1 Beauty Pre-Labeling Guide DIRECT LABELS All Direct POs receive a 3x3 Label to be placed on the front center of a clear and sealable polybag o The polybag must be equal to or greater than the size of the 3x3 label In some cases, products packaged in individual boxes might be able to accommodate the 3 x 3 label on the center back of the box without requiring an additional polybag. Boxed items MUST be polybagged if the 3 x 3 will obscure ANY images or product information on a boxed item. All merchandise that is shipped in a polybag with an opening greater than 5 needs to have a warning label on the bag that states the following (or something similar): Warning: To avoid danger of suffocation, keep away from babies and children. Do not use in cribs, beds, carriages or play pens. THIS BAG IS NOT A TOY When ordering direct labels through Checkpoint or FineLine, they will print the 3x3 label for the product, as well as a job label next to it. Do not use the job label. 2 For tubular shaped products, such as shampoos and hairspray, the label must be placed with the barcode running lengthwise with the tube. Example

3 RETAIL LABELS Examples Retail orders receive a smaller label (less than 1x1) o This should be placed on the item itself over any existing UPC barcode o Do not place label on the bottom of the item or package o Make sure the label does not cover any important product markings

4 **FRAGILE MERCHANDISE** All ceramics, glassware, and other fragile items are required to be tested to ensure that the packaging will meet the drop test requirements (see chart below) without affecting the product. If the packaging is negatively affected, this indicates that there is not sufficient protection for the fragile goods and therefore, will result in a chargeback. Direct orders: Be sure to bubble wrap individual items before placing in in the polybag. The 3x3 direct label needs to be on the outside of the polybag. Do not place the direct label directly on the product. Please see below for examples of proper packaging. Retail orders: Be sure to be sure to bubble wrap individual items. You do not have to place the items in a polybag. The less than 1x1 retail label needs to be placed on the product itself. Do not place the retail label on the bubble wrap or polybag. Please reference page 3 for proper labeling placement. **Protective bags can be orders here:

5 IMPORTANT PRODUCT MARKING REQUIREMENTS Beauty Products All beauty products must following all labeling requirements under the FDA Food, Drug, and Cosmetic Act (FD&C Act) and the Fair Packaging and Labeling Act (FPLA). These laws and their related regulations are intended to protect consumers from health hazards and deceptive practices and to help consumers make informed decisions about the personal care products they purchase. Please see below requirements. 1. The principal display panel, i.e., the part of the label most likely displayed or examined under customary conditions of display for sale must state: a. Identity Statement- The common name of the product, in both English and French (common name such as Perfume, Body Lotion, Eye Shadow, etc). b. Net quantity in English and French (e.g. weight, volume, number of items) in both metric units of measurement and US units of measurement. 2. Information Panel a. The company name and business address b. Distributer Information c. Country of Origin d. Ingredients- declaration must be listed on the outer label (or, where there is only one label, on the label affixed directly to the product s container).ingredients must be declared in descending order of predominance. Ingredients must be listed only by their INCI names, in the absence of an INCI name you may also use the name given in the United States Pharmacopeia, National Formulary, Food Chemical Codex, USAN, and the USP dictionary of drug names e. Directions, warnings or cautions in English and French where necessary for safe use (if hazardous to consumers when misused) f. Batch number Additional Note: Material Safety Data Sheets are required for cosmetics that include alcohol or any other flammable materials within the ingredients. MSDS sheets must be submitted to your buyer/buying operations contact for review PRIOR to a PO being issued. These must be included with the shipping documents that are provided to the freight forwarder/courier service. *If you product is HAZARDOUS please refer to the Shipping of Hazardous Materials (ORM-D) section of the Vendor Guidelines for instructions on how cartons are to be packed and marked for acceptance into our distribution centers.
