualification Assessment (EQA) has established that the items may not perform their intended safety function if there is a substitution of materials, changes in design or changes in manufacturing methods. This is especially true for changes or substitutions of non-metallic components used in the construction of these items. Some non-metallic components are known to be subject to degradation in a harsh environment and can deteriorate quickly when exposed to steam, heat, or especially when exposed to high levels of radiation. The OPG Engineer accepts that changes in design, sub-suppliers, materials, and manufacturing methods may be necessary as technology and manufacturing processes change with time. The OPG Engineer also accepts that companies may not be able to track all changes that occur over a long period of time. It is the OPG Engineer s objective to minimize unnecessary changes that would necessitate a reevaluation of the items ability to perform their intended safety function following a Design Basis Accident as even minor changes may dictate the need to retest or re-qualify the items. In any event, it shall certainly dictate the need to evaluate the effect of the change to ensure that the new item shall perform the same intended safety function as the items that were previously qualified. The "changes" referred to in this section are interpreted to mean any changes in materials of construction, changes in manufacturing processes that affect interchangeability, changes in form, fit and function, or any changes from the original design or performance requirements. Insignificant changes that do not affect any of these criteria (i.e. that do not result in a change in the item part number) are considered to be outside the scope of this requirement. 3.0 QUALITY ASSURANCE PROGRAM FOR ENVIRONMENTALLY QUALIFIED ITEMS For the items that Pentair/Anderson Greenwood Crosby manufactures/dedicates to OPG, Pentair/Anderson Greenwood Crosby shall implement and maintain a quality assurance program that provides assurance that the design, materials used in construction, methods used in fabrication/construction, non-destructive testing, are such that the Pentair/Anderson Greenwood Crosby item both meets the design requirements and is equivalent to the Pentair/Anderson Greenwood Crosby item evaluated and qualified by OPG Engineer. In order to provide this assurance Pentair/Anderson Greenwood Crosby quality assurance program shall meet the requirements of CSA CAN3-Z or CSA N QA Standards, including the requirement for product traceability. This clause applies to purchase orders placed on or after March 20, 2018 Page 2 of 8

3 Pentair/Anderson Greenwood Crosby s QA Manual as audited by NUPIC to the requirements of 10CFR50 App B quality program, and reviewed/accepted by OPG to the requirements of CSA CAN3-Z , or CSA N QA Standards, along with the requirements specified in the P.O., is acceptable to OPG to meet this requirement. Traceability is required by this contract, and therefore in accordance with CSA CAN3-Z or CSA N QA Standards, Pentair/Anderson Greenwood Crosby shall assign to each product or service or batch a unique identification/unique identifier such as serial number, date code, batch code to distinguish apparently identical products. Note: The requirements of 10CFR Part 21 (Reporting of Defects and Noncompliance) shall survive the completion of the contract as if the item was supplied to a USA based nuclear utility. Nonconformance with a disposition to Repair or Use-as-is require the concurrence of all responsible parties, and submission to OPG for acceptance. In addition: For ASME, Section III materials, the quality program shall meet the requirements of ASME, Section III, subsection NCA 3800 or NCA 4000 (where the scope of the quality program includes acting as a material organization), to the code edition and addenda specified in the purchase order. For ASME, Section III components, the quality program shall meet the requirements of ASME, Section III, subsection NCA 4000, to the code edition and addenda specified in the purchase order. A valid certificate of authorization issued by an authorized registrar (i.e. ASME, TSSA, other Canadian provincial jurisdiction etc) shall be required, in accordance with the CSA N285.0 standard. Any item sourced from any other facility shall be dedicated by Pentair/Anderson Greenwood Crosby using suitable EPRI methodology, under the Pentair/Anderson Greenwood Crosby s 10CFR50 App. B QA program. All items shall be traceable to Pentair/Anderson Greenwood Crosby plant located in Mansfield, Massachusetts. Pentair/Anderson Greenwood Crosby shall ensure that activities undertaken by its authorized distributor do not jeopardize traceability. 4.0 DEDICATION PLAN AND CRITICAL PROCESS PROCEDURE ACCEPTANCE For new EQ items not previously dedicated, Pentair/Anderson Greenwood Crosby shall submit a Dedication Plan (CGD Plan) using suitable EPRI methodology to the OPG for review and acceptance following the award of the contract and prior to initiating the work described on the plan, unless directed otherwise in the OPG P.O. An I&TP submission to OPG for acceptance is not needed for dedication activities. Pentair/Anderson Greenwood Crosby shall perform dedication activities as required, to ensure that the item listed in the OPG P.O. item description (environmentally qualified for its intended end use by OPG Engineer) is what is supplied to OPG. Acceptance of the specified item(s) shall be in accordance with Pentair/Anderson Greenwood Crosby CGD Plan reviewed and accepted by OPG, and any other additional requirements specified in the OPG P.O. The CGD plan need not be resubmitted for acceptance if it has been previously accepted by OPG. Changes to this dedication plan must to be submitted to OPG for acceptance. For repeat orders of a given item, Pentair/Anderson Greenwood Crosby may utilize a previously OPG accepted CGD Plan. In this situation Pentair/Anderson Greenwood Crosby shall provide reference to the previously OPG accepted CGD Plan. This reference shall be included in the Certification to Accompany Shipment. A copy of the OPG accepted, completed CGD plan shall be submitted with the shipment. Pentair/Anderson Greenwood Crosby CGD Plan may be in any format to meet the company s needs but shall, as a minimum, indicate: The methods by which Pentair/Anderson Greenwood Crosby shall verify that the manufacturer of the item has not introduced additional age sensitive (non-metallic) components into the item. Where Pentair/Anderson Greenwood Crosby identifies additional non-metallic components, non-metallic components that are not specified in the primary controlled documents, Pentair/Anderson Greenwood Crosby shall obtain further direction This clause applies to purchase orders placed on or after March 20, 2018 Page 3 of 8

4 from the OPG Engineer to verify additional items do not affect the Engineer s assessment that the item is qualified for installation in an EQ application, The methods by which Pentair/Anderson Greenwood Crosby shall verify the materials/compounds used for the non-metallic components and their equivalency to those used in the assessed (based on the tests or analysis) items, as specified in item description and/or primary controlled documents, Similarly, Pentair/Anderson Greenwood Crosby shall identify all the other materials used in the construction of the item (metallic, glass, grease, etc) which can be critical to the function of the item, The products or services to be subcontracted and the quality assurance program requirements that shall be imposed on the subcontractors, The way Pentair/Anderson Greenwood Crosby seismically qualify the items (as applicable), The method by which Pentair/Anderson Greenwood Crosby shall verify subcontractors performance (incoming inspection, verification and test, source surveillance, etc), The identification of each inspection and test point located relative to the production cycle, The characteristics to be inspected and tested together with the sampling plan and acceptance criteria to be used (including those used for final inspection), Justification for using a sampling plan (i.e. How does Pentair/Anderson Greenwood Crosby show that the lot is homogeneous), Customer witness and or hold points, If lots or batches shall be used, Records to be maintained during and after inspections, Notes: Personnel performing inspection, witnessing or monitoring of item critical characteristics for their acceptance must not be those who have performed the work being inspected or report directly to immediate supervisors responsible for producing the work unless specifically permitted otherwise in the I&T Plan and agreed to in writing by the customer before the contract is let. Activities related to the item registration with the Pressure Vessel Division of the Technical Standards and Safety Authority (TSSA )are separate from the dedication activities described above, and are not required to be repeated during dedication once the item is registered. After completing the dedication activities, the dedicator shall submit the OPG accepted, completed CGD plan, including all test results (destructive and non-destructive) and final acceptance results to OPG with shipment of the item. Test results shall include the serial number(s) of the measuring and test equipment used and shall be approved by an authorized QA Representative (as a minimum) of the dedicator s organization. OPG reserves the right to specify special process procedures and non-destructive testing procedures (if any) that are to be submitted for acceptance upon receipt of the CGD Plan. It is agreed that OPG will not retain copies of these critical process procedures. OPG accepted copies of these procedures are to be retained by Pentair/Anderson Greenwood Crosby and Pentair/Anderson Greenwood Crosby shall provide evidence that the work has been done in accordance with the approved procedures when called upon to do so by OPG s source inspector. If during the course of the dedication process, there are any changes or deviations from the accepted CGD plan that affect the Form, Fit, Function, design basis or any other characteristic of the item, including test failure, Pentair/ Anderson Greenwood Crosby will notify OPG of the non-conformance. Pentair/Anderson Greenwood Crosby will document the non-conformance by submitting N-FORM-10393; Concession Application by Supplier for OPG acceptance and, after acceptance by OPG, the item may be shipped with all the required documentation. The plan and associated procedures as well as any change/upgrade of the subject plan and associated procedures are to be submitted for acceptance 4 weeks prior to commencement of the work outlined on the plan or procedure. Pentair/Anderson Greenwood Crosby should allow 3 weeks for review and acceptance. This clause applies to purchase orders placed on or after March 20, 2018 Page 4 of 8

5 In the event that delay in receiving the OPG Engineer s acceptance impacts completion of the work, Pentair/Anderson Greenwood Crosby is to notify the OPG Buyer at least a week in advance of the delay occurring, in order that OPG Buyer can expedite OPG Engineer s acceptance and thus avoid the delay. If an expedited acceptance is required to meet the scheduled date of delivery, Pentair/Anderson Greenwood Crosby should contact OPG Engineer to indicate the urgency and expedite the approval process. OPG Buyer should assist the Pentair/Anderson Greenwood Crosby in this process. 5.0 PRESSURE RETAINING ITEMS REGISTRATION WITH AUTHORIZED INSPECTION AGENCY Pressure retaining items are to be registered with an Authorized Inspection Agency (AIA). Piping systems shall be registered by technical standards & safety authority (TSSA), located in Ontario. Pressure vessels, storage vessels, pumps, valves, fittings or supports shall be registered as follows: By TSSA located in Ontario, or By Association of Chief Inspectors (ACI) central located in Prince Edward Island. ACI central provides registration for several Canadian provinces, including New Brunswick. As New Brunswick is the only one of these provinces that has nuclear class installations, registration shall be requested for the province of New Brunswick. Fittings are as defined in CSA N and update 2: clause and table 1. This registration shall be in accordance with CAN/CSA-N and Update No. 2 and the ASME code and class specified on the OPG Design/technical specification referenced in the OPG purchase order (P.O.) item description Pentair/Anderson Greenwood Crosby shall provide evidence of that registration by way of a Canadian Registration Number (CRN) along with the shipment. For nuclear class 1, 2 or 3 components and materials, Pentair/Anderson Greenwood Crosby shall supply items that meet the requirements of NCA 4000 (for components) or NCA 3800 (for materials). Additionally, in compliance with CAN/CSA-N & Update No. 2, Pentair/Anderson Greenwood Crosby is to provide a History Docket which is to include at minimum: Statutory Declaration, Certified Material Test Report (CMTR) for SST housing material, Certificate of Compliance referencing serial numbers and applicable test data. 6.0 SEISMIC QUALIFICATION REQUIREMENTS Pentair/Anderson Greenwood Crosby shall demonstrate the seismic qualification for the item(s) is in accordance with the OPG technical specification referenced in the OPG Purchase Order (P.O.) item description. Demonstration of the seismic qualification is permitted by physical testing and/or analysis. For repeated orders Pentair/Anderson Greenwood Crosby certification statement that the item supplied is equivalent to that originally qualified and supplied by Pentair/Anderson Greenwood Crosby (see Section 10.0, Certification to Accompany Shipment ) is deemed to satisfy the Engineer s requirement for seismic qualification of the item. 7.0 MARKING AND TAGGING OF THE ITEM Whenever possible, a permanent, corrosion resistant metal tag shall be affixed to each item. Tags are to be permanently attached when appropriate with mechanical fasteners (e.g., screws, rivets, weld or stainless steel wire). The tag shall be applied in such a manner so as not to affect the function of the item. If it is not possible to attach a tag, Pentair/Anderson Greenwood Crosby may permanently mark, etch or scribe data on the item, whichever is applicable and appropriate usually based on equipment size. This tag and/or appropriate marking on the item shall contain the following information: Pentair/Anderson Greenwood Crosby / Manufacturer s name and model/ part number, Pentair/Anderson Greenwood Crosby / Manufacturer s unique identifier such as serial number, batch/lot control number or cure date/ manufacturer date (to ensure traceability) Pressure Rating (if applicable), Date of manufacture This clause applies to purchase orders placed on or after March 20, 2018 Page 5 of 8

6 Pentair/Anderson Greenwood Crosby shall provide the following details either on the metal tag described above or on an additional tag attached to the item by a stainless steel wire in such a manner so as not to affect the function of the item: Canadian Registration Number (CRN) (for pressure retaining items), OPG Catalog ID number Note: Where the item has been supplied with an original manufacturer s (OEM) tag, it shall be secured to the equipment in the same way as required for the Pentair/Anderson Greenwood Crosby s tag. As an alternative to attaching a tag, the manufacturer may permanently mark, etch or scribe the above data on the items. All marking and tagging shall be clear, unambiguous and indelible. Spare parts do not need to be tagged but shall be shipped in suitable packaging and labeled as outlined below. 8.0 PROTECTION FROM DEGRADATION DURING SHIPMENT OR STORAGE The items shall be individually packaged and protected from degradation during shipment and suitably packaged for long term storage complete with desiccant where appropriate in accordance with ASME NQA-1a-2009, Part II, Subpart 2.2 Level B or the predecessor industry standard ANSI-N Level B. Individual packaging shall provide protection from ultraviolet (UV) radiation, where applicable. Where sealed packaging is used, the seal is to be such that the items can be removed for inspection and resealed in the same package for long term storage in level B. It is OPG s intention to store EQ materials in level B storage. Packaging requirements for level B items are specified in NQA-1a-2009, part II, Subpart 2.2, section 302.2, which refers to Level A packaging specification defined in section with the exclusion of requirement 302.1(a) (b) Items shall have been inspected for cleanness immediately before packaging. Dirt, oil residue, metal chips, or other forms of contamination shall have been removed by approved cleaning methods. Any entrapped water shall have been removed (c) Items not immediately packed shall be protected from contamination (d) Items requiring protection from water vapor, salt air, dust, dirt or other forms of contamination penetrating the package, shall be packaged with a barrier (e) Items that require protection from damage during shipping and handling shall be packaged in containers or crates (see para 307 of NQA-1a-2009, Part II, Subpart 2.2) (f) Items that can be damaged by condensation trapped within the package shall be packaged with approved desiccant inside the sealed waterproof and vapor-proof barrier or equivalent method (g) All openings into items shall be capped, plugged or sealed. Weld end preparations shall be protected against corrosion and physical damage (h) Items packed in containers shall be blocked, anchored, braced or cushioned to prevent physical damage to the item or barrier (i) Items and their container shall be identified by marking. Pentair/Anderson Greenwood Crosby is to include a shelf life statement, when applicable, with every purchase order indicating the shelf life date of each item (Catalog ID Number) on the purchase order. Shelf life for every item should be assigned by Pentair/Anderson Greenwood Crosby under their quality assurance program and in accordance with industry (e.g., EPRI, Mil Spec, etc.) recommended shelf life values for similar components. Shelf life can be determined by using EPRI Report: NP6408, Guidelines for Establishing, Maintaining and Extending the Shelf Life Capability of Limited Life Items (NCIG-13): Typical Data from EPRI Report: NP6408, App. B, List of Generic Shelf Lives (Partial List) Material Shelf life in years (EPRI NP6408 May 1992) EPDM/EPR 60 This clause applies to purchase orders placed on or after March 20, 2018 Page 6 of 8

7 Typical Data from EPRI Report: NP6408, App. B, List of Generic Shelf Lives (Partial List) Material Shelf life in years (EPRI NP6408 May 1992) Neoprene 36 Nitrile 14 Nomex 60 Teflon 60 UHMWPE 14 Viton 49 Tefzel 60 OPG Receiving Inspector should not accept the item for delivery if the remaining shelf life is less than 70% of the recommended shelf life at the time of delivery without prior written consent. 8.1 Butterfly Valves (Assemblies) Where packaging is intended to protect the items and spare parts from degradation during storage, the packaging individual cartons or the packaging shall be clearly marked with the following: Pentair/Anderson Greenwood Crosby / Manufacturer s name and model/ part number, Pentair/Anderson Greenwood Crosby / Manufacturer s unique identifier such as serial number, batch/lot control number or cure date/ manufacturer date (to ensure traceability) OPG Purchase Order number and Line Item number, OPG Cat Id number, CR Number (as applicable) Date of manufacture, Item shelf life (if applicable) The weight of each butterfly valve and suitable cautions relating to the eccentric centre of gravity shall be clearly marked on shipping tags attached to each butterfly valve. Where appropriate, suitable lifting lugs shall be installed in locations, that will not interfere with, the operation of the Keystone butterfly valve or the seismic, and EQ test certification(s). Sling points and/or lifting points shall be marked, which take into account good lifting practices and the eccentric centre of gravity on all Keystone butterfly valves, regardless of whether they have been supplied with lifting lugs. Only valves of the same model number may be banded to the same skid. In this event, each Keystone butterfly valve shall be banded separately so that any one Keystone butterfly valve can be removed from the skid without disturbing the butterfly valves that remain on the skid. The entire skid is to be draped with plastic and the plastic stapled to the skid in such a fashion as to permit free air movement from underneath the skid but deflect dust and debris that may settle from above. The gross maximum weight of the skid is not to exceed 1000 lbs. and the actual weight of the skid is to be marked on a tag, which can be read from a distance of 20 feet. If possible, the packaging should be clearly marked with the following notations: Packaging materials are essential for maintaining the EQ qualification of this item. This packaging contains Shelf Life Information. Do not discard the packaging until the installation in the intended application is complete. If the item is issued for use and not installed, the item shall be returned to station inventory in its original packaging. Any special storage or handling instructions. All markings and tagging shall be clear, unambiguous, and indelible. This clause applies to purchase orders placed on or after March 20, 2018 Page 7 of 8

8 8.2 Packaging Instructions for O-Rings O-Rings shall be individually packaged in a fashion to resist ultraviolet radiation (UV) and must not be re-sealable, so that the integrity of the package can be used to provide assurance that the part contained inside is exactly as labeled. Since packaging is intended to protect the part from degradation during storage, the packaging individual cartons or the packaging shall be clearly marked with the following: Pentair/Anderson Greenwood Crosby part number Unique identifier such as serial number, batch/lot control number or cure date/ manufacturer date OPG Purchase Order number OPG Catalog ID number The date on which the shelf life will expire The packaging shall be clearly marked with the following notation: The information on the package provides traceability information. Do not remove this part from its package (receipt inspection included) until ready for installation in the intended application. 9.0 CERTIFICATION TO ACCOMPANY SHIPMENT Pentair/Anderson Greenwood Crosby shall supply a certificate, which shall accompany the shipment of the item. This certificate shall be signed by Pentair/Anderson Greenwood Crosby s QA authority and shall certify that: The item has been manufactured/ dedicated in accordance with Pentair/Anderson Greenwood Crosby s QA manual that meets the requirements of 10CFR50 App B quality program and was reviewed/accepted by OPG to the requirements of CSA CAN3-Z , or CSA N QA Standards and, the requirements of the OPG P.O. The nuclear class 1, 2 or 3 components and materials have been manufactured in accordance with a QA program that meets the requirements of ASME, Section III subsection NCA 4000 (for components) or ASME, Section III subsection NCA 3800 (for materials). The item supplied by Pentair/Anderson Greenwood Crosby both meets the design requirements and it is equivalent to the item described in the primary controlled document(s) referenced in OPG P.O. item description (specifically referencing the document by number, revision, and/or date). Note: If there is a change or substitution of material, design, compounds used in construction, manufacturing method(s) or nondestructive method is proposed or identified, that change shall be reviewed/accepted by the OPG Engineer in writing prior to shipment.
